Postural sway during quiet stance is attenuated by actively maintained contact of the index finger with a stationary surface, even if the level of applied force (<1 N) cannot provide mechanical stabilization. In this situation, changes in force level at the fingertip lead changes in center of foot pressure by approximately 250 ms. These and related findings indicate that stimulation of the fingertip combined with proprioceptive information about the hand and arm can serve as an active sensor of body position relative to the point of contact. A geometric analysis of the relationship between han

BACKGROUND: Proprioception, the sense of limb position and movement, is impaired in over 50% of stroke survivors. Proprioceptive impairments are known to impede motor recovery, yet proprioception is rarely a focus of stroke rehabilitation. One strategy aimed at facilitating proprioceptive recovery and/or compensation is to have patients look at their arms while performing tasks. We have previously shown that many stroke survivors cannot use vision to compensate for impaired proprioception at ~ 10 days post-stroke, but it is unknown if compensation occurs at later timepoints. We aimed to charac…

Aberrant proprioception is hypothesised to contribute to suboptimal motor adaptations that promote pain persistence. However, when these impairments in proprioception develop in the transition from acute to chronic pain is not well understood. Understanding the nature and time course of proprioceptive changes early in pain development may reveal critical insights. The primary aim of this systematic review and meta-analysis was, therefore, to evaluate the influence of acute pain on proprioception. Six databases (Pubmed, Medline, Web of Science, Psychinfo, Embase, and Scopus) were searched from …

Sensory perception relies on the brain's integration of multiple imprecise inputs, a process known as cue combination. Previous research has investigated multisensory integration or unisensory integration within non-haptic senses. In contrast, cue combination within the sense of touch has been understudied. Here, we investigated whether humans optimally combine haptic cutaneous and finger configuration cues when discerning the size of a disk held edge-on between the thumb and index fingers. When these two fingers span the disk to contact its perimeter, a finger configuration cue (the perceived

BACKGROUND: Thumb carpometacarpal joint osteoarthritis is a prevalent degenerative condition causing pain, reduced strength, and impaired hand function, primarily in postmenopausal women. Proprioceptive deficits contribute to joint instability and functional decline, yet the effectiveness of proprioceptive training remains unclear.
PURPOSE: To systematically review randomized controlled trials evaluating proprioceptive exercise interventions vs conventional rehabilitation alone (strengthening, orthotics, and activity modification) on pain, hand function, proprioceptive acuity, and strength in

The mesencephalic trigeminal nucleus (MTN) contains trigeminal proprioceptive neurons, a unique class of primary sensory neurons with centrally located cell bodies and a developmental origin distinct from that of peripheral ganglion-derived spinal proprioceptors. MTN neurons have long been recognized for their morphological heterogeneity, but their functions were traditionally viewed as confined to the jaw jerk reflex and oromotor control, reflecting their predominant innervation of jaw-closing muscles. Recent single-cell transcriptomic studies have provided new insight into MTN neurons by unc

This case report describes a 46 year old female with complaints of dizziness and decreased ability to complete activities of daily living although aboard a U.S. Navy ship. She was unable to walk unsupported and her ability to work at a computer was severely limited because of the severity of her symptoms. She had a long history of motion sickness, predominantly seasickness (mal de mer), Eustachian tube dysfunction, and bilateral tympanic membrane perforations. Finding minimal benefit from pharmacological interventions, she was evaluated and diagnosed with seasickness by a physical therapist. T

Proprioception is crucial for movement, yet the role of proprioceptive reflexes in legged locomotion is still poorly understood. While previous simulation studies have shown great potential for reflex-based control strategies, these controllers are typically geared to specific gaits, using hand-crafted feedback pathways that are linked to specific gait phases. In this work, we explore the control capabilities of a simple reflex controller that consists of only homonymous and antagonistic length and force feedback pathways with constant gains. This control model can be considered a highly simpl…

OBJECTIVE: Patients with primary orthostatic tremor (OT) are affected by a strong feeling of instability. Our aim was to investigate whether cortical response to proprioceptive information is altered in OT compared with healthy control subjects (HC) using magnetoencephalography (MEG).
METHODS: Fifteen OT patients and 15 HCs were examined using whole-head MEG. Proprioceptive stimulation was evoked by passive movements of the right index finger and right foot. Time-frequency responses were extracted in the frequency range of 8-30 Hz. The MEG sensor with the highest amplitude change time-locked

In this study, we present a wearable device for local vibration stimulation aimed to support postural stability in balance-compromised persons. Experiments were conducted with 15 young and 9 middle-aged adults without lower limb disabilities in a standing posture with vibration stimulation. The results showed a decrease in the mean velocity of the center-of-pressure sway in the standing posture during vibration presentation. In middle-aged and older adults, the rate of increase in the postural stability index was higher than that in the absence of vibration when the standing balance test was r

BACKGROUND: Slowed somatosensory conduction in multiple sclerosis (MS) increases postural instability and decreases proprioception. Despite these delays, individuals with MS rely more on proprioception for balance compared to controls. This heightened reliance, combined with slowed signal transduction, increases fall risk. Backward walking (BW) increases proprioceptive reliance by reducing visual cues. However, no study has conclusively linked proprioception to BW. This study aims to assess proprioception's role in BW compared to forward walking (FW) in MS and to compare differences in proprio…

The field of wrist proprioception, as it relates to rehabilitation and surgery, has gone through a period of intense growth in the past decade. From being primarily focused on the function of the joint and ligaments in patients with wrist trauma or after wrist surgery, the understanding is now that of a greater complexity in treating not just the wrist but the hand and arm as a whole. Proprioception is derived from the Latin words "proprius" - belonging to (oneself) and "-ception" to sense. In other words, how to sense ourselves. To have a complete sense of self, multiple sensory afferents ori…

Proprioceptive cervicogenic dizziness (PCGD) is the most prevalent subcategory of cervicogenic dizziness. There is considerable confusion regarding this clinical syndrome's differential diagnosis, evaluation, and treatment strategy. Our objectives were to conduct a systematic search to map out characteristics of the literature and of potential subpopulations of PCGD, and to classify accordingly the knowledge contained in the literature regarding interventions, outcomes and diagnosis. Electrical stimulation of the vagus nerve has been shown to enhance cortical plasticity and may benefit upper extremity rehabilitation following stroke. As an initial step towards assessing the potential of other craniocervical nerves as neuromodulation targets during rehabilitation, we explored the ability of non-invasive stimulation of cervical spine afferents, paired with a proprioceptive discrimination task, to improve sensory function in neurologically intact human subjects. On each trial, subjects' arms were moved by a robot from a test position, along a random path, to a judgment positi

Proprioception, the ability to sense body position and limb movements in space without visual feedback, is one of the key factors in controlling body movements and performing activities of daily living. However, this capability might be affected after neural injuries such as stroke. Robotic platforms can be used to monitor and promote arm movements and, therefore, can assist in developing rehabilitation protocols that aim to improve proprioception through repetitive reaching motions without vision. The objective of this paper is to investigate if a robotic training protocol improves the end-po…

Familial dysautonomia (FD) is a recessive neurodegenerative disease caused by a splice mutation in Elongator complex protein 1 (ELP1, also known as IKBKAP); this mutation leads to variable skipping of exon 20 and to a drastic reduction of ELP1 in the nervous system. Clinically, many of the debilitating aspects of the disease are related to a progressive loss of proprioception; this loss leads to severe gait ataxia, spinal deformities, and respiratory insufficiency due to neuromuscular incoordination. There is currently no effective treatment for FD, and the disease is ultimately fatal. The dev…
