In this tribute article to D.G. Miller, we review some historical and recent contributions to understanding the myoelastic-aerodynamic (MEAD) theory of phonation and the related acoustic phenomena in subglottal and vocal tract. At the time of the formulation of MEAD by van den Berg in late 1950s, it was assumed that vocal fold oscillations are self-sustained thanks to increased subglottal pressure pushing the glottis to open and decreased subglottal pressure allowing the glottis to close. In vivo measurements of subglottal pressures during phonation invalidated these assumptions, however, and …

Phonation is controlled by complex synergism of muscles over the front neck region. Proper evaluation of the muscular activities in this region would not only help to estimate phonation function, but may also provide characteristics to diagnose dysphonia. While surface electromyography (sEMG) technique has been used to study the physiological aspects of phonation in previous studies, it remains unclear if the phonating function could be dynamically characterized by the sEMG signals of the neck muscles associated with phonation. In this study, almost 80 channels of high-density (HD) sEMG signal…

In an important paper on the physics of small amplitude oscillations, Titze showed that the essence of the vertical phase difference, which allows energy to be transferred from the flowing air to the motion of the vocal folds, could be captured in a surface wave model, and he derived a formula for the phonation threshold pressure with an explicit dependence on the geometrical and biomechanical properties of the vocal folds. The formula inspired a series of experiments [e.g., R. Chan and I. Titze, J. Acoust. Soc. Am 119, 2351-2362 (2006)]. Although the experiments support many aspects of Titze'…

Vocal fold geometry plays an important role in human phonation. The intraglottal quasi-steady pressure and velocity distributions depend upon the shape, size, and diameter of the glottis. This study reports the effects of the variation of glottal shapes on intraglottal pressures and velocities using a Plexiglas model with a glottis having nine symmetric glottal angles (uniform, as well as convergent and divergent 5 degrees, 10 degrees, 20 degrees and 40 degrees), while the minimal glottal diameter was held constant at 0.06 cm. The empirical data were supported by penalty finite element computa…

OBJECTIVES: Positive expiratory pressure (PEP) devices have become an additional therapeutic approach for treating voice disorders. Similar to water resistance therapy (WRT), phonation in a PEP device introduces a secondary source of vibration within the vocal tract. This investigation aimed to compare the effects of phonation using a PEP device and silicone tube phonation (STP) commonly used in WRT on the vocal mechanism during phonation.
METHODS: Three normophonic subjects participated in the study. High-speed videoendoscopy, pressure, airflow, electroglottography, and acoustic recordings w…

Blunt force trauma to the larynx, which may result from motor vehicle collisions, sports activities, etc., can cause significant damage, oft … degrades significantly. To address this need, the influence of inferior-superior VF displacement on phonation is investigated in 1.0mm increments using synthetic, self-oscillating VF models in a physiologically-representative facility. Acoustic (SPL, frequency, H1-H2, jitter, and shimmer), kinematic (amplitude and phase differences), and aerodynamic paramet …

Phonation type, a phonatory dimension ranging from hypofunctional/breathy to hyperfunctional/pressed, is important both from a clinical and acoustical point of view; hyperfunctional voice can lead to voice disorders and hypofunctional voice reduces text intelligibility. Five male singers sang diminuendo sequences of the syllable /pae/ and three of them also produced speech or singing samples with different phonation types which were rated for phonatory pressedness by a panel of voice experts. The sequences were analyzed by inverse filtering and the associated subglottal pressures were estimate…

BACKGROUND: Exercises involving tube phonation in water (TPW) have emerged as an alternative to conventional voice therapy techniques. The objective was to determine whether the results of these techniques were comparable to each other for the treatment of voice pathology, as well as to compare the costs implicated in the delivery of each treatment.
METHODS: A retrospective, descriptive, and observational study was performed that included all patients evaluated in our voice unit in 2015 who received vocal rehabilitation treatments. They were divided into two groups depending on the treatment …

OBJECTIVES: The present study aimed to investigate the contact quotient (CQ) values of breathy, normal, and pressed phonation types in four different sections of the female singing range.
METHODS: Electroglottography (EGG) and acoustic signals were recorded from 10 female singing teachers. Five vowels were sung for 1-3 seconds each, in three phonation types-normal, breathy, and pressed, in four pitches representing registration change points in the singing range. CQ values were automatically generated from the EGG signal using VoceVista at 35% threshold level. Sound pressure levels were check…

According to Titze's 1988 derivations for phonation threshold pressure (PTP), there are a number of important variables that PTP depends upon. A primary one is the vertical glottal duct length T: PTP decreases if T is increased. Changing the length of T, however, may have a significant effect on other variables that PTP depends upon. This study examined the effects of lengthening the vertical uniform glottal duct on the transglottal and intraglottal pressures, and the derived transglottal pressure coefficients, for five glottal diameters (0.01, 0.02, 0.04, 0.08, and 0.16 cm) and three transglo…

Although stiffness conditions in the multi-layered vocal folds are generally considered to have a large impact on voice production, their specific role in controlling vocal fold vibration and voice acoustics is unclear. Using a three-dimensional body-cover continuum model of phonation, this study shows that changes in vocal fold stiffness have a large effect on F0 and the means and amplitudes of the glottal area and flow rate. However, varying vocal fold stiffness, particularly along the anterior-posterior direction, has a much smaller effect on the closed quotient, vertical phase difference, …

Maintaining a small glottal opening across a large range of voice conditions is critical to normal voice production. This study investigated the effectiveness of vocal fold approximation and stiffening in regulating glottal opening and airflow during phonation, using a three-dimensional numerical model of phonation. The results showed that with increasing subglottal pressure the vocal folds were gradually pushed open, leading to increased mean glottal opening and flow rate. A small glottal opening and a mean glottal flow rate typical of human phonation can be maintained against increasing subg…

Vocal cord palsy (VCP) is common in otolaryngological clinics, as are postoperative patients with VCP at general and university hospitals. Although VCP treatment strategies have been discussed, decisions on treatment methods and timing are complicated by the spontaneous recovery exhibited by some patients. We studied clinical features of VCP patients and their histories. Subjects were 171 VCP patients seen at our phonation clinic during a 15-year period-18% of all phonation clinic cases. By gender, 69% were male and 31% were female, ranging in age from 4 to 89 (mean, 58) years. Some 18% were b…

We report a case of involuntary phonation caused by abnormal vocal cord movements during expiration in a patient with Parkinson's disease. A 60-year-old woman had been treated for parkinsonism at the outpatient clinic of the Department of Neurology since August 1999. She began to groan involuntarily in the daytime in September 2001. She could not eat well while groaning. Stridor was not noted during sleep at night. Endoscopic examination of the larynx revealed insufficient abduction of the bilateral vocal cords, although the glottis was not so small as to cause stridor during inspiration. Duri…
