Pneumocystis carinii is a common opportunistic pathogen in patients infected with the human immunodeficiency virus (HIV). Pneumocystis carinii pneumonia is common, while extrapulmonary infections with Pneumocystis carinii have been reported sparingly. The clinical features are frequently nonspecific. The detection of Pneumocystis carinii in cerebrospinal fluid (CSF) has not been reported thus far. In this report, an unusual case of Pneumocystis carinii meningoradiculitis in an HIV-infected patient who had previously received primary prophylaxis with trimethoprim-sulfamethoxazole is presented.

BACKGROUND: The aim of the present was to study the clinical characteristics and prognostic factors of 20 cases of pneumonia by Pneumocystis carinii (NPC) diagnosed over the last 10 years in renal, renopancreatic, liver and bone marrow transplant recipients.
METHODS: The clinical histories of all the patients transplanted in the authors' hospital from 1985 to 1994 who developed NPC were analyzed. Diagnosis was established by visualization of cysts in methenamine silver staining of bronchialveolar lavage (BAL) samples.
RESULTS: The global incidence of NPC in our transplant population was 1%. …

During a period of 10 months, 87 children with Pneumocystis carinii pneumonia accompanied transient cellular immunity disorders and with normal humoral immunity were observed. It is suggested that change the invasiveness of the parasite has changed. The necessity of taking into consideration a pneumocystis etiology in diagnosis of respiratory tract diseases among children is pointed out.

Pneumocystis carinii is the opportunistic pathogen frequently causing pneumonitis in the acquired immunodeficiency syndrome. Extrapulmonic manifestation of P carinii is unusual and is commonly associated with severe systemic illness, other immune deficiency status, malignancy, or immune suppression. We describe a case of acquired immunodeficiency syndrome with manifestations of P carinii otitis media with severe otalgia and conductive hearing loss.

Oligonucleotide primers and probes were used in the polymerase chain reaction to amplify Pneumocystis carinii specific DNA sequences from alveolar lavage samples from 47 diagnostic bronchoscopies. No P carinii DNA was found in lavage from 10 immunocompetent patients; only low levels were found in 3 of 13 samples from immunosuppressed individuals without P carinii pneumonia (PCP), and the highest levels, readily demonstrated by simple ethidium bromide staining, were found in all of 16 samples from immunosuppressed patients with PCP confirmed by means of standard silver staining and in 4 from pa…

OBJECTIVE: To study the effect of pentamidine aerosol inhalation, known to be effective and well tolerated in the primary prevention of HIV-associated Pneumocystis carinii pneumonia (PCP), on the severity, on clinical and radiological findings and prognosis of break-through PCP.
PATIENTS AND METHODS: Data were obtained from the case notes of 17 men (mean age 36 [23-55] years) with HIV-associated PCP, treated between 1989 and 1994, who had received primary prophylaxis with pentamidine aerosol. During the same period 42 patients with HIV-associated PCP but no pentamidine prophylaxis (39 men, th…

To establish an experimental model of Pneumocystis jirovecii pneumonia (PJP) in the context of drug-induced immunosuppression, cohorts of macaques were chronically treated with dexamethasone and subsequently exposed to natural transmission of Pneumocystis through co-housing with other Pneumocystis colonized or infected 'seeder' macaques. Through flow cytometry, we observed that long-term dexamethasone treatment reproducibly reduced the frequency and cell numbers of CD4 T cells in the peripheral blood and bronchoalveolar lavage (BAL) in both Japanese (Macaca fuscata) and rhesus macaques (Macaca…

A 43-year-old male patient presented to the emergency department with progressive dyspnea. CT scan showed pronounced cystic lesions and ground glass opacitiy in both lungs and diagnosis of HIV infection was established. Bronchoscopy confirmed diagnosis of pneumocystis jirovecii pneumonia (PCP). The radiological presentation with perihilar large cysts is typical for PCP in HIV-infected patients, but rarely encountered today.

INTRODUCTION: Despite the dramatic decrease in opportunistic infections after the introduction of highly active antiretroviral therapy, Pneumocystis jirovecii pneumonia remains the major AIDS related infection in France.
BACKGROUND: The clinical, radiological and microbiological diagnosis is usually easily made in HIV-infected patients. The preferred treatment is high dose trimethoprim-sulfamethoxazole, in association with steroids in cases of severe hypoxaemia. This has led to a dramatic reduction in mortality in these patients. Prophylactic treatment is mandatory in highly immunosuppressed …
