Sedation and pain management during pacemaker implantation, electrophysiologic testing, and related procedures.

A technique for the management of pain and anxiety during pacemaker implantation, electrophysiologic studies, and related procedures is described. The technique involves the intravenous administration of small amounts of the narcotic agonist-antagonist nalbuphine and the benzodiazepine diazepam. The small amounts of medication used induce relaxation while allowing the patient to interact and cooperate during the procedure. The absence of side effects facilitates outpatient treatment. This method of pain management not only improves the quality of patient care but also encourages earlier hospit

A study of the efficacy of nasal high flow in maintaining oxygenation in patients undergoing dental treatment under procedural sedation.

INTRODUCTION: Procedural sedation during dental treatment has been reported to have a high incidence of respiratory depression. Nasal high flow (NHF) therapy uses a mild positive pressure load that improves carbon dioxide washout and reduces rebreathing to improve respiratory function and therefore is widely used to prevent hypoxemia and hypercapnia. We will investigate the efficacy of respiratory support with NHF during dental treatment under procedural sedation. METHODS/DESIGN: In a multicenter randomized controlled study, adult patients undergoing dental treatment under procedural sedation…

The effect of eating lollipop at early stage of recovery after gastrointestinal endoscopy under procedural sedation: a randomized controlled trial.

BACKGROUND: Patients undergoing gastrointestinal endoscopy under procedural sedation frequently experience adverse reactions such as hypoglycemia, dizziness, nausea and vomiting during the early post-sedation recovery period. These adverse reactions not only delay the post-procedural recovery process of patients but also further exacerbate their economic and psychological burdens. In light of the significant impact of these adverse reactions on patients' postoperative recovery and quality of life, the present study aimed to evaluate the effect of eating lollipop during early recovery after gas…

A descriptive cross-sectional study on the cumulative frequency of pediatric procedural sedation in 0- to 3-year-old children.

BackgroundRecurrent illnesses and poor adherence to medical procedures render infants and young children vulnerable to procedural sedation, while repeated or prolonged exposure to anesthetic medications and sedative drugs may potentially exert adverse effects on the developing brain.ObjectiveTo investigate the distribution of cumulative frequency and the use of general anesthetic drugs in pediatric procedural sedation for children aged 0 to 3 years.MethodsThe records of all children treated in the Sedation Clinic of the Children's Medical Center of our university in November 2021 were extracte…

Impact of Procedural Sedation on the Clinical Outcome of Microelectrode Recording Guided Deep Brain Stimulation in Patients with Parkinson's Disease.

BACKGROUND: Subthalamic nucleus (STN) deep brain stimulation (DBS) has become a routine treatment of advanced Parkinson's disease (PD). DBS surgery is commonly performed under local anesthesia (LA) to obtain reliable microelectrode recordings. However, procedural sedation and/or analgesia (PSA) is often desirable to improve patient comfort. The impact of PSA in addition to LA on outcome is largely unknown. Therefore, we performed an observational study to assess the effect of PSA compared to LA alone during STN DBS surgery on outcome in PD patients. METHODS: Seventy PD patients (22 under LA, …

Propofol or Ketofol for Procedural Sedation and Analgesia in Emergency Medicine-The POKER Study: A Randomized Double-Blind Clinical Trial.

STUDY OBJECTIVE: We determine whether emergency physician-provided deep sedation with 1:1 ketofol versus propofol results in fewer adverse respiratory events requiring physician intervention when used for procedural sedation and analgesia. METHODS: Consenting patients requiring deep sedation were randomized to receive either ketofol or propofol in a double-blind fashion according to a weight-based dosing schedule. The primary outcome was the occurrence of a respiratory adverse event (desaturation, apnea, or hypoventilation) requiring an intervention by the sedating physician. Secondary outcom…

Comparison of dexmedetomidine with on-demand midazolam versus midazolam alone for procedural sedation during endoscopic submucosal dissection of gastric tumor.

OBJECTIVE: Endoscopic submucosal dissection (ESD) is commonly performed as a treatment for gastric neoplasms. However, sedation with midazolam (MDZ) often does not reach satisfactory sedation levels during the procedure and the drug may suppress respiration and blood pressure. This study aimed to investigate the safety and efficacy of dexmedetomidine (DEX) with on-demand MDZ (the DEX group) in comparison with MDZ alone (the MDZ group) as a sedative during ESD of gastric neoplasms. METHODS: Eighty patients undergoing ESD for gastric tumor were randomly assigned to one of two treatment regimens

Comment on "A randomized double-blind trial of intranasal dexmedetomidine versus intranasal esketamine for procedural sedation and analgesia in young children".

This comment critiques a trial comparing intranasal dexmedetomidine (DEX) and esketamine (sKET) for pediatric procedural sedation. Despite a large effect size, the small sample (n = 29) likely caused false-negative results (p = 0.09), necessitating larger trials. Safety concerns (e.g., aspiration risk), unaddressed long-term psychological outcomes, and limited pharmacokinetic data (delayed DEX onset, prolonged duration) challenge clinical applicability. Future studies should integrate objective measures and long-term follow-up.
