An international randomised controlled trial of a novel antimicrobial dressing for peripheral intravenous catheters (ProP trial).

BACKGROUND: Peripheral intravenous catheters (PIVCs) are extensively used in hospitals worldwide and frequently associated with complications. Phlebitis is a common complication and may indicate infection, while infectious events, although uncommon, contribute to antimicrobial use and healthcare-associated infection. Chlorhexidine gluconate (CHG) impregnated dressings effectively prevent infections in central venous catheters but evidence for PIVC use is lacking. We assessed the feasibility and preliminary clinical outcomes of CHG-impregnated dressings for PIVCs. METHODS: The ProP trial was a…

[250 central venous silastic catheters in premature infants less than 1.500 g. A clinical study of technique and complications].

250 central venous catheters were inserted percutaneously in premature infants weighing less than 1,500 g in a prospective study over a period of five years. The mean catheter life was 25 days. We noted 47 complications (= 1/134 days). The most common complication was phlebitis (9.2%). The risk of phlebitis was highest after inserting catheters through the saphenous vein (60%) and lowest after inserting them through the basilic vein (3.9%). Septicaemia was diagnosed in 15 infants (6%). The incidence of thrombosis was 1.2%. Two infants had cardiac perforation with a resultant pericardial effusi…

Physicochemical Characteristics of Antimicrobials and Practical Recommendations for Intravenous Administration: A Systematic Review.

Most antimicrobial drugs need an intravenous (IV) administration to achieve maximum efficacy against target pathogens. IV administration is related to complications, such as tissue infiltration and thrombo-phlebitis. This systematic review aims to provide practical recommendations about diluent, pH, osmolarity, dosage, infusion rate, vesicant properties, and phlebitis rate of the most commonly used antimicrobial drugs evaluated in randomized controlled studies (RCT) till 31 March 2023. The authors searched for available IV antimicrobial drugs in RCT in PUBMED EMBASE®, EBSCO® CINAHL®, and the C…

[Pathogenesis, diagnosis and therapy of thrombophlebitis and varicophlebitis].

Thrombophlebitis is defined as thrombotic inflammation of a previously healthy superficial vein, varicophlebitis as that occurring in varicosities. The latter appears responsible for the majority of thrombotic venous occlusions. In contrast to venous thrombosis, the thrombotic involvement of deep veins, thrombophlebitis usually resolves without sequel and, in general, thrombophlebitis nor varicophlebitis are associated with the risk of pulmonary embolism. The clinical presentation of thrombophlebitis is that of a tender, hardened superficial vein which, in the presence of inflammation, may be …

Autoimmune pancreatitis metachronously associated with retroperitoneal fibrosis with IgG4-positive plasma cell infiltration.

Retroperitoneal fibrosis is an uncommon disorder characterized by the formation of a dense plaque of fibrous tissue in the retroperitoneum, and its etiology remains unknown. Autoimmune pancreatitis is a rare type of chronic pancreatitis characterized by fibrosis with abundant infiltration of IgG4-positive plasma cells and lymphocytes and obliterative phlebitis in the pancreas. We present a case of autoimmune pancreatitis that developed 10 mo after the occurrence of retroperitoneal fibrosis. Histological findings of the resected retroperitoneal mass were marked periureteral fibrosis with abunda…

[Venous involvement in the thoracic outlet syndrome (apropos of 30 cases)].

This paper present 30 patients with subclavian venous compression caused by a thoracic outlet syndrome. 3 stages are differentiated: intermittent compression: 15 patients; acute occlusion: 5 patients; chronic occlusion: 10 patients. Every stage corresponds with a clinic and radiologic unit: venous claudication of the upper limb; phlebitis; post-phlebitis syndrome. 8/30 patients had a bilateral compression. 22/30 patients had an associated arterial compression. The hypertrophy of the subclavian muscle is the main etiology of the venous compression. The venography dynamic of the upper limb is th…

An Overlapping Case of IgG4-related Disease and Klinefelter Syndrome with Lupus-like Serological and Neurological Features: A Case Report and Literature Review.

A 46-year-old man with Klinefelter syndrome (KS) presented with obliterative phlebitis of the lower legs with a deteriorated renal function, and elevated serum alkaline phosphatase and ataxia levels. Examinations demonstrated tubulointerstitial nephritis, obliterative phlebitis and lymphadenopathy with IgG4+ plasma cell infiltrate and sclerosing cholangitis. Although the serological profile and central nerve system involvement were compatible for systemic lupus erythematosus (SLE), a definite diagnosis of SLE was difficult to make. IgG4-related disease (IgG4-RD) with KS was finally diagnosed, …

Nursing Interventions to Minimize the Unnecessary Replacement of Peripheral Intravenous Catheters: A Systematic Review.

Background: Peripheral intravenous catheters (PIVCs) is widely considered the most common invasive procedure in hospitals, and it is estimated that these devices are used annually up to two billion times worldwide. The main complication of this procedure is the appearance of phlebitis, which can cause pain, edema, tenderness, erythema, or the development of a palpable cord around the cannulated vein. Premature failure of PIVCs increases the cost of health care. Objective: To describe interventions carried out by nursing professionals aimed at increasing the lifespan of PIVCs. Methods: A system…

Improved Patient Safety and Quality Outcomes With Amiodarone Infusions.

BACKGROUND: Amiodarone is a common intravenous medication and a known irritant to the vessel wall when administered peripherally. LOCAL PROBLEM: Nurses identified an increase in phlebitis associated with peripheral amiodarone leading to multiple catheter replacements and interruptions in drug therapy. Central venous access is recommended by the manufacturer but not practical for a short-term infusion based on the risk to the patient, time, and cost. METHODS: A 4-phased approach was used to identify a more suitable peripheral intravenous catheter. INTERVENTIONS: A collaborative effort betwee…

[The pampiniform plexus in the chronic phase of human Chagas disease: histologic assessment].

The occurrence of Trypanosoma cruzi intracellular clusters and phlebitis was searched for on pampiniform plexus vein walls of chronic chagasic patients. For this purpose, 23 pairs of spermatic cords, epididymides and testes (17 from chagasic patients and 6 from non-chagasic controls) were obtained, at autopsy. Trypanosoma cruzi was investigated by immuno-histochemistry on slides obtained from several sections of the gonads and vessels of each case. Only discrete and focal undetermined chronic phlebitis was observed, with no parasites, in 5 chagasics (bilateral in 3) and 2 controls (chi 2: p < …

Closing the gap in vascular access safety for children with cancer in LMICs: a quality improvement initiative evaluating a PICC care bundle in pediatric oncology.

BACKGROUND: Peripheral intravenous cannulas (PIVCs) are commonly utilized in pediatric oncology but are associated with a high incidence of phlebitis and recurrent venipuncture, particularly in children requiring prolonged therapy. Structured quality improvement (QI) strategies, such as standardized peripherally inserted central catheter (PICC) care bundles, have the potential to enhance vascular access safety, yet evidence from low- and middle-income countries (LMICs) remains limited. LOCAL PROBLEM: The baseline assessment of our pediatric oncology unit identified the absence of a standardiz…

[Chronic venous insufficiency: clinical aspects].

Manifestations of chronic venous insufficiency of the lower limbs are related to congenital or acquired stasis in the deep veins, to post-phlebitis occlusion of the deep veins or to stasis in the superficial veins. Functional impairment may be associated with varicose veins or not. Clinical signs include a heavy feeling in the legs, fatigue, and sometimes cramps or impatience in the evening. Sometimes there is a seasonal character to the complaints. The varicosities are localized in the territory of the medial saphenous vein, the lateral saphenous vein, or both and sometimes affect the pelvic …

Heparinization of alimentation solutions administered through peripheral veins in premature infants: a controlled study.

A randomized controlled study was done to determine whether the addition of heparin (1 U/mL) to peripheral intravenous alimentation solutions would affect the incidence of phlebitis and duration of patency of intravenous catheters in premature infants. Twenty-two-gauge Teflon catheters were uniformly used. One hundred five catheters infused with heparin were placed in 13 infants, and 122 catheters were placed in the control group of 13 infants. The time, nature, and incidence of complications were noted for each infusion site. Infusion of heparin was found to double the duration of patency of …

The benefits of central vein feeding and long term access to the circulation.

Although parenteral nutrition can be safely administered by peripheral vein for long periods most workers find it a difficult technique primarily because of phlebitis induced by the nutrient solutions. Additionally the need for immobilisation of an arm for at least part of the day restricts the patient's mobility. On the other hand, nutrition through a catheter inserted into a large vein, with its tip positioned in the superior vena cava, allows the infusion of large volumes of nutrients with only minimal risk of phlebitis. If the catheter is inserted through the subclavian or jugular veins th…
