Evolution of surgical modalities in the management of rhinophyma: our experience.

Rhinophyma is a progressive disease of the nose, which is characterised by skin thickening and sebaceous hyperplasia. Patients with rhinophyma are often stigmatised due to worsening disfigurement of their nose. This can also result in functional impairment such as reduced nasal patency. Severe cases of rhinophyma are best managed with varied surgical interventions, as there is no clear 'gold standard' treatment that has been described. We present our experiences in the management of rhinophyma and the evolution of treatment modalities that have been employed over a nine-year period.

Fractionated ablative carbon dioxide laser for the treatment of rhinophyma.

BACKGROUND: Rhinophyma is a progressive and disfiguring proliferative disorder of the nose, which is related to chronic rosacea. Many different treatment modalities have been utilized both alone and in combination including: loop cautery, CO2 laser, argon laser, dermabrasion, cryotherapy, radiotherapy, full-thickness excision, skin graft, flap reconstruction, and cold scalpel. CO2 resurfacing has been considered first line therapy but is often associated with a shiny, scarred appearance, with patulous pores, and with loss of pigmentation. We report a technique using aggressive parameters with …

[Rhinophyma and skin carcinoma: a case report and literature review].

Rhinophyma, final stage of rosacea is considered as benign pathology. We present the case of a patient with basal cell carcinoma diagnosed on rhinophyma. The removal of all cutaneous nasal unit and its analysis has diagnosed the presence of three basal cell carcinomas and two in situ squamous cell carcinomas. Reconstruction was performed by full-thickness skin graft. The literature reports a few cases of association between rhinophyma and skin cancers but none ever reported the simultaneous presence of basal cell carcinoma and squamous cell carcinomas. The low number of articles does not revea…

Treatment of massive rhinophyma by combined electrosurgery and CO2 laser.

A case of massive rhinophyma that produced significant functional and cosmetic difficulties was treated using electrosurgery and carbon dioxide laser. Minimal bleeding occurred during the operative procedure despite grotesque enlargement and high degree of vascularity of the skin and soft tissue. Using this technique, restoration of normal function and excellent cosmetic effect was achieved without any complication.

Rhinophyma: diagnosis and treatment options for a disfiguring tumor of the nose.

Rhinophyma is a benign dermatologic disease of the nose affecting primarily Caucasian men in their fifth to seventh decades of life. It is characterized by a slowly progressive enlargement with irregular thickening of the nasal skin and nodular deformation. It is assumed to be the end stage of chronic acne rosacea. Main reasons that urge the patients to seek help are plastic cosmetic and functional impairments such as nasal obstruction. Surgical removal of the hyperplastic tumor mass is the treatment of choice for rhinophyma. In a retrospective review, the authors describe the pros and cons of…

[The restored masterwork. The old man, his rhinophyma and the child].

"The old man and the child" is a masterpiece by the Renaissance painter Domenico Ghirlandaio, which was restored a few years ago. We speculate as to the likely origin of the scratches on the old man's face and discuss the complicated restoration of the painting.

Comparison of carbon dioxide laser and electrosurgery in the treatment of rhinophyma.

Cosmetic results of rhinophymas treated with carbon dioxide laser or electrosurgery were compared in six patients 2-5 years after surgery. Although the results were comparably good the use of the carbon dioxide laser proved to be more time-consuming and less convenient. Scar formation was observed in both patient groups equally and was related to the depth of tissue removal, independent of the instrument used.

Removal of rhinophyma with the carbon dioxide laser: a preliminary report.

Surgery is the generally accepted treatment for rhinophyma. Problems associated with standard therapy include moderately profuse hemorrhage impairing accurate removal, and some difficulty in providing smooth demarcation between the rhinophyma and the surrounding tissue. Four patients with rhinophyma have been successfully treated with the carbon dioxide laser. Satisfactory cosmetic results were achieved in all cases with excellent hemostasis and minimal morbidity. Complete healing takes three to four weeks, which is similar to conventional methods.

Five-blade scratcher for treating severe rhinophyma: A retrospective study.

BACKGROUND: Rhinophyma, a late-stage subtype of rosacea, is characterized by excessive sebaceous glands and connective tissue proliferation. Patients may experience respiratory disturbances and psychological distress that significantly affect their quality of life when excessive nasal hyperplasia obstructs the external nasal valves. Surgery is the treatment of choice for rhinophyma. However, excessive bleeding, scarring, pigmentation, and high recurrence rates frequently characterize current surgical methods. AIM: To evaluate the clinical effectiveness and recurrence rates after treating seve…

Surgical treatment of rhinophyma: Retrospective monocentric study and literature review.

BACKGROUND: Treatment of rhinophyma consists primarily of destructive procedures. There is currently no consensus regarding treatment. In this study, we propose an algorithm based on a cohort of 25 patients and a literature review. PATIENTS AND METHODS: This was a retrospective study conducted between January 2016 and December 2018. The cosmetic outcome was evaluated by 2 independent assessors based on pre- and postoperative photographs. Patients were ranked according to the severity (mild, moderate, severe) of their rhinophyma. The different surgical methods used were cold blade excision or …

Novel Management of Rhinophyma by Patterned Ablative 2940nm Erbium:YAG Laser.

INTRODUCTION: Rhinophyma is a cosmetic disorder that causes emotional distress if the symptoms are extensive or obvious enough. Treatment options range from topical antibiotics or isotretinoin, surgical resection, cryosurgery, electrocautery, dermabrasion and more recently laser therapy. With the limitations of surgical techniques, lasers gained popularity for treating rhinophyma. However, laser ablation is invasive and can lead to side effects and prolonged downtime. Fractional photothermolysis (FP) was introduced to overcome the limitations posed by conventional ablative lasers. To the best …
