A new furoquinolinone derivative, namely 4-hydroxymethyl-1,6,8-trimethylfuro[2,3-h]quinolin-2(1H)-one (HOFQ), was synthesized and its biological activity studied. By UVA activation, HOFQ induced strong antiproliferative effects in Ehrlich ascite cells, which lost their ability to transmit the tumor by transplantation. HOFQ exhibited poor genotoxicity and absence of skin phototoxicity. Actually, HOFQ sensitization forms DNA-protein cross-linkages but not interstrands cross-links. Therefore, HOFQ appears to be a new promising drug for PUVA photochemotherapy and photopheresis.

Mycosis fungoides (MF) is the most common type of primary cutaneous lymphomas. Several clinical variants of MF have been described. Purely, hypopigmented variant of MF (HMF) is rare. Sixty-nine patients with mycosis fungoides, plaque stage, were treated in an open study with photochemotherapy (PUVA) or the combination of oral retinoids and PUVA (RePUVA). The response rate of Re-PUVA was equal to that of PUVA, with complete remission in 73% and 72%, respectively. Remissions were obtained with fewer PUVA sessions, and with a lower UVA dosage, if PUVA was combined with retinoids. A lower UVA dosage was needed if treatment was given four times weekly in stead of twice weekly. The duration of the remissions tended to be prolonged if retinoids were given as maintenance therapy.

A 34-year-old woman with scleromyxoedema failed to respond to initial treatment with cyclophosphamide or psoralen-UVA photochemotherapy (PUVA). She developed neurological symptoms which resolved after five 4 l plasma exchanges performed on successive days, together with pulse methylprednisolone therapy. The combination of systemic interferon-alpha and systemic photochemotherapy is one of the most effective and most frequently administered treatment regimens for mycosis fungoides. Two patients with mycosis fungoides stage IIb with ulcerated tumors were treated with this regimen. While the plaques responded favorably to the combination therapy, the ulcerated tumors were quite resistant despite treatment for several weeks. Two patients with severe, disabling polymorphous light eruption, who were unable to tolerate photochemotherapy and who were unresponsive to alternative recognized therapies, are described. Photochemotherapy with psoralen and long-wave ultraviolet light, so-called PUVA-treatment, is currently being evaluated in many dermatologic departments. Side effects such as nausea, pruritus and erythema are well known. Recently the development of acneiform eruptions was reported in a British patient treated with PUVA (3). We found that 4 out of 80 patients treated in our clinic with 8-methoxy-psoralen according to the usual weight schedule (6) and long-wave ultraviolet irradiation developed perioral dermatitis, in 2 cases, together with acneiform eruptions localised to the forehead.

A 6-year follow-up of 12 patients with mycosis fungoides (MF) treated with psoralen photochemotherapy (PUVA) at the Cleveland Clinic revealed that 7 patients are clear of MF with 4 no longer receiving therapy and 3 on maintenance PUVA. Five patients died within 2 years of onset of therapy. Of 25 others with MF who were treated at Northwestern University, 20 are clear or partially clear after PUVA. Plaque-stage MF responds best to this type of treatment; Sézary syndrome, erythrodermic MF, and tumor-stage MF respond poorly to PUVA.

In the last five years we observed in Berlin seven cases of psoriasis vulgaris combined with bullous eruptions of the pemphigoid type. Exogenious factors provoking the bullous eruptions were sun exposure, photochemotherapy and treatment with dithranol. These factors did not cause contact allergy of type IV, but pemphigoid eruption which could be characterised by immunfluorescence. This, was accompanied in two cases with high numbers of eosinophils in tissue and blood.
