Xanthogranulomatous pyelonephritis: A rare case report of a 54 year old female (a potentially fatal infection).

INTRODUCTION AND IMPORTANCE: Xanthogranulomatous pyelonephritis (XPGN) is a rare pathology of the kidneys occurring in 0.6 to 1% of all cases of renal infections, in both men and women. It is characterized by severe inflammation of the renal parenchyma leading to formation of granulomatous tissue containing lipid-laden macrophages. This condition may mimic less aggressive or benign conditions but may worsen or be fatal if not treated aggressively. CASE PRESENTATION: Our patient is a 54 year old Caucasian female who presented with five days of left flank pain, hematuria, chills, nausea and vom

[Xanthogranulomatous pyelonephritis presenting as a Grawitz tumor in a 21-year-old female patient. Case report] ].

Xanthogranulomatous pyelonephritis is a chronic inflammatory disease of the kidney characterized by destruction and replacement of the renal parenchyma with granulomatous tissue containing lipid-filled macrophages. The disease affects mainly elders, more frequent females and is difficult to differentiate it from renal neoplasms by standard diagnostic imaging methods. This case report confirms it.

Laparoscopic Heminephrectomy of Chronically Obstructed Horseshoe Kidney Moiety with Staghorn Calculus, Massive Pyonephrosis, and Xanthogranulomatous Pyelonephritis.

Laparoscopic heminephrectomy was performed in a 64-year-old woman with a chronically obstructed horseshoe kidney moiety. More than 3000 cc of pyonephrosis was drained through two percutaneous nephrostomy tubes for infection control before left moiety laparoscopic resection. This case report attests to the feasibility of laparoscopic resection of a massively obstructed horseshoe kidney when performed in a staged manner after prolonged drainage. Since an incision was required for removal of the large specimen, ligation of the horseshoe isthmus was completed through the same incision after hilar

Management of Xanthogranulomatous Pyelonephritis: A Review of Current Evidence and Treatment Strategies and a Proposed Clinical Management Algorithm.

Xanthogranulomatous pyelonephritis is an uncommon destructive inflammatory process of the renal parenchyma associated with urinary tract obstruction and infection. Preoperative diagnosis remains challenging with correct identification in less than half of cases. Management paradigms include immediate surgical intervention or staged approaches that emphasize preoperative optimization and percutaneous drainage. Radical nephrectomy remains the de facto definitive treatment though critical evidence gaps underscore the need for more rigorous study to guide management.

Xanthogranulomatous Pyelonephritis and Plummer-Vinson Syndrome: A Case Report Exploring Potential Connections in a Single Patient.

Xanthogranulomatous pyelonephritis (XGP) and Plummer-Vinson Syndrome (PVS) are two rare disorders that pose considerable diagnostic difficulties mainly because their signs overlap and are multifaceted. XGP is a severe form of pyelonephritis imitating cancer, whereas PVS is defined by dysphagia, iron deficiency anemia, and esophageal webs. This article presents the case of a 53-year-old female with a previous history of renal calculi and multiple transfusions who presented with dysphagia, flank pain, and hematuria. Findings from investigations showed severe anemia, a renal lesion suggesting mal

Perioperative outcomes and risk factors for major complications associated with nephrectomy for Xanthogranulomatous pyelonephritis: a multicenter study.

PURPOSE: To determine the risk factors associated with major complications in patients with histologically confirmed Xanthogranulomatous pyelonephritis (XGP) who underwent nephrectomy. METHODS: A multicenter retrospective study was performed including patients who underwent nephrectomy between 2018 and 2022 with histopathological diagnosis of XGP. Clinical and laboratory parameters at the initial presentation were evaluated. Data on extension of XGP was recorded as per the Malek clinical-radiological classification. Characteristics of nephrectomy and perioperative outcomes were obtained. The

Xanthogranulomatous pyelonephritis with polycystic kidney disease as a mimic of cystic renal cell carcinoma: a case report.

BACKGROUND: Xanthogranulomatous pyelonephritis (XGP) is a rare chronic pyelonephritis that often mimics other renal diseases, when combined with autosomal dominant polycystic kidney disease(ADPKD), preoperative diagnosis is exceedingly difficult. It is important for clinicians to be aware of an XGP with ADPKD since a misdiagnosis can lead to unnecessary surgical intervention. CASE PRESENTATION: Here, we report a case of a 66-year-old female with a history of bilateral ADPKD and urinary tract infection admitted to our hospital due to right flank pain, feeble, and low-grade fever. Contrast-enha

A rare case of ureteric involvement in xanthogranulomatous pyelonephritis associated with giant ureteric calculus: An unusual entity.

Xanthogranulomatous pyelonephritis (XGPN) is an uncommon chronic destructive granulomatous inflammation of the kidney with variable clinical and radiological presentation. Due to its similarities to other benign and malignant pathologies, a high index of suspicion is required for preoperative diagnosis, which will ensure appropriate management of this condition. The invasion into the surrounding structures such as pararenal spaces, psoas muscle, small bowel, diaphragm, lung or soft tissues has been reported. However, involvement of ureter is very rarely reported. We report a rare case of left-

An Interesting Finding of Xanthogranulomatous Pyelonephritis Revealed Concomitant Incidental Squamous Cell Carcinoma (SCC) of the Renal Pelvis.

Squamous cell carcinoma, originating in the renal pelvis, is an infrequent form of kidney malignancy. The occurrence rate remains below 1% for all neoplasms in this specific area. Most of these carcinomas are moderately or poorly differentiated, and diagnosis typically occurs at an advanced stage. Xanthogranulomatous pyelonephritis is an uncommon form of severe chronic infection that affects the parenchyma of native kidneys. We present the case of a 34-year-old male with a history of end-stage renal disease secondary to recurrent pyelonephritis, which was incidentally diagnosed as renal squamo

Nephron-sparing management of Xanthogranulomatous pyelonephritis presenting as spontaneous renal hemorrhage: a case report and literature review.

BACKGROUND: Xanthogranulomatous pyelonephritis (XGP) is an uncommon infectious disease of the kidney known to mimic other renal maladies. A rare presentation of this uncommon disease is spontaneous renal hemorrhage (SRH). CASE PRESENTATION: We report a case of XGP in a 58 year old woman who presented with abdominal pain, hematuria, and radiating left flank pain. CT scan was felt to be consistent with perirenal hemorrhage abutting a fat-containing renal mass. The patient was eventually taken to surgery for left partial nephrectomy. Pathology report returned as XGP, and the patient has no compl

A rare case of spontaneous kidney rupture from xanthogranulomatous pyelonephritis presenting with neurological symptoms and septic peritonitis.

Xanthogranulomatous pyelonephritis (XGP) is a rare, chronic inflammatory kidney disorder. It typically arises in patients with a history of nephrolithiasis, urinary tract obstruction, or recurrent infections. Additional risk factors include diabetes and immunocompromised states. We present an unusual instance of XGP in a patient without known risk factors, who developed spontaneous kidney rupture complicated by life-threatening sepsis. This case underscores the importance of maintaining XGP in the differential diagnosis even for atypical presentations, particularly when sepsis is the dominant

Rare coexistence of keratinizing squamous metaplasia with xanthogranulomatous pyelonephritis. Report of a case with the role of immunocytochemistry in the differential diagnosis.

BACKGROUND: Xanthogranulomatous pyelonephritis (XPN), a rare form ofchronicpyelonephritis, is commonly associated with lithiasis and rarely leads to keratinizing squamous metaplasia. Its manifestations closely mimic those of a renal neoplasm, leading to misdiagnosis of malignancy, often resulting in radical nephrectomy. The role of immunocytochemistry in the preoperative cytologic diagnosis is assessed in the present case report. CASE: A 20-year-old male presented with fever and an enlarging mass in the right renal angle. Ultrasonography revealed a heterogeneous mass in the renal pelvis. Fine

Xanthogranulomatous pyelonephritis in children: A contemporary multicentre cohort and the emerging role of parenchyma-sparing and minimally invasive management.

BACKGROUND: Xanthogranulomatous pyelonephritis (XGP) is a rare chronic destructive granulomatous pyelonephritis. Paediatric experience is limited and mostly historical. The largest published series reported nephrectomy in 95.4 %. Whether this radical-only paradigm still reflects best practice in the cross-sectional imaging and minimally invasive surgery era is unclear. MATERIAL AND METHODS: We retrospectively reviewed all paediatric patients (≤16 years) treated for XGP at four tertiary paediatric-surgery centres of Assistance Publique - Hôpitaux de Paris (AP-HP), the university hospital trust

Xanthogranulomatous Pyelonephritis: A Complicated Febrile Urinary Tract Infection Detected by Point-of-Care Ultrasound in the Emergency Department.

BACKGROUND: Febrile urinary tract infections (UTIs) include a spectrum of pathologies from uncomplicated pyelonephritis to urosepsis, including xanthogranulomatous pyelonephritis (XGP). Most febrile UTIs are treated with antibiotics alone, but studies indicate nearly 12% of cases of presumed simple pyelonephritis require emergent urologic intervention. How to identify these individuals, while limiting unnecessary advanced imaging and delays in diagnosis, challenges all emergency providers. We review the diagnosis and management of XGP, as well as the evidence regarding the role of renal ultras

Rare coexistence of keratinizing squamous cell carcinoma with xanthogranulomatous pyelonephritis in the same kidney: report of two cases.

Squamous cell carcinoma of urinary tract is a rarely encountered tumor. It is more frequently reported in urinary bladder and male urethra than renal pelvis. Squamous cell carcinoma of renal pelvis is usually associated with nephrolithiasis. However, coexistence of keratinizing squamous cell carcinoma with xanthogranulomatous pyelonephritis is exceedingly rare with only one case on record so far. We report two such cases detected incidentally in patients who have undergone nephrectomy for hydronephrosis. The post operative histological evaluation revealed unsuspected squamous cell carcinoma of

From Kidney Crisis to Heart Revival: A Case of Septic Cardiomyopathy Following Septic Shock Induced by Xanthogranulomatous Pyelonephritis.

Septic cardiomyopathy (SCM) is a reversible cardiac dysfunction occurring in patients with sepsis or septic shock, characterized by transient impaired myocardial contractility. Despite its prevalence in severe sepsis and significant impact on patient outcomes, SCM remains underdiagnosed and is one of the least studied forms of cardiomyopathy. Its recognition and management are further complicated by the absence of a universally accepted consensus on its diagnostic criteria and definition. We report the case of a 54-year-old female presenting with septic shock secondary to xanthogranulomatous p

Atypical focal xanthogranulomatous pyelonephritis without clinical symptoms presenting as infiltrative renal cancer: a case report and literature review.

BACKGROUND: Xanthogranulomatous pyelonephritis (XGP) is an uncommon form of chronic pyelonephritis. Most patients of XGP are diffuse in radiology and the clinical features are typical. CASE PRESENTATION: We present a case of 24-year-old female with the absence of symptoms and normal laboratory examinations. Contrast computed tomography and intravenous pyelography demonstrate infiltrative renal mass and renal cell carcinoma is presumed. Laparoscopic right radical nephrectomy is performed, but the final pathological result shows XGP. CONCLUSIONS: As far as we know, this is the first case repor
