A case of frog breathing.

Frog breathing (glossopharyngeal breathing) is a useful technique employed to increase ventilation when respiratory muscles are paralysed. It is a technique used by many patients with chronic poliomyelitis, yet many chest physicians and physiotherapists are unfamiliar with this breathing maneuver. Glossopharyngeal breathing coordinates movements of the tongue, cheeks and pharynx to force air from the mouth into the lungs. We report a case of glossopharyngeal breathing, demonstrating a 3 fold increase in vital capacity in a subject with chronic poliomyelitis.

Adult respiratory failure.

Pulmonary complications following thoracic surgery are common and associated with significant morbidity and mortality. In particular, acute respiratory distress syndrome (ARDS) can occur postoperatively or after trauma. This syndrome, when complicated by multisystem organ failure, often leads to a poor outcome. This article describes the etiology and pathophysiology of ARDS and evaluates recent advances in pharmacological and nonpharmacological therapies. In addition, newer modalities of mechanical ventilatory support are reviewed.

Inspiratory muscle resistive training in respiratory failure.

In this study, 4 patients who had failed weaning attempts using traditional methods underwent inspiratory muscle resistive training (IRT). Three patients showed improvements in inspiratory muscle strength (mean peak negative inspiratory pressure improved from 38 to 54 cm H2O) and in respiratory muscle endurance (mean PCO2 at the end of the same or a longer period of spontaneous breathing improved from 70 to 52 mmHg). These 3 patients were successfully weaned from mechanical ventilation after 10 to 24 days of IRT. A fourth patient showed no improvement and could not be weaned. This pilot study

Sleep Apnea and Cardiovascular Disease: Lessons From Recent Trials and Need for Team Science.

Emerging research highlights the complex interrelationships between sleep-disordered breathing and cardiovascular disease, presenting clinical and research opportunities as well as challenges. Patients presenting to cardiology clinics have a high prevalence of obstructive and central sleep apnea associated with Cheyne-Stokes respiration. Multiple mechanisms have been identified by which sleep disturbances adversely affect cardiovascular structure and function. Epidemiological research indicates that obstructive sleep apnea is associated with increases in the incidence and progression of corona

The effect of increased positive end expiratory pressure on brain tissue oxygenation and intracranial pressure in acute brain injury patients.

Cerebral hypoxia is an important cause of secondary brain injury. Improving systemic oxygenation may increase brain tissue oxygenation (PbtO2). The effects of increased positive end-expiratory pressure (PEEP) on PbtO2 and intracranial pressure (ICP) needs to be further elucidated. This is a single center retrospective cohort study (2016-2021) conducted in a 34-bed Department of Intensive Care unit. All patients with acute brain injury under mechanical ventilation who were monitored with intracranial pressure and brain tissue oxygenation (PbtO2) catheters and underwent at least one PEEP increme

Novel uses of noninvasive ventilation.

Noninvasive ventilation (NIV) and continuous positive airway pressure (CPAP) have been used in various unusual settings to assist breathing. NIV is now frequently used to treat exacerbations of chronic obstructive pulmonary disease and chronic respiratory failure in neuromuscular disease. This paper discusses CPAP and NIV for postoperative hypoxemia, preventing intubation in high-risk bronchoscopy, respiratory failure in pandemics, obesity hypoventilation syndrome, and respiratory support during percutaneous endoscopic gastrostomy tube placement.

[Noninvasive ventilation in medical intensive care medicine].

Noninvasive ventilation refers to mechanical respiratory assist not requiring an endotracheal airway. Potential advantages include the reduction of complications resulting from intubation and invasive long-term mechanical ventilation and possibly a reduced time demand on medical personnel in the long run. Patients with additional severe organ failure, lacking ability to cooperate or high risk of aspiration are not suitable for noninvasive ventilation. Noninvasive ventilation has proven to be effective in reducing symptoms, need for intubation and mortality in selected patients with exacerbatio

Termination of central sleep apnea episodes by upper airway stimulation using intermittent positive pressure ventilation.

A 52-year-old man presented with pure central sleep apnea syndrome that failed to respond to nasal continuous positive airway pressure (NCPAP) therapy. Intermittent positive pressure stimulation via a nasal mask was instituted using a portable positive pressure ventilator. We found marked reduction in the respiratory disturbance index, number of desaturations, and the number of arousals with this mode of therapy.

Noninvasive nasal mask-assisted ventilation in respiratory failure of Duchenne muscular dystrophy.

The effects of noninvasive nasal mask-assisted ventilation were studied in two patients with chronic respiratory failure due to Duchenne's muscular dystrophy. Observations were made with continuous recordings of transcutaneous CO2 and O2 and ear oximetry. In one case, the mean tcPCO2 fell from 72 mm Hg to 43 mm Hg. The tcPO2 increased from 38 mm Hg to 62 mm Hg without supplementary oxygen. In the second case, hypercapnia associated with supplementary oxygen was corrected, and at five months' follow-up, hypoxemia was corrected without supplementary oxygen. Prolonged therapy during sleep has res

Association of Positive Airway Pressure With Cardiovascular Events and Death in Adults With Sleep Apnea: A Systematic Review and Meta-analysis.

IMPORTANCE: Sleep apnea (obstructive and central) is associated with adverse cardiovascular risk factors and increased risks of cardiovascular disease. Positive airway pressure (PAP) provides symptomatic relief, whether delivered continuously (CPAP) or as adaptive servo-ventilation (ASV), but the associations with cardiovascular outcomes and death are unclear. OBJECTIVE: To assess the association of PAP vs control with cardiovascular events and death in patients with sleep apnea. DATA SOURCES AND STUDY SELECTION: MEDLINE, EMBASE, and the Cochrane Library were systematically searched from inc

Monitoring of oesophageal pressure.

PURPOSE OF REVIEW: Studies in patients with acute respiratory distress syndrome (ARDS) have been unable to demonstrate a survival advantage with higher levels of positive end-expiratory pressure (PEEP) to open atelectatic lung regions or prevent their cyclic collapse. This review will discuss the challenges of accurately measuring pleural pressure with balloon-tipped catheters in the oesophagus, and the utility of such pressure monitoring to set PEEP and assess lung mechanics, focusing on patients with ARDS. RECENT FINDINGS: Recent investigations have suggested that the monitoring of oesophag

Neonatal pneumopericardium: analysis of ventilatory variables.

Case reports of all infants admitted to the NICU from 1971 to 1982, who developed a pneumopericardium (PPC) while receiving intermittent positive-pressure ventilation, were reviewed in order to determine those variables of assisted ventilation associated with the occurrence of PPC. Of 19 infants seen during the 11-yr period, 16 had respiratory distress syndrome (RDS) and 3 had other forms of severe pulmonary disease. Mean birth weight was 1720 g, gestational age 32 wk, and age at occurrence 59 h. Peak inspiratory pressure (PIP) (mean 32 cm H2O), inspiratory time (IT) (0.74 sec), and mean airwa

Devising negative pressure within intercuff space reduces microaspiration.

BACKGROUND: Microaspiration past the tracheal tube cuffs causes ventilator-associated pneumonia. The objective of the current study was to evaluate whether creating negative pressure between the tracheal double cuffs could block the fluid passage past the tracheal tube cuffs. METHODS: A new negative pressure system was devised between the double cuffs through a suction hole in the intercuff space. Blue-dyed water was instilled above the cuff at negative suction pressures of - 54, - 68, - 82, - 95, - 109, - 122, and - 136 cmH2O, and the volume leaked was measured in an underlying water trap af
