Primary lung carcinoma in cystic fibrosis: A case report and literature review.

Patients with cystic fibrosis (CF) have increased life span because of improved care over last 50 years. With increasing survival, predisposition of cancer may become evident. We have observed increase risk of gastrointestinal tract cancer, testicular cancer and lymphoid leukemia. Lung cancer in patients with CF is rare. Our patient developed chronic respiratory failure due to CF related bronchiectasis. Patient had progressive disease despite optimum treatment, requiring lung transplantation. Pathology of explant lung showed focus of invasive adenocarcinoma of lung origin. Patient had no evide

Organ donation after cardiac death from withdrawal of life support in patients with amyotrophic lateral sclerosis.

OBJECTIVE: Donation after cardiac death (DCD) or donation of organs after removal of life support is an accepted means of organ retrieval that usually occurs in the setting of sudden illness but has not been described in people with progressive neurologic illness. We report DCD in two people with progressive amyotrophic lateral sclerosis (ALS). METHODS: Case series at an academic medical center of two men with progressive ALS who underwent withdrawal of artificial life support, rapid cardiac death, and subsequent organ donation. The primary outcome was donation of organs in concordance with p

How patient compliance to suggested oral hygiene and maintenance affect periodontal therapy.

1. In medicine and dentistry, compliance tends to be poor in patients who have chronic diseases that they perceive as nonthreatening. 2. Even in the most optimistic studies, the rate of complete compliance with suggested toothbrushing is less than 50%; other studies show it much lower. In studies focusing on the use of interproximal cleaning aids, compliance is poorer still. 3. Patients in university-based programs have had a dropout rate (noncompliance) of 11% to 45%. In private periodontal practices, complete compliance was seen in one third or less of the patients. 4. The nature and the rat

Review of educational interventions for patients requiring palliative care: A scoping review.

BACKGROUND: Palliative care aims to improve the quality of life of individuals with serious, progressive illnesses. Therapeutic patient education is a key approach for enhancing patient autonomy and empowerment. However, structured educational interventions for patients receiving palliative care remain poorly documented in the literature, in contrast to the growing body of evidence regarding education for caregivers and healthcare professionals. OBJECTIVES: This scoping review aims to identify and characterize therapeutic patient education interventions implemented in palliative care, and to

Severe Viper Envenomation: A Case Report From Portugal.

Snakebite envenomation is uncommon in Portugal but may lead to significant morbidity when systemic toxicity develops. We report a case of a 57-year-old woman who presented with hypotension, progressive limb edema, and venom-induced coagulopathy following a viper bite. The progression of edema, together with systemic toxicity, prompted early antivenom administration and transfer to an intermediate care unit, with a second dose required in response to persistent edema. Supportive management included fluid resuscitation, limb elevation, and empirical antibiotic therapy for persistent fever. The p

Anesthetic management for cesarean delivery in a Guillain-Barré syndrome patient -A case report-.

Guillain-Barré syndrome is an acute inflammatory demyelinating polyradiculopathy characterized by progressive motor weakness, areflexia, and ascending paralysis. Guillain-Barré syndrome is extremely rare in pregnant patients, and there are no established guidelines for delivery or safest anesthetic methods. We report a Cesarean delivery in the case of a 32-year old woman who was diagnosed with Guillain-Barré syndrome 18 weeks into gestation. Tracheostomy was performed due to progressive respiratory muscle weakness and respiratory failure, and ventilator support was required in the intensive ca

Metastatic prostatic carcinoma presenting as fulminant hepatic failure.

A 68-year-old male presented with progressive abdominal pain, dyspnea, weight loss, and dysuria. Lab work revealed elevated creatine phosphokinase levels, prostate-specific antigen level (approximately 60 ng/mL), and elevated liver enzymes. He was admitted to the intensive care unit for worsening respiratory distress and confusion. He continued to deteriorate, and his bilirubin peaked at 8.5 mg/dL. The patient subsequently died, and an autopsy revealed extensive hepatic necrosis with diffuse intravascular and intraparenchymal permeation of metastatic prostatic carcinoma. Fulminant hepatic fail

Person-centred health outcomes in the routine care for people with progressive pulmonary fibrosis: the COCOS-IPF project's European survey on healthcare professionals' views and practices.

BACKGROUND: Idiopathic pulmonary fibrosis (IPF) and progressive pulmonary fibrosis (PPF) are interstitial lung diseases (ILD) that carry a high burden and mortality. IPF/PPF experts and patients call for standardized care, outcome harmonization and holistic management in these complex and devastating diseases, with a focus on person-centeredness. In this cross-sectional international survey study, we aimed to gather information on the person-centred health outcomes European healthcare professionals (HCPs) already use or deem important for use in routine care for IPF/PPF. This work is part of t

Sixteen-year changes and stable remission among treated and untreated individuals with alcohol use disorders.

AIMS: This study examined changes over a 16-year interval and predictors of stable remission among previously untreated individuals with alcohol-use disorders who did not obtain help or who entered either alcoholics anonymous (AA) or professional treatment in the first year after initially seeking help. DESIGN AND MEASURES: A sample of individuals (N = 461) who initiated help-seeking was surveyed at baseline and 1, 3, 8, and 16 years later. In addition to providing information on life history of drinking at each contact point, participants described their current alcohol-related and psychosoc

Antibody Positive Miller-Fisher Syndrome and Acute Motor Sensory Axonal Neuropathy With Respiratory Failure: A Rare Overlap.

Miller-Fisher syndrome (MFS), a rare variant of Guillain-Barré syndrome (GBS), is classically defined by ophthalmoplegia, ataxia, and areflexia and is strongly associated with anti-GQ1b antibodies. We report a 35-year-old man presenting with classical MFS features who rapidly deteriorated with progressive limb weakness and respiratory failure requiring mechanical ventilation. Cerebrospinal fluid analysis showed albuminocytologic dissociation, while nerve conduction studies were consistent with an axonal process. Serologic testing revealed positivity for both anti-GQ1b and anti-GD1a antibodies,

Managing the diagnostic challenge: chronic inflammatory demyelinating polyneuropathy during pregnancy.

Chronic inflammatory demyelinating polyneuropathy (CIDP) during pregnancy presents diagnostic and management challenges. We report a case of a primigravida in her 30s, exhibiting progressive quadriparesis starting in the second trimester. Initially, her symptoms of weakness, numbness and progressive quadriparesis were attributed to vitamin B12 deficiency, leading to the administration of intramuscular methylcobalamin injection. After referral to our tertiary care centre at 37 weeks of pregnancy and after further evaluation, neurological examination revealed findings characteristic of CIDP, whi

Fatal case of cryptogenic organizing pneumonia associated with everolimus.

Noninfectious pneumonitis (NIP) has been reported with everolimus; however, the majority of the reported cases were mild to moderate. We report a fatal case of cryptogenic organizing pneumonia (COP) in a 61-year-old man. About 4 weeks after starting everolimus, the patient was admitted to the hospital with complaints of a 1-week history of progressive dyspnea with exertion and cough. The chest radiograph showed bilateral multifocal dense opacities, and he was started on antibiotics. However, his respiratory status deteriorated, and he was subsequently intubated and transferred to the intensive

Fulminant Powassan Virus Encephalitis Presenting as Acute Ischemic Stroke: A Case Report.

Powassan virus (POWV) is a rare tick-borne flavivirus that can cause severe neuroinvasive disease with high morbidity and mortality. Early clinical and radiographic features are often nonspecific and may mimic acute ischemic stroke, leading to diagnostic delay. We report the case of a 77-year-old woman with recent tick exposure who presented with acute-onset focal neurologic deficits and fever. Initial neuroimaging demonstrated focal diffusion restriction, raising concern for acute ischemic stroke, and dual antiplatelet therapy was initiated. Over subsequent days, the patient developed progres

The effect of increased mobility on morbidity in the neurointensive care unit.

OBJECT: The detrimental effects of immobility on intensive care unit (ICU) patients are well established. Limited studies involving medical ICUs have demonstrated the safety and benefit of mobility protocols. Currently no study has investigated the role of increased mobility in the neurointensive care unit population. This study was a single-institution prospective intervention trial to investigate the effectiveness of increased mobility among neurointensive care unit patients. METHODS: All patients admitted to the neurointensive care unit of a tertiary care center over a 16-month period (Apr

Complete mouth rehabilitation with fixed implant-supported restoration for a patient with myasthenia gravis: A clinical report.

This clinical report describes a complete mouth fixed implant-supported rehabilitation for a patient with myasthenia gravis. Patients with myasthenia gravis may have impaired manual dexterity from progressive neuromuscular impairment. Muscle weakness and fatigue, reduced denture stability, and the inability to provide maxillary dentures with a peripheral seal have compromised the ability to wear dentures. Therefore, care is needed when providing an implant-supported prosthesis. This clinical report provides step-by-step management of a patient with myasthenia gravis, providing a complete arch

A 71-year-old man with a rare rhabdoid brain tumour: using a multidisciplinary medical and rehabilitative model of care.

Atypical rhabdoid tumours (AT/RTs) of pineal origin are rare in adults with rapid progression and poor prognosis. We present the case of a 71-year-old man with confusion and memory loss who was diagnosed with a pineal AT/RT after genetic analysis. Due to his limited functional capacity and goal to return home with family, a multidisciplinary care approach was essential for coordination of medical management, radiation treatment and acute inpatient rehabilitation. After diagnosis and rehabilitation, his functional ability improved allowing him to tolerate cranial irradiation, initiate systemic
