Use of initial distribution volume of glucose to determine fluid volume loading in pulmonary thromboembolism and right ventricular myocardial infarction.

We report a case of acute right ventricular myocardial infarction (right AMI) following pulmonary thromboembolism (PTE). Following percutaneous coronary intervention, the patient was treated in our intensive care unit (ICU) with intraortic balloon pumping, anticoagulants, and plasma expansion. Fluid overload may cause a further leftward shift of the interventricular septum in patients with PTE, resulting in decreased cardiac output (CO). The initial distribution volume of glucose (IDVG) has been reported to indicate central extracellular fluid volume. As both PTE and right AMI affect cardiac f

Myocardial Infarction and Complete Heart Block in a Patient With Anomalous Origin of the Left Coronary Artery From the Pulmonary Artery (ALCAPA).

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital malformation. We present a case of an elderly patient with ALCAPA presenting with complete heart block and non-ST-elevation myocardial infarction years after diagnosis and surgical correction. An 81-year-old female with a history of ALCAPA presented to the emergency department with chest pain and progressive mental deterioration. She was bradycardic and hypotensive. An electrocardiogram revealed a complete heart block. Troponin was 4.04 ng/mL. She received atropine and underwent transcutaneous

Intrauterine fetal death of a monochorionic twin with peripheral pulmonary infarcts: potential thromboembolic events following death of co-twin.

In utero fetal lung infarction has rarely been reported. We present a case of intrauterine lung infarction in a 28-3/7 weeks' gestation monochorionic twin following intrauterine fetal demise of the co-twin at 20 weeks. This case highlights the potential for thromboembolic events (TBEs) associated with monochorionic gestations to include pulmonary TBE and infarction among the risks for fetal morbidity and mortality.

Attention to pulmonary arteriovenous fistula in a case of transient hypoxemia and cerebral infarction during pregnancy: a case report and literature review.

BACKGROUND: Pulmonary arteriovenous fistula is rare during pregnancy. Pulmonary arteriovenous fistula presents no pulmonary symptoms in most patients but can be exacerbated by pregnancy. If not diagnosed and treated promptly, pulmonary arteriovenous fistula can lead to respiratory failure, stroke, spontaneous hemothorax, or other fatal complications. CASE PRESENTATION: A 29-year-old healthy pregnant woman presented with a transient drop in blood oxygen level of unknown cause during a routine examination at 34 weeks of gestation and during a cesarean section at 38 weeks of pregnancy. The patie

[The case of an octogenarian who died of acute myocardial infarction complicated with acute pulmonary embolism from a right ventricular thrombus].

An 86-year-old female on dialysis experienced a decrease in blood pressure and worsening of her respiratory condition during dialysis, for which she visited our emergency unit. She was admitted to our Department of Cardiology with a diagnosis of acute myocardial infarction complicated with heart failure because of anterior wall of left ventricular dysfunction, positive troponin T levels and negative T wave on a precordial lead electrocardiogram. On the same day, she underwent coronary angiography and stenting at left anterior descending artery #7 with 99% stenosis. She also showed an elevated

Lethal hypoplasia and developmental anomalies of the lungs in a newborn with intrauterine adrenal hemorrhage and cerebral infarcts: a proposed pulmonary disruption sequence.

We report a case of a 31-week-gestation male newborn who died soon after birth from intractable respiratory failure and persistent pulmonary hypertension. The pregnancy had been complicated by intermittent bleeding between 13 and 20 weeks' gestation, attributed to peripheral placental separation, as well as bilateral fetal adrenal hemorrhage, first detected at 17 weeks' gestation. Postmortem examination revealed small, calcified adrenal glands as well as several remote cerebral and cerebellar infarcts. The lungs were hypoplastic (lung weight/body weight ratio: 1.64%; 10th percentile for 28-36

[Specific features of impairment of systolic and diastolic left ventricular functions in patients with acute myocardial infarction complicated with pulmonary edema].

The purpose of the study was to investigate specific features of systolic and diastolic functions of the left ventricle (LV) in patients with acute myocardial infarction (AMI), complicated by acute left ventricular failure (ALVF) and to estimate their prognostic value. We examined 90 patients with AMI. The following types of left ventricular dysfunction were detected in patients with ALVF: systolic dysfunction (25%), restrictive filling pattern of mitral flow (11.7%), combined dysfunction (31.7%); whereas 31.7% of these patients had no severe dysfunction of LV. The presence of restrictive fill

[High-frequency jet ventilation of the lungs during intensive care of cardiogenic shock and pulmonary edema in patients with myocardial infarction].

High-frequency (HF) jet ventilation through transtracheal catheter has been used for the therapy of acute respiratory failure in 30 patients ith myocardial infarction complicated by pulmonary edema and cardiogenic shock. Gas exchange and central hemodynamics have been assessed, using a Swan-Ganz catheter fixed in the pulmonary artery. HF ventilation led to a decrease of hypoxemia, pulmonary blood shunting and to a moderate increase in the cardiac output.

A machine learning model for predicting short-term in-hospital mortality in acute myocardial infarction with coexisting chronic obstructive pulmonary disease.

BACKGROUND: Patients with acute myocardial infarction(AMI)complicated by chronic obstructive pulmonary disease (COPD) in the intensive care unit (ICU) face a significantly elevated risk of mortality. Therefore, timely and accurate risk stratification is critical for guiding clinical decision-making. However, validated interpretable prediction models for short-term mortality in patients with both AMI and COPD remain limited. METHODS: We retrospectively extracted data from the MIMIC-IV database (version 3.1) and identified ICU patients with acute myocardial infarction complicated by chronic obs

The impact of group II pulmonary hypertension on congestive heart failure patients admitted with ST elevation myocardial infarction, a nationwide study.

BACKGROUND: Pulmonary hypertension (PH) is a condition where the blood pressure increases in the pulmonary arteries, leading to reduced oxygen delivery to the body's tissues due to increased blood flow resistance. This condition can result in right ventricular hypertrophy, low cardiac output, and ischemia. In this study, the authors aim to investigate the impact of group II PH (GIIPH) on patients with congestive heart failure who were admitted with ST elevation myocardial infarction (STEMI) through a retrospective cohort study. METHODS: Using the National Inpatient Sample (NIS) database from

Effect of pulmonary artery catheter, type & combination of vasoactives for optimizing lactate clearance in acute myocardial infarction complicated by cardiogenic shock.

INTRODUCTION: Lactate clearance(LC) is critical in managing critically ill patients. We hypothesized that treatment allocation with different vasoactive drugs or the presence of a pulmonary artery catheter (PAC) could affect the behavior of lactate dynamics and, ultimately, the mortality in AMI-CS. MATERIALS AND METHODS: In 651 patients with AMI-CS, we examined the relationship of LC time with clinical, laboratory, and CS-management variables. Complete LC time was defined as serum lactate levels less than <2 mmol/L. We explore the impact of vasoactive drugs and PAC with LC. The CART method de

Case report: The presence of third-degree atrioventricular block caused by pulmonary embolism masquerading as acute ST-segment elevation myocardial infarction.

BACKGROUND: Pulmonary embolism (PE) typically presents with chest pain, tachypnea, hemoptysis, syncope, and increased markers of myocardial injury. On an electrocardiogram (ECG), sinus tachycardia, right bundle branch block (RBBB), S1Q3T3 pattern, and/or precordial T-wave inversion may be observed. Despite being one of the common causes of chest pain, a third-degree atrioventricular block (III° AVB) is rare in cases of PE, which can lead to difficulties in diagnosis or even overlooking this condition. CASE SUMMARY: In this case report, we present a patient who was transferred to our hospital

Role of hybrid operating room in surgery for the right atrial thrombus, pulmonary thrombi, and ventricular septal rupture after myocardial infarction.

Free-floating right heart thrombi are uncommon and need emergency treatment in view of their tendency to dislodge and cause pulmonary embolism. We report a successful surgical management of a patient who had large mobile right atrial thrombus, bilateral pulmonary thrombi, coronary artery disease, and postmyocardial infarction ventricular septal rupture (VSR). The patient underwent coronary angiography, inferior vena cava filter placement, removal of thrombi from the right atrium and pulmonary arteries, repair of VSR, and coronary artery bypass graft surgery in a hybrid operating room.

Myocardial infarction and other co-morbidities in patients with chronic obstructive pulmonary disease: a Danish nationwide study of 7.4 million individuals.

AIMS: Myocardial infarction is nominally the most important co-morbidity in patients with chronic obstructive pulmonary disease, and the one with the greatest potential for treatment and prevention to improve the overall prognosis of chronic obstructive pulmonary disease patients. We assessed the extent of myocardial infarction and other co-morbidities in individuals with chronic obstructive pulmonary disease in the general population. METHODS AND RESULTS: We used individual participant data for the entire Danish population from 1980 through 2006, comprising 140 million person-years of follow

COPD Severity and Five-Year Mortality After Acute Myocardial Infarction in Patients with Chronic Obstructive Pulmonary Disease: A Retrospective Cohort Study.

Background: Chronic obstructive pulmonary disease (COPD) and acute myocardial infarction (AMI) frequently coexist and share several biological pathways and risk factors. Although COPD is associated with worse outcomes after AMI, the prognostic value of objective pulmonary function has not been well characterized. The aim of this study was to evaluate the association between airflow-obstruction severity and five-year all-cause mortality after AMI among patients with COPD. Methods: This single-center retrospective cohort study was restricted to adult post-AMI patients (2002-2017) with a backgrou
