Bipolar II and unipolar comorbidity in 153 outpatients with social phobia.

Previous studies on social phobia (SP) have focused largely on comorbidity between SP and major depression. Less attention has been devoted to the comorbidity between SP and bipolar disorder. In this retrospective study, we investigated family history, lifetime comorbidity, and demographic and clinical characteristics among 153 outpatients who met DSM-III-R diagnostic criteria for SP. Information regarding axis I diagnoses was obtained using the Structured Clinical Interview for DSM III-R (SCID-UP-R). Social phobic symptoms and the severity of the illness were assessed by the Liebowitz Social

Prevalence of mental disorders among detained asylum seekers in deportation arrest in Switzerland and validation of the Brief Jail Mental Health Screen BJMHS.

BACKGROUND: Though slowly growing, knowledge about prisoners detained for having violated an Alien Act is still marginal and most studies involve detained asylum seekers in the USA and Australia. Little is known about prevalence rates of mental health disorders in such a population. The Brief Jail Mental Health Screening BJMHS has been demonstrated in other prison populations as a valid screening for serious mental illness. AIM: The aims of this study were to describe prevalence rates for mental disorders according to ICD-10 and to validate the BJMHS for this population. METHODS: 80 inmates

[Premorbid period of anorexia nervosa].

Premorbid period of anorexia nervosa was studied in 96 female patients. In 64 cases (group 1) anorexia nervosa was as a separate disease and in 32 cases (group 2)--as a syndrome of endogenous mental disorder. In group 1 premorbid period was characterized by the features of neuropathy and residual organic disorders in combination with accumulation of subjects with personality disorders in the patient's family that promoted development of prolonged neurotic disorders. In group 2 there was hereditary psychosis load, high frequency of obstetrical and other perinatal complications, overvalued ideas

Symptoms of neuroses: profile patterns and factor structure of clinic attenders with non-psychotic functional psychiatric disorders.

Consecutively referred out-patients (n = 102) with non-psychotic functional psychiatric disorders were examined. Nine syndrome scores (panic attack, phobic anxiety, generalized anxiety, obsessive-compulsive, dissociative, conversion, somatoform, depersonalization, and depressive) showed patterns corresponding to ICD-10 diagnoses. However, some also showed moderate correlations between each other. Factor analysis revealed that the nine syndrome scores could be accounted for by three factors-anxiety and somatic, depression and obsessive-compulsive, and hysterical. These findings cast doubt on th

Gender differences in psychophysiological responses to speech stress among older social phobics:: congruence and incongruence between self-evaluative and cardiovascular reactions.

OBJECTIVE: Evidence suggests increased cardiovascular risk and autonomic impairment among individuals with chronic anxiety. Little attention, however, has been paid to the anxiety disorder of social phobia despite its high prevalence. Additionally, gender- and age-related cardiovascular profiles have not been examined in relation to social phobia. This study investigated cardiovascular responses to a socially threatening situation among older men and women with social phobia and control subjects. METHODS: Thirty subjects with social phobia and 30 control subjects (mean age = 65 years) were as

Specificity of interpretation and judgemental biases in social phobia versus depression.

OBJECTIVES: A body of studies shows that social phobia is characterized by content specific interpretation and judgmental biases. That is, they show bias in social situations but not in non-social situations. Comorbid depression, one of the major comorbid disorders in social phobia, might account for these biases in social phobia since depression also is characterized by cognitive distortions in social situations. This study hypothesized that, despite comorbid depression, patients with social phobia would suffer from contentspecific biases. DESIGN: Participants filled out the Interpretation a

Prevalence of anxiety disorders in patients with mitral valve prolapse.

Most previous research on mitral valve prolapse investigated its prevalence in patients whose primary diagnosis was one of the anxiety disorders. This study explored the inverse, i.e., the prevalence of anxiety disorders in patients manifesting mitral valve prolapse. There were no significant differences between patients (N = 48) and control subjects (N = 49) in panic disorder, phobic disorder, or generalized anxiety disorder or in the Zung depression score. The patient group scored significantly higher than control subjects on the Zung Anxiety Scale but significantly lower than Zung's patient

Virtual reality exposure in the treatment of social phobia.

Social phobia is one of the most frequent psychiatric disorders and is accessible to two forms of scientifically validated treatments: anti-depressant drugs and cognitive-behavioral therapies. Graded exposure to feared social situations (either in vivo or by imagining the situations) is fundamental to obtain an improvement of the anxious symptoms. Virtual reality (VR) may be an alternative to these standard exposure techniques and seems to bring significant advantages by allowing exposures to numerous and varied situations. Moreover studies have shown that human subjects are appropriately sens

Psychopathology among cocaine abusers entering treatment.

A number of different indicators of psychopathology were assessed in this study of 76 cocaine and crack abusers who entered outpatient treatment in New York City between June and December 1987. The majority (75%) had used cocaine for 4 years or more, and the majority (62%) spent over one thousand dollars a month on cocaine in the 6 months before entry into treatment. Forty-seven percent of the sample were found to be clinically depressed. Phobic disorders were the only other axis I diagnoses found in addition to depression, and all persons who were found to have phobic disorders also were diag

[Medicolegal assessment of post-traumatic vertigo].

Post-traumatic vertigo refers to a group of different disorders which occur following trauma, mainly closed head injury and whiplash injury of the cervical spine. Aside from headaches, vertigo is the most common symptom in this group of patients. In general, there are two main groups of patients with post-traumatic vertigo: those with documented vestibular dysfunctions and those without. The most common post-traumatic vestibular disorders are benign paroxysmal positioning vertigo, labyrinthine concussion, canal dehiscence and otolithic lesions. Some of these disorders are characterized by spon

Psychiatric comorbidity in primary care somatization disorder.

Lifetime prevalence rates were calculated for comorbid psychiatric disorders in 119 patients who were referred from primary care physicians for unexplained somatic complaints and who met DSM III-R criteria for somatization disorder. Comparisons were made with general population norms from the ECA study. Prevalence of nine comorbid conditions was significantly higher than in the general populations. The most prevalent comorbid diagnoses were major depression (54.6%), generalized anxiety disorder (33.6%), and phobic disorders (31.1%). The least common comorbid disorders were mania (4.2%) and dru

The causal role of self-awareness in blushing-anxious, socially-anxious and social phobics individuals.

This study examined the effect of attentional focus on social anxiety in three groups of subjects: high versus low blushing-anxious participants (n=48); high versus low socially anxious participants (n=60); and social phobic patients compared to patients with other anxiety disorders (n=48). Participants were asked to imagine two series of social situations, in which the hero was in the centre of others' attention. In the first series of stories, the type of feedback from the audience (positive, negative and neutral) and the direction of attention of the hero (self- versus task-focused) were ma

Complicated grief and its correlates in patients with bipolar disorder.

BACKGROUND: While the recent loss of a loved one has been identified as a risk factor for suicide in patients with bipolar disorder, and complicated grief (CG) has been associated with elevated rates of suicidality compared with loss without CG, little is known about the frequency or impact of CG in bipolar disorder. We investigated the frequency and implications of loss of loved ones in an ongoing study of bipolar disorder. METHOD: We conducted a survey of 120 patients with well-characterized DSM-IV bipolar disorder participating in Systematic Treatment Enhancement Program for Bipolar Disord

Patterns of mental disorders among the elderly residents of eastern Baltimore.

An extensive analysis of prevalence rates of cognitive impairment and other mental morbidities was carried out as part of a five-site national study on the health and mental health of an ambulatory population. This study reports on prevalence rates contrasted by age across the 18 and over population for cognitive impairment and other diagnoses in the Baltimore, Maryland, site of this study. Differences in prevalence rates by age are striking. Eight conditions have rates above 1 per cent among those 64 and younger: phobia (13.8 per cent), alcohol use disorder (6.5 per cent), obsessive compulsiv

Clinically significant avoidance of public transport following the London bombings: travel phobia or subthreshold posttraumatic stress disorder?

Following the London bombings of 7 July 2005 a "screen and treat" program was set up with the aim of providing rapid treatment for psychological responses in individuals directly affected. The present study found that 45% of the 596 respondents to the screening program reported phobic fear of public transport in a screening questionnaire. The screening program identified 255 bombing survivors who needed treatment for a psychological disorder. Of these, 20 (8%) suffered from clinically significant travel phobia. However, many of these individuals also reported symptoms of posttraumatic stress d

Infrequent and limited-symptom panic attacks.

Due to their prevalence and relationship to agoraphobia, panic attacks are receiving considerable attention. The DSM-III requires that they include at least four associated symptoms and occur at least weekly for a diagnosis of panic disorder. This study of panic symptoms and frequency in 68 panic sufferers addresses the validity of these DSM-III requirements in terms of symptom patterns and phobic avoidance. Using chi-square, Fisher, and t-test analyses, this study found no clinical distinction between frequent (occurring at least weekly) and infrequent panic attacks. When comparing limited-sy
