Knee effusions, popliteal cysts, and synovial thickening: association with knee pain in osteoarthritis.

OBJECTIVE: To evaluate the association of effusions, popliteal cysts, and synovial thickening with knee symptoms in older persons with and without radiographic (XR) osteoarthritis (OA), using magnetic resonance imaging (MRI). METHODS: Subjects with and without knee symptoms were recruited from Veterans Affairs and community sources. All had weight-bearing knee radiographs. Subjects were divided into 3 groups: Knee pain/XROA group had knee symptoms and radiographic OA; No knee pain/XROA group had no knee symptoms and radiographic OA; and No knee pain/no XROA group had no knee symptoms and a no…

Nuclear magnetic resonance (NMR) tomographic imaging for popliteal cysts in rheumatoid arthritis.

The ability of nuclear magnetic resonance (NMR) tomographic imaging to show a change in proton spin-lattice relaxation time (T1 value) in the joints of patients with rheumatoid arthritis before and after treatment with intra-articular steroid has been assessed. Six patients with seropositive rheumatoid arthritis and clinical evidence of a popliteal cyst were examined by both NMR tomography and arthrography. In all cases the presence of active rheumatoid arthritis was shown by an increase in the T1 values of the synovium of the joints. After arthrography a consistent but small rise in T1 value …

[Subacute ischemia of the lower leg caused by a popliteal synovial cyst. Apropos of a case].

Based on the observation of a cyst compressing the popiteal artery, the authors discuss the characteristic clinical picture and radiology of this benign neoformation. They emphasize two possible causes: cystic degeneration of the arterial median or a cyst of articular derivation. Histological data from this observation supports the second hypothesis.

Medial Open-Wedge High Tibial Osteotomy with Partial Meniscectomy and Without Cyst Excision for Popliteal Cysts: A Case Series.

Introduction: Popliteal cysts (PCs) are occasionally accompanied by knee osteoarthritis (OA) and varus malalignment. However, whether concomitant arthroscopic excision of PCs with medial open-wedge high tibial osteotomy (MOWHTO) improves the osteoarthritic environment remains unclear. Therefore, this study assessed serial changes in C-size, medial meniscus extrusion (MME), and cartilage status for up to 2 years following an MOWHTO. Methods: This study retrospectively used serial magnetic resonance imaging (MRI) evaluations to assess 26 consecutive patients who underwent MOWHTO. Of the 26 patie…

Association between medial knee joint bone morphology and popliteal cyst: a retrospective MRI-based analysis.

OBJECTIVE: Baker's cyst, or popliteal cyst, is a common knee condition associated with osteoarthritis and rheumatoid arthritis. While previous studies have explored the cyst's relationship with soft tissue abnormalities, the connection between popliteal cysts and medial knee bone morphology remains unclear. This study aims to assess the association between anatomical bone structures of the medial knee joint and popliteal cyst presence using magnetic resonance imaging (MRI). METHODS: A retrospective analysis was conducted on 100 patients who underwent MRI for knee pain between June and Decembe…

Delayed Presentation of Popliteal Artery Laceration Mimicking Baker Cyst After Arthroscopic Meniscectomy: A Case Report.

CASE: We report a case of delayed presentation of popliteal artery laceration after arthroscopic meniscectomy. Three weeks after the procedure, the patient developed severe calf pain, swelling over the popliteal fossa, and foot drop, despite a normal initial postoperative period. Clinical examination and radiological investigations suggested a massive hematoma in the popliteal fossa. A decision was made to explore the popliteal artery and drain the hematoma after consultation with the vascular surgeon. A laceration was noted, and it was repaired. CONCLUSION: Orthopaedic surgeons should mainta

Arthroscopic treatment of popliteal cyst using a figure-of-four position and double posteromedial portals.

PURPOSE: The purpose of this study was to introduce a modified arthroscopic treatment technique for popliteal cyst and hypothesize that this modified technique would provide good clinical efficacy and low recurrence rate. METHODS: From January 2013 to January 2017, 34 patients with symptomatic popliteal cysts were treated with our technique. A figure-of-four position and double posteromedial portals were used to achieve adequate enlargement of the posteromedial valvular opening between the cyst and the joint cavity and complete excision of the cyst wall. MRI was used to detect the recurrence …

Double posteromedial portals versus single posteromedial portal for arthroscopic management of popliteal cysts.

BACKGROUND: As a common disease in orthopedic clinics, popliteal cysts often coexist with intra-articular lesions. Compared with traditional open surgery, arthroscopic treatment of popliteal cysts is less traumatic, and intra-articular lesions can be treated. The 'one-way valve' mechanism of the popliteal cyst can be removed by expanding the communication between the articular cavity and the cyst to avoid cyst recurrence. In terms of arthroscopic techniques, the comparison of clinical effects between the double posteromedial portal (DPP) and single posteromedial portal (SPP) has rarely been st…

Clinical outcome of arthroscopic management of popliteal cysts with or without additional posterior open cystectomy.

PURPOSE: The aim of this retrospective study was to compare the clinical outcomes of arthroscopic decompression of popliteal cysts and simultaneous management of intra-articular pathologies alone or in combination with posterior open cystectomy. MATERIAL AND METHODS: This study included 65 consecutive patients with the diagnosis of popliteal cysts. Of the patients 31 received arthroscopy alone (arthroscopic decompression of cysts and management of intra-articular pathologies, AA group) and 34 received arthroscopy combined with open cystectomy (AO group). At the last follow-up after a mean of …

Unusual presentation of popliteal soft tissue sarcoma: not every swelling in the knee is a Baker's cyst.

Popliteal soft tissue sarcoma is a rare tumour, but sometimes it does pose clinical, diagnostic and management challenges. We present here a case report which initially presented as Baker's cyst as it is one of the commonest cyst in this region. It posed a diagnostic challenge even with the help of new imaging modalities. But the patient presented herself to the hospital several times with new symptoms often pointing us towards common pathologies like Baker's cyst, deep-vein thrombosis and vascular lesions but diagnosis was established only after histology. Unfortunately, at this stage, diseas

Arthroscopic treatment of popliteal cyst and visualization of its cavity through the posterior portal of the knee.

In regard to arthroscopic treatment of popliteal cysts, we explored the refuted pathology for popliteal cysts proposed by others. Here we introduce an arthroscopic technique using posterior portals to treat a popliteal cyst based on our observation that the opening of the cyst in the joint is a slit-like structure in the posterior wall of the capsule. By disrupting this slit-like structure with our procedure, the popliteal cyst ceased to be palpable and was no longer symptomatic. This technique also provides excellent arthroscopic visualization of the cavity of the popliteal cyst through the k…

[Sub-adventitial arterial cysts of the popliteal artery. Results from the vascular group of AURC].

A retrospective multicentric study conducted by the vascular group of the AURC (University association of surgical research) included 36 sub-adventitial cysts of the popliteal artery. This is a rare cause of claudication in the young male and usually involves the adventitia of the arterial wall. There is usually only one localization. Echography of the cyst may wrongly suggest partial thrombosis of a popliteal aneurysm. Angiography and CT scan provide the correct diagnosis. The results of surgical treatment were analyzed after a follow-up ranging form 3 months to 20 years (mean 9 years). The r

[Calcified inclusions in a popliteal cyst as a rare cause of persistent knee pain and recurrent effusions].

Popliteal cysts may occur with diseases of the knee and may cause several complications. We report about a patient with a popliteal cyst, that contained calcified concrements after a femur fracture and a complex knee injury and that we found to be a rare cause of persisting knee pain and effusions. The etiology of the concrements will be discussed, that we think are more likely to be posttraumatic loose joint bodies than traumatically induced chondromatosis.

Effectiveness of Surgical Treatment with Complete Cyst Excision for Cystic Adventitial Disease of the Popliteal Artery.

BACKGROUND: Cystic adventitial disease is a rare, nonatherosclerotic disease that affects various arteries and veins, involving the formation of a mucinous cyst within the adventitia. The etiology of the cystic adventitial disease is currently unclear, with several hypotheses having been suggested. The purpose of this retrospective observational study was to evaluate the etiology of popliteal cystic adventitial disease based on imaging and surgical findings and to evaluate the efficacy of surgical treatment. METHODS: From April 2013 to January 2020, nine patients were diagnosed with the popli…
