Real world characteristics of central sleep apnea: a review of 29,000 consecutive patients undergoing polysomnography.

PURPOSE: The characteristics of central sleep apnea (CSA) in real-world sleep referral populations remain poorly understood. We used the electronic health record of a large health system to understand the prevalence and risk factors for CSA among patients referred for sleep evaluation. METHODS: We reviewed the records of 29,803 patients who underwent diagnostic polysomnography at six sleep laboratories over 14 years. A validated algorithm was used to extract polysomnographic results. CSA was defined as having an apnea hypopnea index (AHI) ≥ 5, a central apnea index ≥ 5, and a central apnea in…

In-Laboratory Polysomnography Worsens Obstructive Sleep Apnea by Changing Body Position Compared to Home Testing.

(1) Background: Home sleep apnea testing, known as polysomnography type 3 (PSG3), underestimates respiratory events in comparison with in-laboratory polysomnography type 1 (PSG1). Without head electrodes for scoring sleep and arousal, in a home environment, patients feel unfettered and move their bodies more naturally. Adopting a natural position may decrease obstructive sleep apnea (OSA) severity in PSG3, independently of missing hypopneas associated with arousals. (2) Methods: Patients with suspected OSA performed PSG1 and PSG3 in a randomized sequence. We performed an additional analysis, c…

Obstructive sleep apnoea syndrome: comparison between polysomnography and portable sleep monitoring based on jaw recordings.

INTRODUCTION: Obstructive sleep apnoea syndrome (OSAS) constitutes a new major public health problem because of its several pathophysiologic consequences such as cognitive disorders, excessive daytime sleepiness with risks of traffic accidents, cardiovascular implications, and decrease of quality of life. The necessity of a gold-standard polysomnography to ensure an accurate diagnosis implies an expensive, technical and time-consuming examination. Thus, it seems logical to develop new systems so as to diagnose SAS and to make it possible to detect apnoeas/hypopnoeas easily during sleep even at…

Effectiveness of Tongue-lip Adhesion for Obstructive Sleep Apnea in Infants With Robin Sequence Measured by Polysomnography.

OBJECTIVE: Tongue-lip adhesion (TLA) is commonly used to relieve obstructive sleep apnea (OSA) in infants with Robin sequence (RS), but few studies have evaluated its efficacy with objective measures. The purpose of this study was to measure TLA outcomes using polysomnography. Our hypothesis was that TLA relieves OSA in most infants. METHODS: This is a retrospective study of infants with RS who underwent TLA from 2011 to 2014 and had at least a postoperative polysomnogram. Predictor variables included demographic and birth characteristics, surgeon, syndromic diagnosis, GILLS score, preoperati…

Predictors of Obstructive Sleep Apnea on Polysomnography after a Technically Inadequate or Normal Home Sleep Test.

STUDY OBJECTIVES: Home sleep testing (HST) is an accepted alternative to polysomnography (PSG) for diagnosing obstructive sleep apnea (OSA) in high-risk populations. Clinical guidelines recommend PSG in cases where the HST is technically inadequate (TI) or fails to establish the diagnosis of OSA in patients with high pretest probability. This retrospective study evaluated predictors of OSA on PSG within patients who had a TI or normal HST. METHODS: Electronic medical records were reviewed on 1,157 patients referred for HST at our sleep center. Two hundred thirty-eight patients had a TI or nor…

Death during polysomnography of a patient with cheyne-stokes respiration, respiratory acidosis, and chronic heart failure.

A patient with chronic heart failure and chronic respiratory failure (CRF) underwent ambulatory polysomnography at home. She was found dead on the morning after the recording. The tracings confirmed severe sleep apnea syndrome. After 8 h of incessant Cheyne-Stokes respiration during sleep, respiratory arrest occurred, followed 7 min later by asystole. This report illustrates a case of respiratory drive failure during sleep as the mode of death in a patient with heart failure, sleep apnea syndrome, and CRF.

Does Measurement Site Matter? A Comparison Between Finger, Forehead, and Toe Pulse Oximetry Measurements in Polysomnography.

Accurate peripheral blood oxygen saturation (SpO2) measurements are critical for evaluating sleep patients, particularly those with frequent desaturations due to pathological breathing events. We compared synchronised SpO2 measurements obtained from three anatomical sites (finger, forehead and toe) in patients undergoing routine Type 1 polysomnography (PSG) to assess site-related variability. Continuous and simultaneous pulse oximetry recordings from 41 sleep patients were synchronised with PSG data and scored for SpO2 desaturations (≥ 3% and ≥ 10 s), signal dropouts and artefact occurrences. …
