Gut microbiota in chronic obstructive pulmonary disease varies by CT-verified emphysema status.

BACKGROUND AND AIM: The association of the gut microbiota to chronic obstructive pulmonary disease (COPD) phenotypes is underexplored. We aimed to compare stool samples from patients with COPD and subjects without COPD and relate findings to emphysema status, exacerbation rate, blood eosinophil levels, symptom score, and lung function. METHODS: We report findings from a single-centre case-control study with 62 current and former smoking patients with COPD and 49 subjects without COPD. DNA was extracted from stool samples, and the V3V4-region of the bacterial 16S-rRNA gene was sequenced. Emphy

Signalling via CD131 regulates pulmonary inflammation, alveolar cell death and emphysema in COPD.

BACKGROUND: COPD is a heterogenous disease where chronic inflammation is implicated in airway remodelling and emphysema. The CD131 receptor is indispensable for signalling by the β common (βc) family of cytokines encompassing granulocyte-macrophage colony-stimulating factor, interleukin (IL)-5 and IL-3, which instigate both type-2 and non-type-2 inflammatory responses. This study aims to determine whether antagonising CD131 signalling can prevent pulmonary inflammation, alveolar cell death and emphysema development. METHODS: We performed in-house and in silico transcriptomic analysis to inves

Diaphragmatic motion studied by M-mode ultrasonography in combined pulmonary fibrosis and emphysema.

BACKGROUND: The coexistence of emphysema and pulmonary fibrosis is known as combined pulmonary fibrosis and emphysema (CPFE). The aim of this study was to compare diaphragmatic motion measured by M-mode ultrasonography of patients with CPFE, idiopathic pulmonary fibrosis (IPF), and chronic obstructive pulmonary disease (COPD). METHODS: Pulmonary function, high-resolution computed tomography (HRCT), and diaphragmatic motion were examined in patients with CPFE (n = 25), IPF (n = 18), and COPD (n = 60), and in healthy controls (n = 21). Diaphragmatic motions were measured on M-mode ultrasonograp

Persistent pulmonary interstitial emphysema presenting as solitary lung cyst in a preterm infant.

Solitary unilateral lung cyst is an unusual finding in preterm infants. It may be difficult to distinguish acquired from congenital lung cysts clinically. The definitive diagnosis is histological; however, CT scan of the chest is a useful diagnostic tool. We present an extremely preterm infant with solitary lung cyst and background chronic lung disease. The initial chest x-rays showed solitary right lung cyst. At 6 weeks he required an escalation of ventilator support coupled with x-ray evidence of increased size of the cyst. CT scan confirmed large solitary cyst of the right lower lobe with e

HOX genes in human lung: altered expression in primary pulmonary hypertension and emphysema.

HOX genes belong to the large family of homeodomain genes that function as transcription factors. Animal studies indicate that they play an essential role in lung development. We investigated the expression pattern of HOX genes in human lung tissue by using microarray and degenerate reverse transcriptase-polymerase chain reaction survey techniques. HOX genes predominantly from the 3' end of clusters A and B were expressed in normal human adult lung and among them HOXA5 was the most abundant, followed by HOXB2 and HOXB6. In fetal (12 weeks old) and diseased lung specimens (emphysema, primary pu

Anaesthetic management of a 6-week-old child with unilateral pulmonary interstitial emphysema.

We report the case of a 6-week-old female infant suffering from respiratory distress related to cystic changes affecting the entire left lung. Anaesthesia was induced with sevoflurane in oxygen and spontaneous ventilation was maintained until intubation of the right main bronchus was secured. A left pneumonectomy was performed and the postoperative course was uneventful. The pathological diagnosis was pulmonary interstitial emphysema. This has not been previously reported in an otherwise normal child delivered uneventfully at term. The anaesthetic management of a child with a cystic lung lesio

New approach to management of unilateral tension pulmonary interstitial emphysema in premature infants.

Conservative management of unilateral tension pulmonary interstitial emphysema is reported. Treatment consisted of (1) positioning the infant on his or her side with the emphysematous lung dependent, (2) minimal chest physiotherapy and endotracheal suctioning, and (3) appropriate ventilator management. Conservative therapy is effective and appears to be safer than selective bronchial intubation or surgical therapy.

Computed tomographic measurement of airway remodeling and emphysema in advanced chronic obstructive pulmonary disease. Correlation with pulmonary hypertension.

RATIONALE: Pulmonary hypertension (PH) is an established complication of advanced chronic obstructive pulmonary disease (COPD) associated with increased mortality. The mechanisms coupling PH and bronchial obstruction are unknown; in particular, PH appears to be unrelated to emphysema. We hypothesized that computed tomographic (CT) measurement of airway remodeling instead of emphysema may correlate with PH in COPD. OBJECTIVES: We aimed to describe the clinical and CT characteristics of patients with COPD with or without PH and to correlate CT measurements of airway remodeling and emphysema wit

Mixed Mucus Plug-Emphysema Subtype and Acute Exacerbation in Chronic Obstructive Pulmonary Disease.

BACKGROUND: Conventional clinical assessments inadequately capture chronic obstructive pulmonary disease (COPD) heterogeneity. Quantitative computed tomography metrics may enable refined phenotyping and improved prognostic assessment. RESEARCH QUESTION: What is the prognostic value of quantitative computed tomography-derived mucus plugs and emphysema for acute exacerbations in COPD? STUDY DESIGN AND METHODS: This prospective cohort study collected clinical data, chest computed tomography, and pulmonary function tests from patients with COPD. Patients were stratified by mucus plug score (cuto

Changes in bone metabolism dynamics in response to mechanical stress under pulmonary emphysema.

INTRODUCTION: There are many risk factors for fractures in chronic obstructive pulmonary disease patients, and investigating the effects of any particular factor on bones is extremely difficult. We hypothesized that the presence of pulmonary emphysema would delay the recovery of bone mass by reloading after unloading, and our study aimed to assess changes in bone metabolism dynamics in response to mechanical stress under pulmonary emphysema in model mice. MATERIALS AND METHODS: We used C57BL/6J mice, with or without emphysema, via intratracheal administration of elastase or vehicle. Half of t…

Transforming pulmonary care with applications of endobronchial valves beyond emphysema: a narrative review.

BACKGROUND AND OBJECTIVE: Endobronchial one‑way valves (EBVs) were originally developed for lung volume reduction in severe emphysema. Because EBVs allow unidirectional airflow, they have been used off‑label to manage persistent air leaks (PALs)/bronchopleural fistulas (BPFs), haemoptysis, cavitary tuberculosis (TB), and other complex pulmonary conditions. This review summarises the evidence of endobronchial valve applications beyond emphysema. METHODS: We conducted a narrative review of English‑language literature from January 1, 2000 to August 1, 2025, searching PubMed, Embase, Scopus, and

Combined pulmonary fibrosis and emphysema and lung transplantation: current evidence and future directions.

BACKGROUND: Combined pulmonary fibrosis and emphysema (CPFE) patients generally have a poorer prognosis compared to those with either pulmonary fibrosis or chronic obstructive pulmonary disease (COPD)/emphysema alone, as demonstrated by several studies. Our study aimed to identify undiagnosed CPFE cases within a lung transplantation cohort initially diagnosed with interstitial lung fibrosis or COPD/emphysema. It was hypothesized that the patient cohort might contain overlooked CPFE cases, and CPFE patients would exhibit a higher incidence of pre-transplant pulmonary hypertension and post-trans

Quantifying the spatial clustering characteristics of radiographic emphysema explains variability in pulmonary function.

Quantitative assessment of emphysema in CT scans has mostly focused on calculating the percentage of lung tissue that is deemed abnormal based on a density thresholding strategy. However, this overall measure of disease burden discards virtually all the spatial information encoded in the scan that is implicitly utilized in a visual assessment. This simplification is likely grouping heterogenous disease patterns and is potentially obscuring clinical phenotypes and variable disease outcomes. To overcome this, several methods that attempt to quantify heterogeneity in emphysema distribution have b

Pulmonary emphysema quantification at low dose chest CT using Deep Learning image reconstruction.

PURPOSE: Quantitative analysis of emphysema volume is affected by the radiation dose and the CT reconstruction technique. We aim to evaluate the influence of a commercially available deep learning image reconstruction algorithm (DLIR) on the quantification of pulmonary emphysema in low-dose chest CT. METHODS: We performed a retrospective study of low dose chest CT scans in 54 patients with chronic obstructive pulmonary disease (COPD). Raw data were reconstructed using FBP, iterative reconstruction (ASIR-V 70%) and deep learning based algorithms at high, medium and low-strength (DLIR -H, -M, -…

Free lung desmosine: a potential biomarker for elastic fiber injury in pulmonary emphysema.

INTRODUCTION: Desmosine and isodesmosine (DID) are biomarkers for elastic fibre damage in pulmonary emphysema. However, current methods for measuring lung DID involve tissue hydrolysis and lack specificity for those fibres undergoing breakdown. To address this limitation, free (nonpeptide-bound) DID content in unhydrolyzed tissues was evaluated as a more accurate biomarker in an animal model of pulmonary emphysema. METHODS: Hamsters were treated with either cigarette smoke and lipopolysaccharide (LPS), room air and LPS, or room air alone (controls). Free DID levels in fresh and formalin-fixed…

Brief Report: HIV Is Associated With Impaired Pulmonary Diffusing Capacity Independent of Emphysema.

BACKGROUND: HIV is associated with accelerated decline in lung function and increased risk for chronic obstructive pulmonary disease (COPD). Recently, there has been growing attention toward the impairment in the diffusing capacity of the lungs for carbon monoxide (DLCO), a marker of pulmonary gas exchange, observed among persons living with HIV. Although increased emphysema can contribute to the DLCO impairment observed, other factors may drive this association. METHODS: Using cross-sectional data from the Study of HIV in the Etiology of Lung Disease, we studied the association between HIV a
