Evaluation of current post-concussion protocols.

The growing number of concussions and mild traumatic brain injuries (mTBI) with the lack of evidence-based treatment options is a continuous health concern. This creates problems when evaluating and providing efficacious symptom management to patients suffering from post-concussion syndrome (PCS). Numerous pharmacological and non-pharmacological agents have been utilized in an attempt to treat PCS. Some of these approaches include physical therapy, analgesics, antidepressants, and nutraceuticals. Although these treatments have had some success, there has been inconsistent outcomes, with some e…

Supporting return to study and return to work in the patient with persistent post-concussion symptoms: A biopsychosocial approach.

BACKGROUND: Concussion, or mild uncomplicated traumatic brain injury (mTBI), is a common condition that can occur in sports, motor vehicle accidents, falls, assaults/violence and occupational settings. While symptom resolution is usually expected within 2-4 weeks, a minority of patients (up to 30%) experience persistent post-concussion symptoms (PPCS) that impact cognitive, physical and emotional functioning. These prolonged symptoms can interfere with a patient's ability to return to school or work, creating uncertainty for patients, employers and treating practitioners. OBJECTIVE: This arti

The Role of Psychology in Pediatric Concussion.

Postconcussive symptoms in children and adolescents may include cognitive, psychological, and behavioral changes. When symptoms become chronic they are often not able to be identified with standard medical evaluations. Physicians may find that these chronic symptoms are also resistant to traditional medical treatments. Postconcussive symptoms may be associated with the injury itself and/or secondary to the resulting psychological issues or stressors/changes following a concussion. It is important to conduct an extensive evaluation of psychological and nonorganic factors that may be contributin

Although some authors refuse to accept the reality of post-concussion syndrome (PCS) and confuse it with masked depression, somatic illnesses or post-traumatic stress, we have raised the question again of its existence, without denying the intricate links with other psychiatric or neurological disorders. Safe laparoscopic cholecystectomy in the presence of a cholecysto-enteric fistula.

BACKGROUND: After concussion many people have cervicogenic headache, visual dysfunction, and vestibular deficits that can be attributed to brain injury, cervical injury, or both. While clinical practice guidelines outline treatments to address the symptoms that arise from the multiple involved systems, no preferred treatment sequence for post-concussion syndrome has emerged. PURPOSE: This study sought to describe the clinical and patient-reported outcomes for people with post-concussion symptoms after a protocol sequenced to address cervical dysfunction and benign paroxysmal positional vertig…

MMPI-2 profiles 23 years after paediatric mild traumatic brain injury.

PRIMARY OBJECTIVE: Research suggest that post-concussive syndrome after mild traumatic brain injury (mTBI) is more common than chronic cognitive impairment. The aim of this study was to investigate very long-term outcome of subjective complaints after paediatric mTBI. RESEARCH DESIGN: The study was a follow-up 23 years after a prospective head injury study at a general hospital in Norway. METHODS AND PROCEDURES: Forty-one patients were assessed with the Minnesota Multiphasic Personality Inventory-2 (MMPI-2) 23 years after sustaining mTBI as children. MAIN OUTCOMES AND RESULTS: A good overal

Posttraumatic Stress, Illness Perception, Concussion Symptoms, and Quality of Life Following Mild Traumatic Brain Injury.

BACKGROUND: Working-age individuals with mild traumatic brain injury (mTBI) often experience persistent postconcussion symptoms (PCSs) that significantly impair their quality of life (QoL). Posttraumatic stress disorder (PTSD) may act as a psychological mechanism linking PCSs to a poorer QoL, whereas illness perception may influence the extent to which PCSs lead to PTSD; these relationships have not been tested. Establishing this moderated mediation pathway is critical for identifying patients at a heightened psychological risk and informing early intervention strategies to optimize postacute

Improved ventilatory response during exercise over time after concussion: A case report.

This case study reports the exercise ventilatory responses of a 17-year-old female who sustained a traumatic brain injury (TBI) which resulted in loss of consciousness. Subsequently, she suffered from post-concussion syndrome accompanied by orthostatic and exercise intolerance. A cardiopulmonary exercise test (CPET) was performed 2 years post-TBI. The results demonstrated significant hypoventilation with elevated PetCO2 and ventilatory equivalents, progressive desaturation, and pre-syncope symptoms limiting exercise capacity. A repeat CPET 4 years post-TBI demonstrated a similar exercise capac…

The association between sleep and physical activity with persisting postconcussion symptoms among adolescent athletes.

BACKGROUND: Both sleep duration and physical activity following concussion may influence subsequent recovery. Objective measurement of sleep and physical activity behavior via wearable technology may provide insights into their association with concussion recovery. OBJECTIVE: To determine whether sleep behavior (eg, duration, timing) and/or physical activity (steps/day, or exercise frequency, duration, intensity) in the first month after adolescent sports-related concussion are associated with developing persisting postconcussion symptoms (PPCS). DESIGN: Case-control. SETTING: Outpatient sp

Nutritional Supplement and Dietary Interventions as a Prophylaxis or Treatment of Sub-Concussive Repetitive Head Impact and Mild Traumatic Brain Injury: A Systematic Review.

Mild traumatic brain injury (mTBI) affects 42 to 56 million individuals worldwide annually. Even more individuals are affected by sub-concussive repetitive head impacts (SRHIs). Such injuries may result in significant acute and chronic symptoms. A study of how individuals may adjust or augment their nutritional and dietary habits to prevent cumulative neurotrauma and promote post-injury recovery is necessary. The objective of the current study is to systematically review nutritional and dietary interventions for neurotrauma prevention and mTBI recovery to direct clinical decision-making and id

Symptom Frequency and Persistence in the First Year after Traumatic Brain Injury: A TRACK-TBI Study.

Symptom endorsement after traumatic brain injury (TBI) is common acutely post-injury and is associated with other adverse outcomes. Prevalence of persistent symptoms has been debated, especially in mild TBI (mTBI). A cohort of participants ≥17 years with TBI (n = 2039), 257 orthopedic trauma controls (OTCs), and 300 friend controls (FCs) were enrolled in the TRACK-TBI study and evaluated at 2 weeks and 3, 6, and 12 months post-injury using the Rivermead Post-Concussion Symptoms Questionnaire (RPQ). TBI participants had significantly higher symptom burden than OTCs or FCs at all times, with ave

Post-Concussion Symptoms Rule: Derivation and Validation of a Clinical Decision Rule for Early Prediction of Persistent Symptoms after a Mild Traumatic Brain Injury.

Mild traumatic brain injury (mTBI) is a common problem. Depending on diagnostic criteria, 13 to 62% of those patients develop persistent post-concussion symptoms (PPCS). The main objective of this prospective multi-center study is to derive and validate a clinical decision rule (CDR) for the early prediction of PPCS. Patients aged ≥14 years were included if they presented to one of our seven participating emergency departments (EDs) within 24 h of an mTBI. Clinical data were collected in the ED, and symptom evolution was assessed at 7, 30 and 90 days post-injury using the Rivermead Post-Concus
