Radioanatomical study of the proximity of the sciatic nerve and the pelvic bone.

PURPOSE: Percutaneous thermal ablation of pelvic bone tumours carries a risk of iatrogenic injury to the sciatic nerve. This study aimed to determine the proximity of the sciatic nerve to the iliac bone as it exits the pelvis and evaluate its anatomical variability. METHODS: This retrospective, non-interventional study reviewed consecutive MRI scans of the lumbar spine performed at our institution between September and October 2021. Two experienced musculoskeletal radiologists independently measured the shortest distance between the sciatic nerve and the adjacent bone at six predefined levels

Fatigue stress injuries of the pelvic bones and proximal femur: evaluation with MR imaging.

The purpose of this study was to determine the prevalence and the distribution as well as male/female differences in patients with hip or pelvic pain based on MRI results. Three hundred forty consecutive conscripts (45 women, 295 men; age range 18-29 years; mean age 20.7 years) suffering from stress-related hip, buttock or groin pain took part in the study. All 340 patients underwent MR imaging. Radiographic data were available for 215 patients. Two radiologists interpreted the images by consensus. In MRI 174 stress injuries were diagnosed in 137 patients (32 women, 105 men). The incidence of

Pathologic Pubic Symphysis Diastasis in Pregnant Pelvic Ring Fracture Treated with INFIX: A Case Report.

CASE: Unstable pelvic ring fractures in pregnancy are an uncommon injury. Successful treatment of these patients with an INFIX device is even less common, with sparse literature to document the patient outcomes. We found no literature documenting the acute management of a pregnant patient with an INFIX device with (1) dynamic changes recorded, such as increasing pubic symphysis diastasis, and (2) restoration of normal symphyseal anatomy after birth and INFIX removal. CONCLUSION: Use of a pelvic INFIX during pregnancy allowed functional independence. The construct offered sufficient stability

Massive haemorrhage due to minimally displaced pubic ramus fracture.

Fractures of the pubic ramus are commonly seen in the emergency room, but they are thought to be minor injuries. Occasionally, these fractures might be associated with massive haemorrhage. Here we report a case of bilateral obturator artery damage due to minimal displaced pubic ramus fracture.

Spontaneous septic arthritis of the pubic symphysis mimicking acute abdomen: Diagnostic value of MRI.

Septic arthritis of the pubic symphysis is an uncommon and frequently overlooked condition, typically occurring as a postoperative complication. Spontaneous cases without identifiable risk factors are exceptionally rare and may present with misleading clinical features, often mimicking acute abdominal, gynecological, urological, or hip-related conditions. We report the case of a 59-year-old woman with no prior medical or surgical history who presented with acute febrile hypogastric and suprapubic pain. Initial clinical and laboratory findings suggested a lower abdominal or pelvic inflammatory

A novel technique for approximation of the symphysis pubis in bladder exstrophy without pelvic osteotomy.

PURPOSE: We evaluated the outcome of pubic bone approximation and internal fixation using a miniature metal plate and screws without osteotomy in children with bladder exstrophy. MATERIALS AND METHODS: A total of 17 children with bladder exstrophy underwent surgery. Mean patient age was 27 months. Of the patients 15 had a history of 1 or more failed bladder closures with or without osteotomy. All children underwent bladder closure and complete posterior and anterior urethroplasty with bladder neck wrap. Both ureteral orifices were brought together by the Gil-Vernet antireflux procedure. The p…

Small patella syndrome: a bone dysplasia to recognize and differentiate from the nail-patella syndrome.

We report a 5-year, 9-month-old boy with bilateral patellar hypoplasia. The radiographic skeletal survey revealed absent ossification of the ischium and inferior pubic ramus on both sides, characteristic of the newly recognized benign bone dysplasia known as small patella syndrome, or ischiopatellar dysplasia. No nail change, iliac horn, elbow anomaly, or renal disease is associated with this condition.

Squamous cell carcinoma arising in a chronic osteomyelitic sinus tract in the peri-anal region involving pelvis: a rare occurrence.

Squamous cell carcinoma is a rare, but well documented complication of chronic osteomyelitis. Squamous cell carcinoma arising in a sinus tract following chronic osteomyelitis of the pelvic bone is a very rare occurrence. The patient presented with multiple draining sinuses and an ulcerative growth at the mouth of one of the sinuses in the peri-anal region. X-ray findings and sinogram showed de- struction of part of pubic bone. Biopsy from the growth confirmed a well differentiated squamous cell carcinoma. Therefore it is imperative that these lesions be biopsied for accurate diagnosis, adequat…

Ischiopubic hypoplasia: a rare constituent of congenital syndromes.

Hypoplasia of the ischiopubic region is described in four patients. An adolescent was referred for spinal dysraphism and scoliosis, another one for bilateral aplasia of the patella and scoliosis, and finally two children were treated for congenital dislocation of the hip. The osseous malformation of the ischiopubic bones was not associated with any other intrapelvic disorders, and remained unchanged until skeletal maturity. The appearance of this extremely rare anomaly is usually reported as a syndromic constituent of a limited number of congenital malformation syndromes that can be widened to…

Surgical management of penile amputation in children.

PURPOSE: Penile amputation in children is rare. If the amputated organ cannot be salvaged, standard treatment options include sex reassignme … either division of the suspensory ligament (n = 2) or detachment of the corpora cavernosa from the pubic bones. Then, meatal advancement was attempted and combined with a staged oral mucosa urethroplasty, if necessary. Finally, skin coverage was achieved using local flaps (n = 2) or a free graft harvested from the inguinal region. In 2 patients, a pseudogla …

Stereotactic Body Radiation Therapy (SBRT)-Induced 68 Ga-PSMA PET Bone Flare.

A 65-year-old man with oligometastatic prostate cancer underwent radical prostatectomy and stereotactic body radiation therapy (SBRT) to a right pubic bone metastasis. Four weeks after SBRT, PSMA PET showed increased extent and intensity of uptake at the treated site, despite a 92% PSA decline (8.5 to 0.7 ng/mL) and no new lesions. Later imaging confirmed resolution of uptake, consistent with bone flare, but revealed multiple new osseous metastases and rising PSA (2.8 ng/mL), indicating progression. To our knowledge, this is the first reported case of SBRT-induced bone flare, highlighting the …

[Functional anatomy of the abdominal wall].

The anterolateral abdominal wall covers a region defined cranially by the xiphoid process and ribs, laterally by the medial axillary line, and caudally by the anterior ilium and pubic bone. Knowledge of the various parts of the abdominal wall is essential to the surgeon for effective laparotomy and primary and secondary hernia care. The abdominal musculature, aponeuroses, vascularity, and innervation are examined in detail along with the according vascular and neural structures of the dermis.

[X-ray anatomy of the recto-genital pouch in the child].

Peritoneography is a simple radio-anatomic method to get informations about the depth and the orientation of the Douglas pouch in children in relation to fixed bony landmarks in the pelvis (upper part of the pubic bone-lowest sacral segment). Measurements were recorded on patients ranging from 15 days-old to 15 years-old. The older the patient, the deeper and the more posterior seemed to be the Douglas pouch.

Early skeletal development in Talpa europaea, the common European mole.

An ontogenetic series of 22 cleared and double-stained prenatal specimens was used to study the sequence of ossification of selected postcranial skeletal elements of Talpa europaea. Results were compared with nine other therian mammals, with Alligator, Chelydra, and Lacerta as outgroups. Using the event-pairing method, shifts in the onset of ossification in T. europaea, Sus, and Homo were identified. In T. europaea, the ossification of the cervical vertebrae starts before the metatarsals. In Homo and Sus, the tarsals ossify before the pubic bone. These shifts in the sequence of ossification ar…

Triple pelvic osteotomy for the treatment of residual hip dysplasia. Analysis of complications.

The aim of the paper was to present the results achieved with triple pelvic osteotomy in the treatment of residual hip dysplasia, with special interest in identifying recurrences and complications and how to avoid them. 60 patients (76 hips) with developmental dysplasia of the hip (DDH), treated by the triple pelvic osteotomy according to Tönnis and modified by Vladimirov, were included in the study. Hips were evaluated radiologically with the center-edge (CE) angle, break of the Shenton-Menard arch and index of acetabular depth by Heyman-Herndon, and functionally using the Harris Hip Score (H
