The use of electro-acoustic impedance measurements in detecting early clinical otosclerosis.

The first evidence that sodium fluoride (NaFl) can stop the otosclerotic process was recently presented. This development has placed new emphasis on the early detection of clinical otosclerosis. Electro-acoustic impedance measurements often detect minute changes in absolute impedance and compliance of the ossicular chain. The most valuable diagnostic information, however, is a negative on-off (biphasic) type of acoustic reflex. These results are often evident prior to the detection of positive clinical signs of otosclerosis. The negative on-off acoustic reflex is reviewed in this paper along w

[Experimental studies on the suppression of the stapedius reflex in comparison with EMG-results in idiopathic facial paresis].

The recording of intra-aural muscle reflexes elicited by acoustic stimulation is a routine method in clinical audiology. The most important quantitative examinations of the reflex dynamic, i.e. Metz recruitment and reflex decay in low frequencies, allow the judgement of function of the afferent pathway in the stimulated inner ear and acoustic nerve. Quantitative determinations of the efferent branch were carried out by several authors in a few normal subjects and in several patients with multiple sclerosis or facial palsy. Absolute acoustic impedance - a method yielding wide scattering - was u

Cochlear otosclerosis: a review of audiometric findings in 150 cases.

Audiometric findings and histories were reviewed for 150 patients diagnosed by polytomography as probable cochlear otosclerosis cases. Most patients were under 65 and had a relatively short duration of hearing impairment. Most had a mild to moderate loss with excellent speech discrimination. Seventy-five percent of the ears had acoustic reflex findings indicative of minimal stapes fixation. We recommend the acoustic reflex test for identification of patients with probable cochlear otosclerosis.

A multifrequency method for determining cochlear efferent activity.

A test based on measures of distortion-product otoacoustic emissions (DPOAEs) was developed in lightly anesthetized guinea pigs and alert rabbits to assess the effective activation or functional "strength" of the cochlear efferent system. The multifrequency method described here used the DP-gram frequency function to evaluate the fast component of the olivocochlear adaptive effect on DPOAE levels over a 2-octave frequency range. An estimate of any concurrent muscle activation was also determined over the identical frequency range by monitoring the levels of the eliciting f1 primary tone throug

An alternate method for determining functional gain of hearing aids.

The functional gain of a hearing aid typically is determined by comparing aided and unaided behavioral thresholds. With this method, however, true gain may be underestimated in frequency regions of normal or near-normal hearing sensitivity (i.e., in cases of sloping, rising, or trough-shaped audiograms). Internal hearing-aid noise and/or amplified room noise imposes a lower limit on obtainable aided thresholds. In these cases, comparing aided and unaided acoustic-reflex thresholds may be a valuable clinical alternative to traditional means of determining real-ear gain. This study compared soun

Evidence for adaptive plasticity in elderly monaural hearing aid users.

Hearing aids can induce perceptual changes in some elderly listeners but few studies have investigated physiological changes in this population. Loudness discomfort levels and acoustic reflex thresholds were measured in long-term users of a single hearing aid. The results show an asymmetry of +2 to +9 dB between ears in the sound level that (i) causes loudness discomfort and (ii) elicits a reflex contraction of the middle ear muscles. The elevation of the acoustic reflex threshold occurs in the ear with hearing aid experience, irrespective of the ear of stimulation. Therefore, there is evidenc

Acoustic immittance in children without otoacoustic emissions.

UNLABELLED: Considering the hypothesis that middle ear changes can impair the recording of otoacoustic emissions, it is possible that absent otoacoustic emissions in infants could be associated with a light tympanometric change. AIM: To study the association between transient otoacoustic emissions and changes in acoustic immittance measurements with 226 Hz probe tone in neonates. METHODS: Cross-sectional contemporary cohort study. 20 infants with no transient otoacoustic emissions (study group) and 101 infants with transient otoacoustic emissions (control group), with ages ranged from birth

Stapedius reflexes to electrical stimulation in the rabbit for the assessment of hearing.

The possibility of using electrically induced stapedius reflexes as a means of objective hearing evaluation was investigated in the rabbit as an animal model. The contralateral stapedius reflex to acoustic and electrical stimulation was measured in anesthetized rabbits. For electrical stimulation, the middle ear was opened surgically and stimulation was applied by a monopolar electrode placed at or into the round window. Contraction of the middle ear muscle was monitored by digital sampling and averaging of the impedance changes in the contralateral ear. Acoustically evoked reflexes were recor

Effects of contralateral acoustic stimulation on otoacoustic emissions following vestibular neurectomy.

This study demonstrates that, following unilateral vestibular neurectomy, the inhibitory effect of contralateral acoustic stimulation on evoked otoacoustic emissions is absent. The patient acts as her own control in that the unoperated side shows normal suppression of otoacoustic emission amplitude with contralateral acoustic stimulation. The lack of interaural suppression of otoacoustic emissions on the sectioned side, in the presence of normal acoustic reflex threshold levels, provides evidence that observed phenomena are not merely a function of middle ear reflex activity. It is concluded t

[Significance of dynamic measurements of acoustic impedance for early detection of hypoesthesia in otosclerosis].

To clarify the role of dynamic registration of acoustic impedance in early detection of hypoacusis in otosclerosis, the authors analyzed impedometry readings in subjects with unilateral otosclerosis. Special emphasis is laid on cases with minor audiogram bone-air intervals. Based on comparison of contra- and ipsilateral reflexes, registration of the acoustic reflex may provide early signs of otosclerosis.

[The normal value and clinical applications of the blink reflex].

OBJECTIVE: To study the normal value and clinical applications of blink reflex. METHODS: Using the clinical electrodiagnostic techniques, the orbital nerve was stimulated to elicit the blink reflex. RESULTS: In 20(40 sides) normal subjects, two components (R1 and R2) had been documented. The latencies were about 10.1 ms for R1, 30.4 ms and 30.7 ms separately for R2 and R2'. In 30 patients with trigeminal and facial nerve diseases and acoustic neuromas, the latencies on the affected sides were apparently longer or the reflex disappeared (P < 0.01). CONCLUSION: This study indicates that the s

[Effects of diazepam on stapedius reflex: physiological research].

The effect of diazepam (10 mg i.m. X 7 days) on crossed and uncrossed acoustic stapedius reflex parameters (threshold, amplitude and duration) has been studied in 7 normal hearing subjects. The results showed that diazepam has a not clinically significant damping effect on the threshold, the amplitude and, very often, the duration of both ipsilateral and contralateral acoustic stapedius reflexes.

Intraoperative recordings of electromyogenic responses from the human stapedius muscle.

Following surgical implantation of a cochlear implant, the external processor must be programmed individually, which is typically achieved by behavioral measurements, with the user indicating the perceived loudness between the threshold and the maximum comfort levels (MCLs). However, the stapedius reflex measurement could be used instead, as the basis for programming the fitting maps. The electrically evoked stapedius reflex threshold is known to have a high correlation with the MCLs and thus is used as an objective tool to determine the MCL, especially in children and non-cooperative patients

The stapedial reflex as a topographical marker of proximal involvement of the facial nerve in leprosy. A pilot study.

This study aimed to determine the parameters necessary for a study of stapedial reflexes in leprosy patients to ascertain if the facial nerve is involved more proximally than the stylomastoid foramen. It involved leprosy patients with and without facial nerve involvement and non-leprosy controls. Clinical examination of the patients' ears, a tympanogram and audiogram to exclude conductive and sensorineural deafness, followed by the measurement of a stapedial reflex and the acoustic reflex threshold, were carried out. The number of absent reflexes and the acoustic reflex thresholds did not diff

Induction of enhanced acoustic startle response by noise exposure: dependence on exposure conditions and testing parameters and possible relevance to hyperacusis.

There has been a recent surge of interest in the development of animal models of hyperacusis, a condition in which tolerance to sounds of moderate and high intensities is diminished. The reasons for this decreased tolerance are likely multifactorial, but some major factors that contribute to hyperacusis are increased loudness perception and heightened sensitivity and/or responsiveness to sound. Increased sound sensitivity is a symptom that sometimes develops in human subjects after acoustic insult and has recently been demonstrated in animals as evidenced by enhancement of the acoustic startle
