A self calibration method using a soft clustering procedure for eye movement recordings.

A nearly automatic method for calibrating eye movement records has been developed. This very robust method is based on a soft clustering algorithm which allows exploration of the whole range of eye movement records for reliable calibration. In contrast to many other methods which carry out the calibration on several discrete points, this method is suitable for continuous determination of the transfer function of the eye movement transducer. Moreover it simultaneously uses the combined properties of vestibulo-ocular reflex, neck-to-eye reflex and smooth pursuit system to reach approximately a u

Video ocular counter roll: A bedside test of otolith-ocular function.

Here we review the clinical value of a video-oculography test for clinical evaluation of vestibular otolith function. This test is known as the video ocular counter roll (vOCR) and is based on measurement of torsional vestibulo-ocular reflex with a lateral head tilt. The vOCR test consists of a simple maneuver during which the head and torso are tilted en bloc by the examiner. The pattern of vOCR deficit among patients highlights its clinical value in identifying the stage of vestibular loss and recovery. The quick application of vOCR allows examination of otolith-ocular function and assessmen

Horizontal eye movement disorders after posterior vermis infarctions.

The horizontal saccade, smooth pursuit, and vestibulo-ocular reflex gains were recorded in 19 patients with cerebellar infarction documented with MRI, and in a group of control subjects. Bilateral saccade hypometria and a decrease in ipsilateral smooth pursuit gain were found only in patients with a lesion affecting the posterior vermis. These results in humans support experimental findings suggesting that the posterior vermis controls both saccade accuracy and smooth pursuit velocity.

Transient upbeat nystagmus due to unilateral focal pontine infarction.

Upbeat nystagmus is known to be caused by an imbalance of the vertical vestibulo-ocular reflex (VOR), favoring downward VOR activity, due to bilateral lesions of the medulla, ventral tegmentum, anterior cerebellar vermis, adjacent brachium conjunctivum and the midbrain. We report on two patients who had transient upbeat nystagmus due to unilateral pontine infarction that may have disrupted bilateral upward VOR pathways running in the ventral tegmental tracts.

Niemann-Pick type C disease in two affected sisters: ocular motor recordings and brain-stem neuropathology.

Two sisters with Niemann-Pick disease type C were examined: the brain in one sister, who had died, was examined, and eye movements in the other, surviving sister were recorded. Ocular motor recordings showed marked slowing of vertical saccades with relative sparing of horizontal saccades, pursuit, and the vestibulo-ocular reflex. Neuropathological findings included glial fibrillary lesions in the area of the posterior commissure and neuronal loss in the rostral interstitial nucleus of the MLF with preservation of the interstitial nucleus of Cajal and ocular motor complex. These neuropathologic

Oculomotor abnormalities in chronic GM2 gangliosidosis.

The clinical and electro-oculographic eye abnormalities in chronic GM2 gangliosidosis have been described. The most prominent oculomotor disturbances in our patients involved the pursuit system. This was evident during performance of eye tracking and vestibulo-ocular reflex suppression. Saccadic eye movements were dysmetric but were normal in velocity. These findings point to disturbances of cerebellar oculomotor control. While the oculomotor defects are nonspecific in this chronic form of beta-hexosaminidase A deficiency, they broaden the spectrum of clinical features considered to be typical

Static cervico-ocular reflex in healthy humans.

BACKGROUND: Dynamic cervico- (COR) and vestibulo-ocular reflex (VOR) contribute to stabilise visual images in the retina. The gain in dynamic COR is small in healthy individuals but increases in patients with vestibular dysfunction. Conversely, static COR has not been directly observed in healthy individuals. OBJECTIVES: To elucidate the presence of static COR and quantify it in normal individuals in the roll plane. METHODS: Eleven healthy participants were included in the study. Eye torsions were measured using video oculography to evaluate the static COR induced by lateral neck flexion dur

Astronauts eye-head coordination dysfunction over the course of twenty space shuttle flights.

BACKGROUND: Coordination of motor activity is adapted to Earth's gravity (1 g). However, during space flight the gravity level changes from Earth gravity to hypergravity during launch, and to microgravity (0 g) in orbit. This transition between gravity levels may alter the coordination between eye and head movements in gaze performance. OBJECTIVE: We explored how weightlessness during space flight altered the astronauts' eye-head coordination (EHC) with respect to flight day and target eccentricity. METHODS: Thirty-four astronauts of 20 Space Shuttle missions had to acquire visual targets wi

Bilateral Vestibular Dysfunction.

Bilateral vestibular dysfunction (BVD) refers to hypofunction of the vestibular nerves or labyrinths on both sides. Patients with BVD present with dizziness, oscillopsia, and unsteadiness, mostly during locomotion, which worsen in darkness or on uneven ground. Although aminoglycoside ototoxicity, Meniere's disease, infection, and genetic disorders frequently cause BVD, the etiology remains undetermined in up to 50% of the patients. The diagnosis of BVD requires both symptoms and documentation of deficient vestibulo-ocular reflex function using head-impulse, bithermal caloric, and rotatory chai

Loss of Vestibular Ocular Reflex in Nonconvulsive Status Epilepticus.

BACKGROUND: Electroencephalogram (EEG) findings of generalized periodic discharges (GPDs) with triphasic morphology were introduced as a metabolic phenomenon, but more recently have been associated with epileptic phenomenon. Resolution of EEG findings along with clinical improvement from treatment is diagnostic. The known causes of reversible, isolated loss of OVR include medication toxicity, lead exposure, and thiamine deficiency, but its association with nonconvulsive status epilepticus (NCSE) has never been published. Medication induced loss of OVR resolves after a 24-hour washout period. W

Multi-system signs and symptoms in X-linked ataxia carriers.

Neurological, auditory, vestibular and ocular motor examinations were performed on 3 definite and 3 possible heterozygous carriers of a previously described X-linked multi-system disorder with early childhood onset, rapid progression and a fatal outcome (Arts et al., 1993). The symptoms, i.e., delayed motor development, ataxia, hearing loss and subnormal intelligence, were so evident in 2 of the possible carriers that they could be redesignated as probable carriers. Other symptoms in the definite and probable carriers were clubfeet, dysarthria, intention tremor and abnormal gait, while their s

Investigation of vestibular damage by antituberculous drugs.

Vestibular function testing was performed regularly on patients who were administered streptomycin, kanamycin, or enviomycin, and vestibular damage was detected at an early stage, and quantitatively. We investigated the point in time at which the therapy should be discontinued. Subjects consisted of 204 cases of tuberculosis treated with streptomycin, kanamycin, enviomycin. They were admitted to the hospital between December 1984 and August 1989. Twenty-eight cases of vestibular dysfunction due to streptomycin, kanamycin, and enviomycin could easily be detected at an early stage by performing

Phenytoin-induced ophthalmoplegia.

Total external ophthalmoplegia was observed in five patients consequent to the oral or intravenous administration of phenytoin. Coincident with the ophthalmoplegia, the state of consciousness varied from drowsiness to coma and the blood levels of phenytoin ranged from 36 to 55 mug per milliliter. Initially, the eyes were fixed in midposition, and oculocephalic and oculovestibular stimulation failed to evoke either horizontal or vertical eye movements. The return of vestibulo-ocular responsiveness lagged behind the return of consciousness and other reflex activity. The mechanism underlying this

Familial congenital vestibular areflexia.

Three cases in one family are presented with oscillopsia due to vestibular areflexia, but without hearing loss. There was no history of other neurological or otological diseases (including infectious diseases) or use of neuro-ototoxic drugs. Laboratory tests, including tests for autoimmune diseases, were undisturbed. Petrosal radiographs and brain CT scans were normal. The pedigree suggested autosomal recessive inheritance.

[Characteristics of vestibulo-ocular interactions in patients with unilateral lesions of the peripheral part of the vestibular system. Sinusoidal rotation. 3].

In 40 patients with unilateral injury of the peripheral compartment of the vestibular system in the stage of decompensation (Group 1) and subdecompensation (Group 2), the vestibulo-ocular reflex (VOR) was investigated during sinusoidal rotation with the eyes closed. Group 1 patients showed a significant asymmetry of the VOR coefficient and a distinct shift of the VOR displacement towards injury during sinusoidal stimulation of any intensity. Group 2 patients displayed similar but less significant changes.

Eye Movements in Mild Traumatic Brain Injury: Ocular Biomarkers.

Mild traumatic brain injury (mTBI, or concussion), results from direct and indirect trauma to the head (i.e. a closed injury of transmitted forces), with or without loss of consciousness. The current method of diagnosis is largely based on symptom assessment and clinical history. There is an urgent need to identify an objective biomarker which can not only detect injury, but inform prognosis and recovery. Ocular motor impairment is argued to be ubiquitous across mTBI subtypes and may serve as a valuable clinical biomarker with the recent advent of more affordable and portable eye tracking tech
