Abnormal keratin expression pattern in prurigo nodularis epidermis.

BACKGROUND: Prurigo nodularis (PN) is a highly pruritic, chronic dermatosis and difficult to treat. PN lesions are characterized by existence of many hyperkeratotic, erosive papules and nodules. However, the pathogenesis of PN still remains unelucidated. AIM: To clarify the keratin role in the epidermis hyperproliferation, the keratin expression pattern in the PN lesional skin. METHODS: In this study, we enrolled 24 patients with PN and 9 healthy control volunteers. K1/K10, K5/K14, K6/K16/K17 expression pattern were investigated by using immunohistochemical staining. RESULTS: The lesional s…

[Chronic prurigo revealing an angioimmunoblastic T cell lymphoma].

BACKGROUND: Cutaneous manifestations in angio-immunoblastic T cell lymphoma (AITL) can be seen in almost 50% of patients. They are especially represented by maculo-papular pseudotoxic exanthema. The other manifestations, like nodular prurigo are rarely observed. We report a case of AITL diagnosed after an etiologic screening for chronic prurigo. AIM: The objective of our work is to stress on the possibility to diagnose a hemopathy in etiologic screening of chronic pruritus. CASE REPORT: A 45-year-old patient presented a chronic pruritus of 18 months associated with general manifestations (fe…

Topical vitamin D3 (tacalcitol) for steroid-resistant prurigo.

A topical vitamin D3 ointment (tacalcitol) was prescribed for patients with long-lasting pruriginous lesions (four with prurigo nodularis and seven with subacute prurigo, four of whom had atopic dermatitis). Seven of 11 cases had not responded to a topical steroid ointment and even to occlusive application of the ointment. Nine of 11 cases showed a significant clinical response to this new regimen within 4 weeks. Epidermal Fc epsilon R1(+) dendritic cells were increased in number in prurigo nodularis and reduced to normal level after the therapy. Topical vitamin D3 ointment might be an alterna…

Multiple cutaneous granular cell tumors simulating prurigo nodularis.

A 37-year-old black man with 64 pruritic cutaneous granular cell tumors represents the highest number of such lesions ever reported. The clinical resemblance to prurigo nodularis are the unique features. Partial response to intralesional corticosteroid injection is noted.

Shared Mechanistic Pathways in Bullous Pemphigoid, Chronic Spontaneous Urticaria, Prurigo Nodularis, and Chronic Prurigo of Unknown Origin: Implications for Targeted Therapies.

Bullous pemphigoid, chronic spontaneous urticaria, prurigo nodularis, and chronic prurigo of unknown origin are distinct chronic skin diseases with a high disease burden and an ongoing need for safe and effective therapies. Advances in our understanding of disease mechanisms have highlighted convergent type 2-associated neuroimmune pathways that may contribute to chronic itch and skin lesions across these conditions. In bullous pemphigoid, autoantibody binding of two dermal-epidermal junction proteins followed by complement system activation shapes the local immune environment to favor T helpe…

Spanish translation and cross-cultural adaptation of a new severity scale to measure activity in patients with chronic prurigo: Prurigo Control Test (PCT).

OBJECTIVE: Perform the translation and cross-cultural adaptation of the severity scale to measure activity in patients with chronic prurigo: Prurigo Control Test (PCT) for the locality of Río de la Plata, Argentina. METHODS: A cross-sectional study using a mixed-methods (qualitative and quantitative) approach was performed. The process involved seven stages: translation, expert reconciliation, back-translation into English, equivalence review, expert evaluation, cognitive interviews with patients, and development of the final version. RESULTS: A Spanish version of the questionnaire was obtai…

Successful Treatment of Pediatric Atopic Prurigo Nodularis with Dupilumab.

PURPOSE: This study aims to evaluate the efficacy and safety of Dupilumab in treating pediatric atopic prurigo nodularis (PN). PATIENT AND METHODS: We present a case of an 11-year-old child with refractory prurigo nodularis for 9 months, accompanied by concurrent allergic rhinitis for 2 years. Conventional therapies and Janus kinase 1 (JAK1) inhibitors had not provided satisfactory results. At presentation, the child presented with a Peak Pruritus Numerical Rating Scale (PP-NRS) score of 9 on a scale of 10, the Investigator's Global Assessment (IGA) score of 3 out of 3, and the Children's Der…

Interleukin Profiling in Atopic Dermatitis and Chronic Nodular Prurigo.

The clinical manifestations of atopic dermatitis (AD) and chronic nodular prurigo (CNPG) include pruritus and eczema/lesions, posing significant challenges for patients. Th2 cells and ILC2, marked by cytokine production-particularly IL-4/13-are crucial therapeutic targets. Despite displaying a dose-dependent lack of pruritus induction post-injection, IL-13 acts through the IL-13Rα1 and IL-13Rα2 receptor system. Our study focused on investigating ex vivo skin biopsies in AD (n = 17), CNPG (n = 14) and healthy controls (HC; n = 10), examining the gene expression landscape of interleukins linked …
