Diagnostic value of the gallium-67 pulmonary leak index in pulmonary edema.

UNLABELLED: We studied the value of a noninvasive, bedside, dual-radionuclide method (67Ga-circulating transferrin and 99mTc-red blood cells) to measure pulmonary microvascular permeability in efforts to discriminate between pulmonary edema due to adult respiratory distress syndrome (ARDS) and hydrostatic pulmonary edema (HPE). METHODS: Patients had respiratory insufficiency and bilateral alveolar pulmonary edema on chest radiographs. All patients, except one, were mechanically ventilated. Patients, were divided into groups according to various sets of etiologic, hemodynamic and ventilatory f…

Unilateral pulmonary hemorrhage caused by negative pressure pulmonary edema: A case report.

BACKGROUND: Unilateral pulmonary hemorrhage is typically reported in young and healthy men with upper respiratory tract obstruction during anesthesia in special situations. Negative pressure in the lungs is created, resulting in negative pressure pulmonary edema (NPPE). CASE SUMMARY: A 78-year-old male patient diagnosed with spinal stenosis was admitted to receive a unilateral laminectomy with bilateral decompression. The patient had been diagnosed with hypertension four years earlier and asthma more than 70 years earlier. We experienced a unilateral alveolar hemorrhage associated with NPPE t…

Clinical and Pathophysiological Features of High-altitude Pulmonary Edema in the Japanese Population: A Review of Studies on High-altitude Pulmonary Edema in Japan.

High-altitude pulmonary edema (HAPE) is a life-threatening, noncardiogenic pulmonary edema that occurs in unacclimatized individuals rapidly ascending to high altitudes above 2,500 m above sea level. Until the entity of HAPE was first identified in a case report published in Japan in 1966, the symptoms of severe dyspnea or coma occurring in climbers of the Japan Alps were incorrectly attributed to pneumonia or congestive heart failure. The Shinshu University Hospital serves as the central facility for rescuing and treating patients with HAPE in the region. Over the past 50 years, a series of s…

Surviving the Unthinkable: A Case of Post-obstructive Pulmonary Edema and Cerebral Edema in a Suicidal Hanging Survivor.

Hanging, ligature strangulation, and manual methods can all result in fatal injuries such as fractures, dislocations, and brain swelling. This case involves a 30-year-old woman who survived a hanging incident but subsequently developed pulmonary and brain swelling. Post-obstructive pulmonary edema (POPE) is characterized by sudden onset non-neurogenic, non-cardiogenic fluid accumulation in the lungs following the removal of an acute upper respiratory tract obstruction. This report explores the patient's clinical presentation, disease progression, management strategies, and potential complicati…

High-altitude Cerebral Edema and High-altitude Pulmonary Edema Diagnosed in the Desert: A Case Report.

INTRODUCTION: Acute mountain sickness, high-altitude pulmonary edema (HAPE), and high-altitude cerebral edema (HACE) are a spectrum of high-altitude conditions, with HACE being the most life-threatening. Most cases develop at altitudes of greater than 4,000 meters (∼13,000 feet) above sea level and after one to five days. CASE REPORT: A previously healthy 46-year-old female presented to the emergency department with ataxia, altered mental status, and vomiting that developed after rapidly ascending to ∼2,400 meters (∼7,800 feet) above sea level. She was treated for HACE and HAPE with resolutio…

Etiology of acute pulmonary edema during liver transplantation : a series of cases with analysis of the edema fluid.

STUDY OBJECTIVES: To describe the clinical features of a group of patients who acutely developed pulmonary edema during orthotopic liver transplantation and to determine the nature (transudate vs exudate) of the edema. DESIGN: Retrospective review of clinical records and radiographic studies. SETTING: Operating room and ICU of a tertiary-care medical center hospital. PATIENTS: End-stage liver disease patients undergoing orthotopic liver transplantation under general anesthesia. INTERVENTIONS AND MEASUREMENTS: Pulmonary edema fluid obtained from seven patients within 15 min of first appeara…

Severe pulmonary edema after pulmonary hypertension interventional surgery: case report and literature review.

BACKGROUND: Pulmonary hypertension caused by fibrosing mediastinitis often presents with clinical manifestations related to involvement of the pulmonary arteries, pulmonary veins, and bronchi. Interventional therapy has become an important treatment option; however, it carries a significant risk of complications. In this case, the patient developed severe pulmonary edema after intervention, which posed major challenges to clinical management. CASE PRESENTATION: A middle-aged man presented with recurrent cough, expectoration, chest tightness, and dyspnea. He was initially treated for chronic o…

Perilla ketone: a model of increased pulmonary microvascular permeability pulmonary edema in sheep.

A model of increased microvascular permeability pulmonary edema was developed in chronically instrumented unanesthetized sheep using perilla ketone (PK). PK did not cause changes in pulmonary hemodynamics but did cause marked increases in the flow of protein-rich lung lymph. The changes in lung lymph flow were accompanied by radiographic evidence of both interstitial and alveolar pulmonary edema as well as hypoxemia. PK did not cause acute changes in lung mechanics. Dynamic compliance of the lungs and FRC decreased later, concomitant with the changes in lung lymph flow, radiographic evidence f…

Moraxella catarrhalis bacteremia in adult with cardiogenic pulmonary edema.

Bacteremia due to Moraxella Catarrhalis is rare. An 85-year-old Japanese woman had productive cough, dyspnea, and fever. She had type 2 diabetes mellitus and chronic heart failure. Infiltration shown on chest radiography could be explained by heart failure or pneumonia. Initial blood culture yielded Gram-negative cocci, identified as M. catarrhalis. We therefore diagnosed cardiogenic pulmonary edema and M. catarrhalis pneumonia. M. catarrhalis has a high prevalence of β-lactamase production, so treatment comprised ampicillin/sulbactam. The clinical outcomes were favorable. Our case shows the i…
