[Curative treatment of paroxysmal atrial fibrillation with radiofrequency ablation].

BACKGROUND: Atrial fibrillation is associated with increased morbidity and twice the mortality compared to individuals without fibrillation. Treatment with antiarrhythmic drugs has limited effect in paroxysmal atrial fibrillation. MATERIAL AND METHOD: The group of patients comprised 59 men and 13 women with an average age of 51 +/- 10, the majority of whom had failed several drug regimens; some had undergone repeated DC conversions. A new method based on radiofrequency ablation and isolation of pulmonary veins from the left atrium may offer curative treatment for paroxysmal atrial fibrillatio…

Post-pericardiotomy syndrome following linear left atrial radiofrequency ablation.

Post-pericardiotomy syndrome may occur after traumatic insults to the pericardium but has not been reported after radiofrequency catheter (RF) ablation. A 54 year old man underwent extensive linear left atrial RF ablation for chronic atrial fibrillation. Five days after the procedure the patient developed signs and symptoms of the post-pericardiotomy syndrome and showed new, intense pericardial inflammation on magnetic resonance imaging. After intensive medical management, the patient recovered fully. It is believed that the patient experienced a unique complication of linear left atrial ablat

Secondary hemocholecyst after radiofrequency ablation therapy for hepatocellular carcinoma.

A 65-year-old Japanese man underwent radiofrequency ablation (RFA) therapy of a hepatocellular carcinoma. Hemobilia from the intrahepatic bile ducts adjacent to the tumor developed on the fifth day after the RFA therapy. Ultrasonograms and computed tomograms showed swelling of the gallbladder, which was filled with a clot, suggesting the diagnosis of hemocholecyst. The hemobilia resolved with conservative therapy, but a cholecystectomy was performed to manage postprandial abdominal pain. The resected gallbladder was filled with a clot, but injury or ulceration of the gallbladder was absent, su…

Radiofrequency ablation of atrial fibrillation during mitral valve surgery.

Twenty-three patients underwent endocardial radiofrequency ablation of atrial fibrillation (AF) during mitral valve procedures with a previously described left atrial lesion pattern. A temperature-controlled 7-coil surgical probe delivered 60-second lesions at 80 degrees C. The left atrial appendage was oversewn after ablation. Ages ranged from 28 to 88 years. Nineteen patients had chronic AF that was present for over 1 year in 74%; 12 patients had rheumatic mitral stenosis. Mean left atrial diameter was 5.4 +/- 0.7 cm. There was 1 operative death unrelated to the ablation, and no strokes or a…

Radiofrequency catheter ablation in pediatrics: experience at Siriraj Hospital.

UNLABELLED: Tachyarrhythmia is one of the life threatening cardiac electrophysiology problems in children. It also affects quality of life of the patients. Radiofrequency catheter ablation (RFCA) has made a significant impact in the treatment of tachyarrhythmia since 1989. The present report is the first and largest report in Thai children. There have been 24 RFCA procedures in 21 children since it was initially performed at Siriraj Hospital from January 1996 to December 1999. The electrophysiology studies and medical records were analyzed retrospectively. Median age and weight at the time of

Predictors of successful radiofrequency catheter ablation of sinoatrial tachycardia.

Predictors of successful elimination of sinoatrial tachycardia (SAT) using radiofrequency current (RFC) were investigated in this report. Within 1991-1996 fourteen patients with SAT were subjected to electrophysiological study and radiofrequency catheter ablation (RFCA). Ten patients had sinoatrial reentrant tachycardia (SART), and four patients had chronic non-paroxysmal sinoatrial tachycardia (CNPSAT). The RFC (15-30 W, duration 10-30 sec) were applied during tachycardia in case of CNPSAT, and during sinus rhythm (SR) in case of SART. In 3 patients with SART RFC were delivered during tachyca

[Radiofrequency catheter ablation of atrial flutter and atrial fibrillation].

Radiofrequency catheter ablation is now considered as a curative approach in patients with typical atrial flutter. Typical atrial flutter is due to a macrore-entrant circuit within the right atrium and it can be eliminated by a linear lesion in the isthmus between the tricuspid annulus and the vena cava inferior. The electrophysiological criterion of a bidirectional isthmus block has been shown to reduce the recurrence rate of atrial flutter after catheter ablation, thus achieving long-term cure of typical atrial flutter. Acute success rates of 85 to 90% and recurrence rates of 10 to 15% have

Catheter ablation of the atrioventricular node using radiofrequency energy.

BACKGROUND: Catheter ablation of the atrioventricular (AV) junction using stored direct current (DC) energy from a standard DC Cardioverter defibrillator was first reported in 1982. Since then many patients have been treated using this procedure for refractory supraventricular arrhythmias, usually atrial fibrillation and flutter. Undesirable thermal effects such as barotrauma and arcing are largely responsible for complications associated with the use of DC energy. This report details our experience of catheter ablation of the AV junction using radiofrequency (RF) energy in a series of 30 cons

Radiofrequency catheter ablation of atrial arrhythmias. Results and mechanisms.

BACKGROUND: Radio frequency catheter ablation is accepted therapy for patients with paroxysmal supraventricular tachycardia and has a low rate of complications. For patients with atrial arrhythmias, catheter ablation of the His bundle has been an option when drugs fail or produce untoward side effects. Although preventing rapid ventricular response, this procedure requires a permanent pacemaker and does not restore the atrium to normal rhythm. Therefore, we evaluated the safety and efficacy of radiofrequency ablation directed at the atrial substrate. METHODS AND RESULTS: Thirty-seven patients…

[Radiofrequency ablation of accessory pathways in pre-excitation syndrome].

Various parameters relating to the radio-frequency ablation of accessory pathways were studied in 53 patients (27 males, 26 females: mean age 38.5 [14-64] years) with a history of paroxysmal tachycardia (over 1 month to 50 years), shown to be caused by an accessory pathway (Wolff-Parkinson-White syndrome). In all patients the following values were obtained: (1) number of procedures necessary to achieve permanent blockage of the accessory pathway (1-4); (2) duration of each procedure (45-420 min); (3) duration of fluoroscopy (5-102 min); (4) number of necessary radio-frequency applications (1-4

Successful radiofrequency current catheter ablation of sustained ventricular tachycardia.

We performed radiofrequency current catheter ablation in two patients with nonischemic sustained ventricular tachycardia (VT). In one patient, two morphologically distinct VTs were induced by electrical stimulation. One showed right bundle branch block pattern and the other left bundle branch block pattern. The earliest site of activation during each VT was determined at the septum of the right ventricle. However, these two sites were close to the His-bundle electrogram recording area. In the other patient, a VT with a left bundle branch block pattern occurred spontaneously after the administr

Awake Endoscopic Radiofrequency Ablation for Lateral Atlantoaxial Joint Pain.

Chronic pain originating from the lateral atlantoaxial (C1-C2) joint is a challenging condition with limited long-term treatment options. Conventional radiofrequency ablation (RFA) techniques rely on indirect guidance, which can lead to incomplete nerve denervation and variable outcomes. This report introduces awake endoscopic radiofrequency ablation (ERFA) as a novel, minimally invasive treatment, complemented by advanced diagnostic imaging. We present the case of a 67-year-old male with intractable occipital and posterior neck pain refractory to previous surgeries. Single-photon emission com…
