Thoracic radiculopathy related to collapsed thoracic vertebral bodies.

Three cases are presented of thoracic radiculopathy related to collapsed thoracic vertebral bodies. In all cases proximal weakness of the legs was present, leading to the diagnosis of myopathy in two cases. Sensory symptoms were present in two cases. In one, anterior thigh paresthesias lead to a diagnosis of meralgia paresthetica. This diagnostic entity must be remembered if appropriate corroborative tests are to be performed. In cases of trauma this diagnosis should be considered if thoracic vertebral collapse is present. Conversely, an evaluation of proximal weakness should include a review …

Dystonic Tremor as an Atypical Presentation of Cervical Radiculopathy.

Cervical radiculopathy often presents with neck pain, sensory disturbances, or motor weakness. This case report describes an atypical presentation of cervical radiculopathy manifesting as dystonic tremor and brachialgia, which has been documented only once previously. A 46-year-old, right-handed, male delivery worker presented with severe neck pain that gradually evolved into a dystonic tremor and sharp aching pain across his right shoulder. Despite medical intervention, his symptoms persisted, leading him to seek chiropractic care. Physical examination and diagnostic tests revealed degenerati…

Application of algometry in patients with cervical and lumbar radiculopathy.

BACKGROUND: Algometry, as a highly sensitive method, provides an objective insight into the degree of pain, while the use of questionnaires can estimate the patient's psychological status in a simple way. OBJECTIVE: This study was conducted in order to measure the pressure pain threshold in patients with cervical and lumbar radiculopathy and to find a possible association of pain with the anxiety and depression. METHODS: The study examined 60 hospitalized patients with cervical radiculopathy and 60 patients with lumbar radiculopathy before starting and after finishing kinesitherapy. The rese…

Phantom Radiculopathy: Case Report and Review of the Literature.

BACKGROUND: Phantom radicular pain is very uncommon. To the best of our knowledge, only 14 cases have been described in the literature. A review of the literature revealed the most common cause of phantom radicular pain to be lumbar disc herniation and, furthermore, that treatment with epidural steroid injection or surgical decompression relieves pain in almost all cases. A significant number of patients with superimposed phantom radiculopathy may be missed because of the high incidence of degenerative lumbar spine diseases in the adult population, as well as the fact that amputee patients ver…

The value of provocative tests in diagnosis of cervical radiculopathy.

BACKGROUND: This study was aimed at assessing the accuracy of provocative tests in diagnosis of acute or chronic Cervical Radiculopathy (CR) based on an electrodiagnostic reference criterion. MATERIALS AND METHODS: Shoulder Abduction Test (SAT), Spurling Test (ST), Upper Limb Tension Test (ULTT), and electromyography were done on 97 patients who referred to Electrodiagnostic center in the university hospital from January 2010 to March 2011. All of the participants had neck and radicular pain for at least 3 weeks. They were classified according to electrodiagnostic findings. Then diagnostic va…

Vertebral artery dissection as a cause of cervical radiculopathy.

The acute onset of neck pain and arm weakness is most commonly due to cervical radiculopathy or inflammatory brachial plexopathy. Rarely, extracranial vertebral artery dissection may cause radiculopathy in the absence of brainstem ischemia. We describe a case of vertebral artery dissection presenting as cervical radiculopathy in a previously healthy 43-year-old woman who presented with proximal left arm weakness and neck pain aggravated by movement. Cervical magnetic resonance imaging (MRI) and angiography revealed dissection of the left vertebral artery with an intramural hematoma compressing…

Surgical treatment of C3 and C4 cervical radiculopathies.

STUDY DESIGN: Case series study. OBJECTIVE: To report the results of surgical intervention in a series of patients with high cervical radiculopathy. SUMMARY OF BACKGROUND DATA: Although midcervical (C5-C7) radiculopathy is common and well recognized, high cervical (C3 and C4) radiculopathy is relatively rare and can be missed clinically. To our knowledge, there are few reports regarding the operative treatment of high cervical radiculopathy. METHODS: Two spine surgeons independently reviewed the charts and radiographs of all patients with high cervical radiculopathy or myeloradiculopathy th…

Mechanical traction for neck pain with or without radiculopathy.

BACKGROUND: Neck pain is a frequently reported complaint of the musculoskeletal system which can be disabling and costly to society. Mechanical traction is often used as an adjunct therapy in outpatient rehabilitation. OBJECTIVES: To assess the effects of mechanical traction for neck disorders. SEARCH STRATEGY: A research librarian searched computerized bibliographic databases without language restrictions up to March 2008 for randomized controlled trials (RCTs) from the medical, chiropractic, and allied health literature. SELECTION CRITERIA: The RCTs we selected examined adults with neck d

[Critical approach to diagnostics and treatment of lumbar radiculopathy].

Although the majority of patients suffering lumbar radiculopathy have a very good prospective outcome, some 20-30% persist having problems even in two or three years time. Diagnosis is based on anamnesis and physical examination. Imaging screening with additional diagnostic methods is indicated only in patients with the extremely complicated illnesses, or in cases where the surgical intervention is inevitable. Passive (bed rest) treatment is replaced by active one. In general, there is a consensus that the initial treatment during 6-8 weeks has to be conservative. Surgical intervention of disc…

Routine electrodiagnosis and a multiparameter technique in lumbosacral radiculopathies.

OBJECTIVE: This study compares Routine nerve conductions studies (NCS)/needle electromyography (nEMG) with a multiparameter recording method (NC-stat; NeuroMetrix Inc., Waltham, MA, USA) in patients with lumbosacral radiculopathies (LSR). METHODS: Charts from 34 consecutive patients with a clinical history and/or examination consistent with an LSR were retrospectively reviewed. All underwent both Routine NCS/nEMG studies and NC-stat EDX. NC-stat testing included peroneal and posterior tibial nerve distal motor latencies and amplitudes and F-wave analysis. Twenty-eight patients had magnetic re

A ligamentum flavum hematoma presenting as an L5 radiculopathy.

This study reports two patients with ligamentum flavum hematoma, of which only seven cases have been reported in the literature. Two elderly male patients (74 and 80 years) presented with a history of chronic lumbar strain and effort. They had low back pain radiating to both legs. Their neurologic examination findings were consistent with left L5 root compression. Magnetic resonance imaging showed an epidural mass lesion at L4-L5 that was continuous with the ligamentum flavum. After removal of the hematoma, the symptoms completely resolved. The most common cystic lesion in the lumbar spine is

[Indications for epidural steroids in back pain and in radiculopathy].

Back pain is very frequent in western countries and it represents a very high social cost. The treatment is based on five modalities: medication, physiotherapy, invasive technics, rehabilitation programs and psycho-social approaches. Several treatments may be proposed simultaneously and introduced gradually. Steroid administration by epidural injection is frequently used in Belgium, even though no prospective randomised studies have shown a real benefit over the long term. The indications for epidural injection must be carefully chosen. These include subacute pain (less than six months) in the

Diabetic cervical radiculopathy with adhesive capsulitis of the shoulder.

The common form of diabetic neuropathy is symmetrical peripheral polyneuropathy, which involves the distal part of the lower extremities whereas diabetic amyotrophy is seen in the proximal part of the lower extremities. Although other regions may also be affected, the presence of upper extremity involvement has rarely been emphasized. Diabetic radiculopathy may involve the cervical region before, after, or concurrently with lumbosacral radiculopathy. We report 2 rare cases of diabetic radiculopathy which involves the cervical region without involving the lumbosacral region. To our knowledge, t…
