Addressing disparities in oral disease in Aboriginal people in Victoria: where to focus preventive programs.

The aim of this study is to determine where Aboriginal people living in Victoria attend public oral health services; whether they access Aboriginal-specific or mainstream services; and the gap between dental caries (tooth decay) experience in Aboriginal and non-Aboriginal people. Analysis was undertaken on routinely collected clinical data for Aboriginal patients attending Victorian public oral health services and the distribution of Aboriginal population across Victoria. Approximately 27% of Aboriginal people attended public oral health services in Victoria across a 2-year period, with approx

Informing comprehensive HIV prevention: a situational analysis of the HIV prevention and care context, North West Province South Africa.

OBJECTIVE: Building a successful combination prevention program requires understanding the community's local epidemiological profile, the social community norms that shape vulnerability to HIV and access to care, and the available community resources. We carried out a situational analysis in order to shape a comprehensive HIV prevention program that address local barriers to care at multiple contextual levels in the North West Province of South Africa. METHOD: The situational analysis was conducted in two sub-districts in 2012 and guided by an adaptation of WHO's Strategic Approach, a predomi

Transgender Youth Experiences and Perspectives Related to HIV Preventive Services.

BACKGROUND: In the United States, transgender youth are at especially high risk for HIV infection. Literature regarding HIV prevention strategies for this vulnerable, often-hidden population is scant. Before effective, population-based HIV prevention strategies may be adequately developed, it is necessary to first enhance the contextual understanding of transgender youth HIV risk and experiences with HIV preventive services. METHODS: Two 3-day, online, asynchronous focus groups were conducted with transgender youth from across the United States to better understand participant HIV risk and ex

The checkerboard area health system: delivering comprehensive care in a remote region of New Mexico.

A comprehensive health center is defined as the synergistic coupling of a medical and administrative structure designed to provide inpatient, outpatient, and public health services. While health centers have been widely established in other countries, only limited implementation has occurred in rural areas of the United States. The successful implementation of the health center concept in a sparsely populated area of northwestern New Mexico, which is predominately inhabited by Navajo Indians and Spanish Americans, is descriptively analyzed. The physical environment and the socioeconomic charac

[The importance of systematic examination of preschool children in new organization of primary health care in B&H and RS].

A systematic examination of 391 children at school age was performed, at age from 1st to 4th grade of grammar school, among which there was 183 boys and 208 girls. Examination was done by the responsible pediatric service of the Primary health care Center Sokolac with help of the humanitarian organization World Vision from Taiwan. The goal of the examination was to get an insight in the general health state of this population, including the risk factor for development of cardiovascular disease (CVD), to suggest appropriate preventive measures, and determine further justification of these exami

Delivering interventions to reduce the global burden of stillbirths: improving service supply and community demand.

BACKGROUND: Although a number of antenatal and intrapartum interventions have shown some evidence of impact on stillbirth incidence, much confusion surrounds ideal strategies for delivering these interventions within health systems, particularly in low-/middle-income countries where 98% of the world's stillbirths occur. Improving the uptake of quality antenatal and intrapartum care is critical for evidence-based interventions to generate an impact at the population level. This concluding paper of a series of papers reviewing the evidence for stillbirth interventions examines the evidence for c

The importance of preventive health care in post-menopausal women.

Women make up 55% of the total world population. This percentage is set to steadily increase over the next three decades. Europe also has the highest proportion of older women in the world. In fact, there are now approximately 3 women for every 2 men over 65. All of this data confirms the importance of prevention. An ideal approach for the female post-menopausal population would be treatment of any condition that can improve physical, mental and social well-being. Nevertheless, it is understood that the efficacy and cost/benefits of every screening programme need to be analysed. One of the lar

Reducing Preventive-to-E/M Code Conversion Using a Diagnosis-Triggered Documentation Template.

BACKGROUND: Preventive visits may be converted to problem-oriented evaluation and management (E/M) services when documentation does not clearly support preventive service criteria, resulting in lost reimbursement despite appropriate care delivery. The objective of this study was to evaluate whether a diagnosis-triggered documentation template embedded within the electronic health record reduces conversion of preventive visit codes to problem-oriented E/M codes. METHODS: We conducted a 4-month observational evaluation in an academic family medicine clinic. A SmartText-based documentation templ

Problems in the control of genetic disorders.

Preventive genetics services based on population screening are now an integral part of maternal and child health programmes. New developments in DNA technology, ultrasound scanning, and assay of factors in maternal blood are greatly increasing their potential to improve human health. If these services are to be delivered fairly to populations, there must be much more emphasis on community information, professional education, and service monitoring.

Unequal Gains? A Literature Review on the Affordable Care Act's Effects on Healthcare Utilization Across Racial and Ethnic Groups.

The Affordable Care Act (ACA), implemented in 2010, aimed to expand healthcare access, reduce costs, and address long-standing disparities in the U.S. healthcare system, particularly among racial and ethnic minorities. This paper reviews the ACA's impact on healthcare utilization for these populations, with a focus on insurance coverage, preventive services, and health outcomes. While Medicaid expansion significantly reduced uninsured rates and increased access to care in states that adopted the expansion, millions of low-income individuals, many of whom are racial and ethnic minorities, remai

Chlamydia screening for sexually active young women under the Affordable Care Act: new opportunities and lingering barriers.

The Affordable Care Act of 2010 (ACA) contains a provision requiring private insurers issuing or renewing plans on or after September 23, 2010, to provide, without cost sharing, preventive services recommended by US Preventive Services Task Force (grades A and B), among other recommending bodies. As a grade A recommendation, chlamydia screening for sexually active young women 24 years and younger and older women at risk for chlamydia falls under this requirement. This article examines the potential effect on chlamydia screening among this population across private and public health plans and i

Secondary analysis of primary and preventive services accessed and perceived service barriers by children with developmental disabilities and their families.

Children and youths with developmental disabilities (DD) have needs for more community-based services to address ongoing health, educational, employment, housing, transportation, and recreational concerns. Secondary data analysis was conducted to examine the array of services accessed and service obstacles to primary, preventive and special health care services of 102 children and youths with developmental disabilities. The two services most frequently reported as being used by families were SSI (29.4%) and MediCal (California's Medicaid program) (27.8%). Speech therapy was the most frequently

Preventive Care and Chronic Disease Management Comparison of Appalachian and Non-Appalachian Community Health Centers in the United States.

INTRODUCTION: The Appalachian region is often characterized by poor health outcomes and economic depression. Health centers (HCs) are community-based and patient-directed organizations that deliver comprehensive, culturally competent, high-quality primary healthcare services in high need areas, including Appalachia, where economic, geographic, or cultural factors can hinder access to healthcare services. PURPOSE: The study compares the clinical quality performance in preventive care and chronic disease management between Appalachian HCs and their non-Appalachian counterparts. METHODS: Using

Chart documentation of clinical preventive services at 9 Texas clinics.

Documentation of clinical preventive services at 9 Texas family practice residency programs, community health centers, and public health regional clinics was examined. Assessment of 11 risks, counseling on those risks, and timeliness of 8 screening tests and immunizations were abstracted. Documentation of clinicians' risk assessment focused on tobacco use (56.4%), alcohol/drug abuse (45.8%), and excess weight (21.6%). Counseling was documented most often for nutrition (20.5%), family planning (11.6%), and physical activity (10.3%). Of the sites studied, public health regional clinics had the h

Integrating Permanent Dental Hygienists in Saudi Preschools and Primary Schools to Address the High Burden of Childhood Dental Caries.

BACKGROUND: Pain from dental caries is strongly linked to increased school absenteeism and poorer academic performance. This burden is pronounced in Saudi Arabia, where the prevalence of caries is double the global rate despite the availability of public dental services and various national oral health outreach programs, suggesting potential gaps in access to dental care. CONTRIBUTIONS TO THEORY: While the public health benefits of school-based oral health programs are well-documented, existing school-based caries prevention programs in Saudi Arabia are limited, episodic, and lack continuity,

The U.S. Preventive Services Task Force: putting evidence-based medicine to work.

The U.S. Preventive Services Task Force is a prominent example of evidence-based medicine put to work. The Guide to Clinical Preventive Services and its Canadian counterpart distill thousands of articles into easily understood guidelines for the busy clinician. Rather than dictating practice, it provides the scientific base for prevention services. The second fundamental part of evidence-based medicine is the filtering of that evidence through the clinical skill of the clinician. Only the clinician can judge the applicability of the evidence and the preferences of the woman and her family. Som

A little planning goes a long way: multilevel allocation of HIV prevention resources.

BACKGROUND: HIV prevention funds are often allocated by decision makers at multiple levels. High-level decision makers may allocate funds to regions, and regional decision makers then allocate those funds to specific programs. Often, funds are allocated proportionally (e.g., in proportion to HIV incidence) rather than efficiently (i.e., to maximize HIV infections averted). The authors investigate the impact of efficient and proportional allocation methods at 2 different decision levels. METHODS: The authors developed an optimization model of resource allocation at 2 levels-an aggregate upper

Co-development of implementation strategies to assist staff of a mental health community managed organisation provide preventive care for health behaviours.

ISSUE ADDRESSED: People with a mental health condition are at risk of developing chronic physical disease due to smoking tobacco, inadequate nutrition, high alcohol consumption, low physical activity and poor sleep (SNAPS). Community managed organisations (CMOs) represent an opportune setting to support mental health consumers to improve their health behaviours through providing preventive care. Reporting of methods used to co-develop implementation strategies to assist CMO staff to deliver preventive care for SNAPS are scarce yet warranted. OBJECTIVES: This study aims to: (1) describe a co-d
