Outpatient evaluation of vision and ocular motricity in 123 children with cerebral palsy.

PURPOSE: Children with cerebral palsy (CP) may present varied ophthalmological problems. Paradoxically, however, investigation of visual function and possible related disorders is not routinely conducted in this population during medical consultations. METHOD: This paper proposes a simple, practical guide for assessing vision and ocular motricity in children with CP. One-hundred and twenty-three patients (mean age: 8.4 years +/- 2.3) from the Sarah Network of Rehabilitation Hospitals (Brasilia, Rio de Janeiro and São Luis units) were studied. Various parameters were investigated (smooth pursu

Decisional role of the dorsolateral prefrontal cortex in ocular motor behaviour.

Three patients with a unilateral cortical lesion affecting the dorsolateral prefrontal cortex (DLPFC), i.e. Brodmann area 46, were tested using different paradigms of reflexive saccades (gap and overlap tasks), intentional saccades (antisaccades, memory-guided and predictive saccades) and smooth pursuit movements. Visually guided saccades with gap and overlap, latency of correct antisaccades and memory-guided saccades and the gain of smooth pursuit were normal, compared with controls. These results confirm our anatomical data showing that the adjacent frontal eye field (FEF) was unimpaired in

Visually induced eye movements in Wallenberg's syndrome.

Eighteen patients with Wallenberg's syndrome were investigated concerning visually induced eye movements. All results were analysed quantitatively using a computer. In 16 out of 18 patients, OKN slow-phase velocities were impaired, in the remaining 2 patients they were normal. Patients who showed impaired OKN were classified into two groups according to directional asymmetry of OKN impairment. In 7 out of the 16 patients, slow-phases toward the lesion side were predominantly impaired in contrast to the slow-phases toward the intact side (group A). In the remaining 9 patients, slow-phases towar

Bedside video-oculographic evaluation of eye movements in acute supratentorial stroke patients: A potential biomarker for hemispatial neglect.

BACKGROUND AND PURPOSE: The presence of hemispatial neglect adversely affects functional outcomes in stroke patients; consequently, it warrants early targeted rehabilitative intervention. Nevertheless, hemispatial neglect in the acute phase of stroke has often been underdiagnosed. In this study, we aimed to detect hemispatial neglect at the bedside in acute stroke patients by measuring eye movements using video-oculography (VOG). METHODS: Forty-seven patients with acute unilateral supratentorial stroke were enrolled. We quantitatively measured horizontal saccade (latency, velocity, and amplit

Eye Tracking Results in Postconcussive Syndrome Versus Normative Participants.

PURPOSE: Standard physical, neurologic, and neuropsychologic examinations may not detect abnormalities after mild traumatic brain injury (mTBI). An analysis of eye movements may be more sensitive to neurologic dysfunction. METHODS: We performed eye tracking assessments in 71 active duty and veteran military personnel with persistent postconcussive symptoms (3 months to 5 years after mTBI) and 75 volunteers with no history of brain injury. Both eyes were sampled at 500 Hz and analyzed for various eye measurement parameters during visual tasks involving the saccadic and smooth systems. RESULTS

Object motion perception is shaped by the motor control mechanism of ocular pursuit.

It is still a matter of debate whether the control of smooth pursuit eye movements involves an internal drive signal from object motion perception. We measured human target velocity and target position perceptions and compared them with the presumed pursuit control mechanism (model simulations). We presented normal subjects (Ns) and vestibular loss patients (Ps) with visual target motion in space. Concurrently, a visual background was presented, which was kept stationary or was moved with or against the target (five combinations). The motion stimuli consisted of smoothed ramp displacements wit

Oculometric biomarkers of visuomotor deficits in clinically asymptomatic patients with systemic lupus erythematosus undergoing long-term hydroxychloroquine treatment.

INTRODUCTION: This study examines a set of oculomotor measurements, or "oculometric" biomarkers, as potential early indicators of visual and visuomotor deficits due to retinal toxicity in asymptomatic Systemic Lupus Erythematosus (SLE) patients on long-term hydroxychloroquine (HCQ) treatment. The aim is to identify subclinical functional impairments that are otherwise undetectable by standard clinical tests and to link them to structural retinal changes. METHODS: We measured oculomotor responses in a cohort of SLE patients on chronic HCQ therapy using a previously established behavioral task

Gait apraxia in communicating hydrocephalus.

Apraxia of gait in patients with communicating hydrocephalus appears in the context of a generalised motor disorder that includes defective righting reflexes, generalised increased tone to passive movements, grasp reflexes, difficulty with serial movements of the hands and defective smooth pursuit eye movements. The inability to walk does not appear to be due to a motor disorder but to release of proprioceptive supporting reactions. This mechanism is triggered by proprioceptive stimuli.

Integrating Multi-Task Eye Tracking and Interpretable Machine Learning for High-Accuracy Screening of Amblyopia in Pediatric Populations.

Amblyopia is a developmental disorder of spatial vision in which abnormal visual experience leads to persistent reductions in acuity and contrast sensitivity, even after optimal optical correction. We introduce a brief, child-friendly battery of task-evoked eye tracking that probes fixation stability, fine pattern processing, and smooth pursuit control across three simple paradigms. Oculomotor traces are transformed into physiologically interpretable markers-fixation dispersion and saccadic strategy, orientation-dependent drift and stability, pursuit gain, and tracking error-and used to train

Association between oculomotor function testing parameters and Parkinson's disease.

BACKGROUND: Parkinson's disease (PD) is characterized by motor and non-motor symptoms with oculomotor dysfunction being a notable feature. This study aimed to determine the oculomotor function in patients with PD using video-oculography (VOG) and to identify the association between physiologic parameters and PD. METHODS: The study enrolled 45 patients diagnosed with PD at onset and 45 age- and gender-matched healthy controls from July 2019 to July 2023. All participants underwent VOG testing. Eye movement parameters, including velocity, latency, gain, and accuracy, were quantified. The presen

Clinical neurophysiology of dementia.

The role of EEG in the study of the dementias is to help in the differential diagnosis of the multiple causes of this syndrome. EEG is useful in differentiating early on between treatable and as of now untreatable forms of dementia. Space-occupying lesions that give rise to dementia are reliably detected by EEG. Infectious, toxic, and metabolic processes are associated with early and severe electroencephalographic abnormalities. The "slow virus" infections show characteristic electrical patterns that reliably distinguish them from the cortical or subcortical dementias. Finally, the EEG may con

[Correlation between eye movement and white matter hyperintensity characteristics in patients with cerebral small vessel disease].

OBJECTIVES: The mechanism of abnormal eye movements in patients with cerebral small vessel disease (CSVD) remains unclear. This study aims to explore the potential link between eye movement in CSVD patients and the severity and distribution of white matter hyperintensities (WMH), and to evaluate the possibility of using eye movement assessment as a tool for specific diagnosis. METHODS: This retrospective cross-sectional study was conducted at Xiangya Hospital, Central South University between September 7th, 2022 and October 27th, 2023, enrolling a total of 161 patients with CSVD. Baseline Oculomotor Parameters Are Prospectively Associated with Cognitive Processing Speed at 6-Year Follow-Up in Multiple Sclerosis: An Exploratory Cohort Study.

Background/Objectives: Multiple sclerosis (MS) commonly impairs information processing speed, which is not well captured by conventional disability metrics. Oculomotor paradigms engage neural circuits frequently affected in MS and may provide objective cognitive correlates. We investigated whether baseline oculomotor parameters are prospectively associated with processing speed at ~6-year follow-up. Methods: Forty-four patients with MS underwent a standardized oculomotor battery (visually guided saccades, antisaccades, and sinusoidal smooth pursuit). The Symbol Digit Modalities Test (SDMT) was

Quantitative vestibular function tests in posterior circulation stroke patients: A review.

While specific bedside examinations are known to be sensitive in identifying stroke among acute vestibular syndrome patients, complementary quantitative vestibular function testing can be helpful to quantify vestibular loss due to stroke. In contrast to peripheral vestibular dysfunction, diagnosis of central vestibular dysfunction can be challenging for unskilful clinicians. This article presents a comprehensive overview of quantitative vestibular function test findings such as the video head impulse test (vHIT), cervical vestibular evoked myogenic potentials (cVEMPs), ocular vestibular evoked

Freezing of saccades in dopa-responsive parkinsonian syndrome.

PURPOSE: Ocular motor abnormalities such as abnormal saccades are common in idiopathic Parkinson's disease (PD) and atypical parkinsonian syndrome, such as progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD). In this study, we describe a case of patient with PD and show a video illustrating severe delay of reflexive saccades. OBSERVATIONS: A 68-year-old Caucasian woman with diagnosis of PD presented for evaluation of diplopia. Neuro-ophthalmic examination revealed good visual acuity in both eyes and normal optic nerves but prominent ocular motor abnormalities, including h

Allocation of attention during pursuit of large objects is no different than during fixation.

Attention allocation during pursuit of a spot is usually characterized as asymmetric with more attention placed ahead of the target than behind it. However, attention is symmetrically allocated across larger pursuit stimuli. An unresolved issue is how tightly attention is constrained on large stimuli during pursuit. Although some work shows it is tightly locked to the fovea, other work shows it is allocated flexibly. To investigate this, we had observers perform a character identification task on large pursuit stimuli composed of arrays of five, nine, or 15 characters spaced between 0.6° and 4

[Eye movement disorders in hereditary degenerative ataxia. Electro-oculographic study of 11 cases].

An electro-oculographic study has been performed in 11 patients affected by hereditary degenerative ataxias (5 Friedreich's ataxias, 4 olivopontocerebellar atrophies and 2 late onset cerebellar ataxias). Electrooculographic records were obtained during saccades, pursuit movements and fixed gaze. Saccadic latency, saccadic speed and morphologic features were studied. In Friedreich's ataxia ocular motility was less accurate than in olivopontocerebellar atrophy and in late onset cerebellar ataxia.

A limitation of position constancy.

When the eyes track a moving object, the image of a stationary target shifts on the retina colinearly with the eye movement. A compensation process called position constancy prevents this image shift from causing perceived target motion commensurate with the image shift. The target either appears stationary or seems to move in the direction opposite to the eye movement, but much less than the image shift would warrant. Our work is concerned with the question of whether position constancy operates when the image shift and the eye movement are not colinear. That can occur when, during the eye mo

Wearable impedance oculography: a new method for eye motion classification.

Electrooculography (EOG) for eye movement detection and condition monitoring are affected by variable baseline drifts and are susceptible to motion artifact-induced noise. To address these limitations, this work introduces an eye movement classification approach, covering blink, saccade, smooth pursuit, vergence, and vestibulo-ocular movements, using impedance measurement from an eye wearable system. Impedance oculography (IOG) data were collected from subjects performing the specified eye movements using a spectacle-mounted two-electrode system connected to a IOG measurement set up, which, wi
