Acute Flaccid paralysis in adults: Our experience.

UNLABELLED: Acute flaccid paralysis (AFP) is a complex clinical syndrome with a broad array of potential etiologies that vary with age. We p … tions. We did not encounter any case of acute polio mylitis in adults. In-hospital mortality due to respiratory paralysis was 9%. CONCLUSION: Neuroparalytic snakebite and Guillain Barre syndrome were the most common causes of acute flaccid paralysis in adults in our study.

Bell Palsy: Facts and Current Research Perspectives.

Bell palsy is a non-progressive neurological condition characterized by the acute onset of ipsilateral seventh cranial nerve paralysis. People who suffer from this type of facial paralysis develop a droop on one side of their face, or sometimes both. This condition is distinguished by a sudden onset of facial paralysis accompanied by clinical features such as mild fever, postauricular pain, dysgeusia, hyperacusis, facial changes, and drooling or dry eyes. Epidemiological evidence suggests that 15 to 23 people per 100,000 are affected each year, with a recurrence rate of 12%. It could be caused

Fatal self-envenomation in a brown tree snake, Boiga irregularis, from south-east Queensland.

The case history and clinical signs of a fatal self-envenomation event by a brown tree snake, Boiga irregularis, in South-east Queensland, Australia, are presented. Clinical signs began 20 minutes post-envenomation with muscle twitching, ataxia, and heat seeking behavior which progressed to partial paralysis by 6 hours, generalised paralysis and respiratory arrest at 10 hours and cardiac arrest by 12 hours post-envenomation. Clinical signs are suggestive of potent neurotoxicity for B. irregularis to its own venom.

Phrenic nerve injury following blunt trauma.

Phrenic nerve trauma in the absence of direct injury is unusual and may present diagnostic difficulty. Diaphragmatic paralysis resulting from phrenic nerve injury may closely mimic diaphragmatic rupture. This case highlights the value of magnetic resonance imaging in establishing diaphragmatic integrity and of ultrasonographic assessment during respiratory excursion in confirming diaphragmatic paralysis. In cases of non-contact injury involving torsional injury to the neck, an index of clinical awareness may help to establish the diagnosis of phrenic nerve trauma.

[Bilateral diaphragmatic plication for an adult patient].

Bilateral phrenic nerve paralysis is a very rare complication in open-heart surgery. An 65-year-old woman sustained respiratory distress after coronary artery bypass grafting because of bilateral phrenic nerve paralysis. Bilateral diaphragmatic plication was performed on the 43rd postoperative day. She successfully weaned from ventilator support without difficulty a few days after plication. We believe that surgical plication of the diaphragm is a safe and effective technique for a distressed adult patient with paralyzed diaphragm.

Imaging diagnosis--acquired pectus excavatum secondary to laryngeal paralysis in a dog.

A 13-year-old neutered female Labrador retriever had inspiratory dyspnea secondary to bilateral laryngeal paralysis. Radiographically, there was pectus excavatum with a mediastinal shift to the right. Arytenoid lateralization was performed, relieving the upper respiratory obstruction, and the sternal deformity also resolved. Chronic upper respiratory obstruction should be considered in dogs with pectus excavatum. Furthermore, correction of upper respiratory obstruction has the potential to result in resolution of pectus excavatum.

Three cases of wood-lover paralysis: clinical insights from Victoria, Australia.

INTRODUCTION: Wood-lover paralysis is a rare complication of Psilocybe mushroom ingestion. The clinical spectrum is poorly characterized and published cases remain limited. CASE PRESENTATIONS: We describe three patients who developed acute muscle weakness following ingestion of Psilocybe mushrooms in Victoria, Australia. Written informed consent was obtained from these patients for publication of this case series. • CASE 1: Mild generalized weakness affecting gait, without other end-organ involvement. • CASE 2: Severe generalized weakness involving upper and lower limbs, associated with tak

Non-invasive Ventilation in Diaphragmatic Paralysis Associated With Neonatal Brachial Plexus Palsy.

Obstetric brachial plexus injury (OBPI) is an uncommon yet well-recognized complication of childbirth, occasionally accompanied by phrenic nerve involvement, causing diaphragmatic paralysis. Management of these cases often requires advanced respiratory support, with limited reports describing the use of early non-invasive ventilation (NIV) as first-line therapy. We report a full-term female neonate, delivered by vacuum-assisted birth complicated by shoulder dystocia, who developed right upper limb flaccid paralysis, Horner syndrome, and respiratory distress at birth. Imaging confirmed right he

Acquired paralysis of the diaphragm.

Acquired DP in adults is multifactorial. Symptoms largely depend on whether or not the paresis is unilateral or bilateral and on the etiology, onset, progression, and presence of underlying pulmonary disease. UDP is a well-tolerated condition with a favorable prognosis. Diaphragmatic plication is indicated in carefully selected patients with UDP and can be done by minimally invasive techniques. Patients with BDP are usually severely symptomatic and require some form of mechanical ventilatory support. Diaphragmatic pacing is an option for a select group of patients with BDP in centers with spec

Vocal fold motion impairment in multiple system atrophy.

OBJECTIVES/HYPOTHESIS: Multiple system atrophy (MSA) is a degenerative neurologic disorder that can affect vocal fold mobility. Our aim was to further elucidate the impact of vocal fold motion impairment (VFMI) in MSA. STUDY DESIGN: Retrospective case series. METHODS: We undertook a retrospective review of all MSA patients reporting voice or respiratory symptoms from 1975 to 2010 at Mayo Clinic Rochester who also received otolaryngologic examination. RESULTS: Thirty-eight MSA patients demonstrated VFMI. Median duration of vocal or respiratory symptoms prior to diagnosis of MSA was 12.0 mont

The management of respiratory insufficiency in patients with ALS at or near the end of life.

Amyotrophic lateral sclerosis (ALS) is a devastating motor neuron disease causing progressive paralysis and eventual death, usually from respiratory failure. Treatment for ALS is focused primarily on optimal symptom management because there is no known cure. Respiratory symptoms that occur are related to the disease process and can be very distressing for patients and their loved ones. Recommendations on the management of respiratory insufficiency are provided to help guide clinicians caring for patients with ALS.

Complications and long-term results after partial laryngectomy for the treatment of idiopathic laryngeal paralysis in 45 dogs.

Forty-five dogs with severe respiratory signs caused by idiopathic, acquired laryngeal paralysis were treated by partial laryngectomy. The predominant postoperative complications were coughing in 28 dogs and pneumonia in 15 dogs. Eight dogs required a second operation to alleviate persistent or recurrent upper airway obstruction. Results of surgery were considered excellent in 11 dogs (25%), good in 18 dogs (40%), fair in 6 dogs (13%), and poor in 10 dogs (22%). Poorer results were obtained by surgical residents than by more experienced surgeons. Death in the immediate postoperative period was

Progressive bulbar paralysis of childhood. A reappraisal of Fazio-Londe disease.

Fazio-Londe disease is a label sometimes applied to a degenerative disease of the motor neurons characterized by progressive bulbar paralysis in children. It is very rare with only 22 case reports describing 24 children including four sibling pairs. In two reports mothers and sons were affected. The neuropathology is described in only four cases. Previous authors have recognized that the condition is very heterogeneous. The clinical features of five children with this type of progressive bulbar paralysis, diagnosed at this hospital between 1969 and 1989, are reviewed, and in two cases neuropat

Acute flaccid myelitis caused by enterovirus D68: Case definitions for use in clinical practice.

Acute flaccid myelitis (AFM) was increasingly detected in recent years, coinciding with upsurges of enterovirus D68 (EV-D68) infections. We reviewed the evidence for a causal relationship between both. Based on reported cases, we provide case definitions for AFM caused by EV-D68 infections to enable a standard procedure for affected patients. Current case definitions are focussing on epidemiological aspects but clinical case definitions are still missing. We propose the following case definitions to be used in clinical practice in order to mirror clinical realities and facilitate a common syst

Ortner's syndrome and endoluminal treatment of a thoracic aortic aneurysm: a case report.

Left recurrent laryngeal nerve paralysis was first described in 1897 by Ortner in a patient with mitral valve disease. However, this paralysis has also been attributed to other cardiovascular pathologies, such as thoracic aortic aneurysms. In 1971, Parodi introduced the endoluminal technique by inserting endovascular prostheses for the treatment of vascular aneurysms. The authors present a case of acute dysphonia following left recurrent laryngeal nerve paralysis, confirmed by laryngoscopy, in a patient who was diagnosed with a saccular thoracic aortic aneurysm and treated with endovascular pr

Neuromuscular paralysis for newborn infants receiving mechanical ventilation.

BACKGROUND: Ventilated newborn infants breathing in asynchrony with the ventilator are at risk for complications during mechanical ventilation, such as pneumothorax or intraventricular hemorrhage, and are exposed to more severe barotrauma, which consequently could impair their clinical outcome. Neuromuscular paralysis, which eliminates spontaneous breathing efforts of the infant, has potential advantages in this respect. However, a number of complications have been reported with muscle relaxation in infants, so that concerns exist regarding the safety of prolonged neuromuscular paralysis in ne

Cystic Thymoma-Associated Fulminant Myasthenia Gravis with Rapid Progression in a Cat: Diagnostic and Therapeutic Challenges.

Acquired myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction caused by antibodies against the acetylcholine receptor, leading to generalized muscle weakness and, in severe cases, respiratory muscle paralysis. While MG is well described in dogs, it is rarely reported in cats, and fulminant cases with rapid progression have a poor prognosis. An 11-year-old neutered female Persian cat presented with acute hindlimb weakness, which progressed to tetraplegia within 4 days. The clinical course initially resembled acute polyradiculoneuritis, and electromyography revealed pos
