The use of buprenorphine to-go packs in the emergency department.

OBJECTIVE: Buprenorphine is an effective treatment for opioid use disorder (OUD). Patients in the emergency department (ED) can be initiated or continued on buprenorphine as a bridge to follow-up in the outpatient setting, but gaps in care may arise. The objective was to evaluate the impact of buprenorphine to-go packs as a continuing treatment option for patients presenting to the ED with OUD across a health system. METHODS: Adult patients discharged with a buprenorphine to-go pack from one of ten EDs within a major health system were included. The primary outcomes assessed within 30 days of

A multi-site, two-phase, Prescription Opioid Addiction Treatment Study (POATS): rationale, design, and methodology.

The National Institute on Drug Abuse Clinical Trials Network launched the Prescription Opioid Addiction Treatment Study (POATS) in response to rising rates of prescription opioid dependence and gaps in understanding the optimal course of treatment for this population. POATS employed a multi-site, two-phase adaptive, sequential treatment design to approximate clinical practice. The study took place at 10 community treatment programs around the United States. Participants included men and women age > or =18 who met Diagnostic and Statistical Manual, 4th Edition criteria for dependence upon presc

Opioid Use During the Six Months After an Emergency Department Visit for Acute Pain: A Prospective Cohort Study.

STUDY OBJECTIVE: Despite the frequent use of opioids to treat acute pain, the long-term risks and analgesic benefits of an opioid prescription for an individual emergency department (ED) patient with acute pain are still poorly understood and inadequately quantified. Our objective was to determine the frequency of recurrent or persistent opioid use during the 6 months after the ED visit METHODS: This was a prospective, observational cohort study of opioid-naive patients presenting to 2 EDs for acute pain who were prescribed an opioid at discharge. Patients were followed by telephone 6 months a

The impact of the Florida law HB21 on opioid prescribing patterns after spine surgery.

BACKGROUND: The opioid epidemic represents a major public health issue in the United States and has led to significant morbidity and mortality. On July 1 2018, Florida implemented state-law House Bill 21 (HB21), limiting opioid prescriptions to a 3-day supply for acute pain or 7 days if an exception is documented. The purpose of this study is to evaluate the effects of HB21 on opioid prescribing patterns after spine surgery. METHODS: Patients 18 years and older who underwent spine surgery between January 2017 and January 2021 were eligible for inclusion. Information including demographics, pi

Frequency of self-monitoring blood glucose and attainment of HbA1c target values.

AIMS: Test strips for self-monitoring of blood glucose (SMBG) represent in Argentina, around 50 % of diabetes treatment cost; the frequency of their use is closely associated with hyperglycemia treatment. However, the favorable impact of SMBG on attainment of HbA1c goal in different treatment conditions remains controversial. We therefore attempted to estimate the relationship between use of SMBG test strips and degree of attainment of metabolic control in an institution of our social security subsector (SSS) in which provision is fully covered and submitted to a regular audit system. METHODS

Substance use behavior among early-adolescent Asian American girls: the impact of psychological and family factors.

Confronting developmental tasks and challenges associated with bridging two different cultures, Asian American adolescent girls face increasing risks for substance use. Identifying risk and protective factors in this population is essential, particularly when those factors can inform preventive programs. Guided by family interaction theory, the present cross-sectional study explored the associations of psychological and familial factors with use of alcohol, prescription drugs, and other drugs among early-adolescent Asian American girls. Between August 2007 and March 2008, 135 pairs of Asian Am

Nonsurgical Providers Provide the Majority of Postoperative Opioid Prescriptions After Hand Surgery.

Introduction The increased use of Prescription Drug Monitoring Program (PDMP) websites has helped physicians to limit overlapping controlled substance prescriptions and help prevent opioid abuse. Many studies have investigated risk factors for prolonged opioid use after orthopedic surgery, but few studies have investigated who is prescribing opioids to postoperative patients. The purpose of this study is to investigate the types of medical providers prescribing opioids to hand surgery patients postoperatively. Methods Institutional Review Board approval was obtained prior to initiation of this

The Appropriate Use of Opioids in the Treatment of Refractory Restless Legs Syndrome.

Restless legs syndrome (RLS) is a distinct disorder, differing from chronic pain in many ways. Refractory RLS is characterized by unresponsi … with standard questionnaires, using an opioid contract, using urine drug screens, consulting state prescription drug monitoring programs, and frequent reevaluation of effectiveness and side effects. Opioid use in selected patients with refractory RLS may be life-transforming with favorable risk-benefit ratio.

[Implementation of Clinical Research Aimed at Antimicrobial Stewardship].

The implementation of antimicrobial stewardship (AS) is an important issue that pharmacists should promote through cooperation with physicians and other medical professionals. A core strategy for AS is "proactive intervention and feedback" on antimicrobial prescriptions. We evaluated the AS program to optimize antimicrobial chemotherapy in patients with positive blood cultures. For patients with bacteremia, the rate of bacterial disappearance significantly improved in cases in which the feedback based on the antimicrobial susceptibility testing results by the AS team was accepted than in cases

Could the Inflation Reduction Act Maximum Fair Price Hurt Patients?

Background: The Inflation Reduction Act's Medicare Drug Price Negotiation Program allows the federal government to negotiate caps for select medications. These price caps may reduce revenue for the pharmacy benefit managers (PBMs) that negotiate the actual price paid for medicines in the U.S. To offset the resulting pressure on their profit margins, it is possible that PBMs would, in turn, increase patients' out-of-pocket costs for medicines with capped prices. The model presented here evaluates how increased out-of-pocket costs for the anticoagulants apixaban (Eliquis) and rivaroxaban (Xarelt

Using PDMP data to enhance public health surveillance: Opioid and concurrent benzodiazepine use in a state Medicaid program.

Accurate opioid dispensing surveillance is critical for guiding public health interventions; however, claims data alone may underestimate opioid utilization and opioid-related quality measures. This study evaluated the impact of linking prescription drug monitoring program (PDMP) data with administrative claims on opioid prescription metrics, Concurrent Use of Opioids and Benzodiazepines (COB) quality measure reporting, and predictors of additional concurrent user identification. Trends in opioid prescription metrics were assessed using 2019-2022 PDMP-linked Mississippi (MS) Medicaid claims. A

Opioid prescribing patterns following implementation of Enhanced Recovery After Surgery (ERAS) protocol in pediatric patients undergoing lower tract urologic reconstruction.

BACKGROUND: With increasing awareness of the opioid epidemic, there is a push for providers to minimize opioid prescriptions. Enhanced Recovery After Surgery (ERAS) is a comprehensive multidisciplinary perioperative protocol that includes minimization of opioid analgesia in favor of non-opioid alternatives and regional analgesia. While ERAS protocols have consistently been shown to decrease inpatient opioid utilization, the impact on opioid prescribing practices and use after discharge in pediatric surgical patients is unclear. OBJECTIVE: This study aims to assess the impact of an ERAS protoc

Long-Term Opioid Therapy for Nonterminal Pain.

Chronic pain, defined as persistent pain lasting longer than 12 weeks, affects approximately 20% of US adults and often results in mental and social burdens. In general, nonopioid therapies are preferred for chronic nonterminal pain, and opioid therapy should not be considered a first-line treatment modality. Health disparities in pain management affect vulnerable populations, including racially and ethnically marginalized groups and those with cognitive impairment. Clinicians first must acknowledge the existing health inequities and the stigma surrounding chronic pain and then need to provide

Sociodemographic and prescribing characteristics that impact long-term retention in buprenorphine treatment for opioid use disorder among a statewide population.

BACKGROUND AND AIMS: Partial opioid agonist medications for opioid use disorder reduce mortality and morbidity, however long-term retention in treatment is challenging. The objective of this study was to identify patient and prescription characteristics associated with long-term buprenorphine treatment retention. METHODS: We used data from the Rhode Island prescription drug monitoring program to identify residents who initiated buprenorphine treatment and determine if they were retained in long-term buprenorphine treatment 12-months after treatment initiation.
