Non-communicable health risks during mass gatherings.

Mass gatherings (MGs) have been associated with high rates of morbidity and mortality from non-communicable diseases, accidents, and terrorist attacks, thus posing complex public health challenges. We assessed the health risks and public health responses to MGs to identify an evidence-based framework for public health interventions. Human stampedes and heat-related illnesses are the leading causes of mortality. Minor traumatic injuries and medical complaints are the main contributors to morbidity and, particularly, the need for on-site medical care. Infrastructure, crowd density and mood, weat

Use of Google Earth to strengthen public health capacity and facilitate management of vector-borne diseases in resource-poor environments.

OBJECTIVE: Novel, inexpensive solutions are needed for improved management of vector-borne and other diseases in resource-poor environments. Emerging free software providing access to satellite imagery and simple editing tools (e.g. Google Earth) complement existing geographic information system (GIS) software and provide new opportunities for: (i) strengthening overall public health capacity through development of information for city infrastructures; and (ii) display of public health data directly on an image of the physical environment. METHODS: We used freely accessible satellite imagery

In search of a fix to the primary health care chasm in India: can institutionalizing a public health cadre and inducting family physicians be the answer?

India's woes with an underprioritized and hence underfunded and understaffed public health system continue to plague public healthcare delivery. Though the need for appropriately qualified public health cadre to lead public health programmes is well established, a well-meaning conducive approach to implementing this is lacking. As the COVID-19 pandemic brought back the focus on India's fragmented health system and primary healthcare deficiencies, we discuss the primary healthcare conundrum in India in search of a quintessential fix. We argue for instituting a well-thought and inclusive public

PEPFAR Investments in National Public Health Institutes: HIV Control Through Strong Systems.

National public health institutes (NPHIs) are governmental institutions with a public health mandate, which often includes reference laboratories, surveillance, outbreak preparedness and response, research, public health workforce, scientific communication to translate data into policy, and more. NPHIs are applying public health expertise to help end HIV/AIDS as a public health threat by 2030, in coordination with other key stakeholders, including not just ministries of health but also national AIDS councils, bilateral and multilateral partners, the private sector, civil society, and others. Developing High-Resolution Population and Settlement Data for Impactful Malaria Interventions in Zambia.

Foundational high-resolution geospatial data products for population, settlements, infrastructure, and boundaries may greatly enhance the efficient planning of resource allocation during health sector interventions. To ensure the relevance and sustainability of such products, government partners must be involved from the beginning in their creation, improvement, and/or management, so they can be successfully applied to public health campaigns, such as malaria control and prevention. As an example, Zambia had an ambitious strategy of reaching the entire population with malaria vector control ca

Public health surveillance through community health workers: a scoping review of evidence from 25 low-income and middle-income countries.

BACKGROUND: The last 3 years have witnessed global health challenges, ranging from the pandemics of COVID-19 and mpox (monkeypox) to the Ebola epidemic in Uganda. Public health surveillance is critical for preventing these outbreaks, yet surveillance systems in resource-constrained contexts struggle to provide timely disease reporting. Although community health workers (CHWs) support health systems in low-income and middle-income countries (LMICs), very little has been written about their role in supporting public health surveillance. This review identified the roles, impacts and challenges CH

Hurricane Katrina response and guidance for health-care providers, relief workers, and shelter operators.

Hurricane Katrina struck the coastal areas of Alabama, Florida, Louisiana, and Mississippi on August 29, 2005, causing substantial numbers of deaths among both humans and animals, infrastructure damage, and flooding. Affected areas continue to experience shortages of essential services, including electricity, potable water, food, and fuel; damage to health-care and public health systems; and disrupted communications. CDC/ATSDR, local and state health departments, other federal agencies, and other partners are supporting public health and medical-care functions for persons in affected areas and

Toward Community Health 2067: Social Prescribing, Learning Systems and the Making of an Enabling State.

Responding to commentaries on "Social Prescribing as Strategic Infrastructure," this rejoinder builds on reflections about public provision, community capacity, equity, relational labour and the limits of clinically led referral models. It argues that these contributions sharpen and extend the central proposition: social prescribing can connect local experience to institutional learning, financing and system change. The paper reframes social prescribing across intervention, infrastructure and transformation, and situates it within a learning health policy system and an enabling state that buil

Solid waste management and Aedes aegypti infestation interconnections: A regression tree application.

Public health is at the core of all environmental and anthropic impacts. Urban and territorial planners should include public health concerns in their plans. Basic sanitation infrastructure is essential to maintaining public health and social and economic development. This infrastructure deficiency causes diseases, death and economic losses in developing countries. Framing interconnections among health, sanitation, urbanization and circular economy will assist sustainable development goal achievements. This study aims to identify the relationships between solid waste management indicators in B

Relationship of sanitation, water boiling, and mosquito nets to health biomarkers in a rural subsistence population.

OBJECTIVES: Throughout human evolutionary history, parasites and pathogens were a major cause of mortality-modern urban life with public health infrastructure has changed disease exposure. We examine associations between boiling water, using latrines, mosquito net usage, and biomarkers among the Tsimane, a nonindustrial subsistence population with little public health infrastructure. METHODS: We conducted cross sectional surveys on water, latrines, and bed nets among 507 heads of households (aged 18-92 years, median age 41 years). Regression models estimated associations between behaviors and…

Predicting the Use of New Multi-Use Trails and Parks in Historically Black, Low-Income Neighborhoods: The Role of Interpersonal Discussion and Civic Engagement.

BACKGROUND: Residents of historically Black low-income neighborhoods experience disproportionately poorer health. Public infrastructure redevelopment projects (eg, multi-use trails, parks, and public transportation) may improve quality of life and health outcomes in these neighborhoods. OBJECTIVES: The current study draws from communication infrastructure theory to examine the role of interpersonal discussion and civic engagement (belonging, collective efficacy) in intentions to use newly built trails and parks for physical activity. METHODS: Community-based survey data from 398 randomly sel

The need for adjusting the script; characteristics and future directions of deployable healthcare facilities for climate-change-induced disasters.

Climate change increasingly disrupts public health through frequent, severe disasters, rendering traditional deployable hospitals inadequate for unpredictable climate demands. This paper examines the unique requirements for healthcare infrastructure in extreme environments and proposes a shift toward advanced technological solutions, including air-inflated structures, smart materials, 3D printing, and autonomous robotics. We advocate for modular, sustainable, and age-friendly designs that integrate mental health and community services and remain in the field beyond the acute disaster phase, to

The One Health Blind Spot at the Human‒Animal Interface in US Influenza Surveillance.

Novel influenza A viruses have cost thousands of lives and billions of dollars of economic losses in the 21st century, and 2 of the past 4 pandemic influenza A viruses likely originated in animal populations on the North American continent. Despite this, the human and veterinary influenza surveillance systems in the United States are poorly connected and have fallen short of congressional mandates for data sharing and interoperability. This places the United States in a reactive posture that is focused on containment of outbreaks that have already occurred rather than proactive surveillance an

Caring infrastructures for sexual health and the implications of antibiotic-resistant STIs.

Infrastructures fundamentally shape sexual health, but their gaps and failings often only come into view during moments of crisis. Antimicrobial resistance (AMR) is a looming microbiological and global societal crisis, and one that is posing increasing challenges and risks for medical practice and public health alike. In sexual health, concerns have been raised about growing resistance in Neisseria gonorrhoeae and the less well-known Mycoplasma genitalium. In this article, we examine contemporary sexual health infrastructures and their adequacy in the context of rising antibiotic-resistant STI
