Clival-Meckel's Cave Angle: A Predictor of Glycerol Displacement in Percutaneous Glycerol Rhizotomy for Trigeminal Neuralgia.

BACKGROUND AND OBJECTIVES: Percutaneous glycerol rhizotomy successfully treats trigeminal neuralgia although failure rates and durability of the procedure are variable. Some of this variability in clinical outcome might be due to egress of glycerol from Meckel's cave (MC) because of surgical positioning and individual patient anatomy. In this article, we quantitatively analyzed the anatomic variances that affect glycerol fluid dynamics to better predict patients more amenable for percutaneous glycerol injections. METHODS: Computed tomography imaging of 11 cadaveric heads was used to calculate…

The Long-Term Outcomes and Predictors of Microvascular Decompression with or without Partial Sensory Rhizotomy for Trigeminal Neuralgia.

PURPOSE: Microvascular decompression (MVD) and MVD combined with partial sensory rhizotomy (PSR) are effective surgical treatments for idiopathic trigeminal neuralgia (TN). The aim of this study was to compare the long-term outcomes of both MVD and MVD+PSR for the treatment of TN and to identify the factors that may influence the long-term outcomes after MVD or MVD+PSR. PATIENTS AND METHODS: From March 2009 to December 2017, 99 patients with idiopathic TN who underwent MVD or MVD+PSR in our hospital (40 MVD, 59 MVD+PSR) were included in the study. The indications for MVD+PSR were as follows: …

Role of femoral derotation on gait after selective dorsal rhizotomy in children with spastic cerebral palsy.

AIM: To evaluate the long-term outcome of selective dorsal rhizotomy (SDR) on gait and the influence of previous femoral derotation osteotomy (FDO). METHOD: In a retrospective cohort study of 29 children (16 females, 13 males) with spastic diplegic cerebral palsy, 14 children received FDO before SDR, whereas 15 children with moderate or near-normal internal femoral rotation during gait received only SDR. Three-dimensional gait data were obtained pre-FDO, pre-SDR, 1 year post-SDR, and 3 to 5 years post-SDR, to study the Gait Profile Score (GPS), pelvic tilt, and knee and hip kinematics. A mixe…

Application of MRI and intraoperative CT fusion images with integrated neuronavigation in percutaneous radiofrequency trigeminal rhizotomy.

BACKGROUND: Percutaneous radiofrequency trigeminal rhizotomy (RF-TR) has been an effective treatment modality for medically refractory trigeminal neuralgia. Our group has established a protocol for this procedure that includes intraoperative computed tomography (iCT) navigation. The depth of the puncture needle in our protocol was based on cadaveric studies, and anatomical localization was mainly by electric stimulation test. The limitation of the invisibility of the trigeminal cistern on CT imaging and bias from the patient's subjective expression during neurophysiologic stimulation might aff…

Clinical results of a brindley procedure: sacral anterior root stimulation in combination with a rhizotomy of the dorsal roots.

The Brindley procedure consists of a stimulator for sacral anterior-root stimulation and a rhizotomy of the dorsal sacral roots to abolish neurogenic detrusor overactivity. Stimulation of the sacral anterior roots enables micturition, defecation, and erections. This overview discusses the technique, selection of patients and clinical results of the Brindley procedure. The Brindley procedure is suitable for a selected group of patients with complete spinal cord injury and detrusor overactivity. Overall, the Brindley procedure shows good clinical results and improves quality of life. However, to…

Unanticipated complication of percutaneous radiofrequency trigeminal rhizotomy: rhinorrhea: report of three cases and a cadaver study.

OBJECTIVE AND IMPORTANCE: Several neurosurgical procedures have been developed for the treatment of idiopathic trigeminal neuralgia: vascular decompression of the trigeminal root in the brainstem, percutaneous trigeminal ganglion procedures, and external beam radiosurgery. Percutaneous radiofrequency electrodes target the trigeminal fibers in the gasserian ganglion through the foramen ovale. Several complications of radiofrequency trigeminal rhizotomy (RF-TR) have been described, including puncture of the carotid artery, the cavernous sinus, and the cranial nerves. This study presents a very r…

Assessment of upper-extremity function in children with spastic diplegia before and after selective dorsal rhizotomy.

The upper-extremity function of 26 children (mean age 6.0 years) with spastic diplegia was assessed prior to and one year following selective dorsal rhizotomy (SDR). No significant change was found in upper extremity range of motion, muscle tone, or strength (MMT). A significant increase was noted in grasp strength bilaterally which could not be explained simply on the basis of maturation. Manipulation patterns were noted to improve significantly one year after SDR; however, this was proposed to be a maturation rather than intervention effect. Standardized test results revealed no significant …

[Long-term follow up of trigeminal neuralgic patients treated by retrogasserian rhizotomy or by microvascular decompression].

The authors studied the long-term follow up of trigeminal neuralgic patients treated by retrogasserian rhizotomy with anhydrous glycerol injection (GI) or by microvascular decompression (MVD) and compared the satisfaction rates between them. Since 1983, 67 patients had been treated either by GI or MVD. The authors analyzed the results of the treatment by the method of questionnaire. Fifty-two patients of these 67 answered the questions, on which the present analysis was based. GI group consisted of 27 patients and MVD group 29 patients. The average follow-up period was 30.9 months in GI group …

Effects of thoracic dorsal rhizotomy or vagotomy on inspiratory muscle activity at various levels of chemical drive.

The relationship between relative peak activity (moving average EMG) of the diaphragm (Adi) and of the cranial (2nd and 3rd) external intercostal or parasternal muscles (Aic) was assessed during rebreathing in animals before and after bilateral thoracic (T1-T4) dorsal rhizotomy (TDR) and/or bilateral vagotomy (VGT). The relationship had the form Aic=a Adib under all conditions. In intact rabbits and cats mean values for b were 1.48 and 1.79, respectively, a being unity by definition. Neither TDR nor VGT changed b; a decreased to about 0.15 with TDR and halved with VGT only if performed before …

Rootlet Selection by Crescendo Single-Pulse Evoked Compound Muscle Action Potential Interpretation During Selective Dorsal Rhizotomy.

Selective dorsal rhizotomy (SDR) is a surgical intervention aimed at reducing spasticity in children with cerebral palsy. This chapter introduces an innovative approach to SDR, utilizing crescendo single-pulse evoked compound muscle action potential (CMAP) interpretation for rootlet selection. The method combines preoperative spastic muscle identification, intraoperative neurophysiological monitoring, and a precise stimulation protocol to guide the selective sectioning of sensory nerve roots. The chapter outlines the setup for crescendo single-pulse stimulation-guided SDR, including preoperati…

Enhancement of postural control and upper limb function following selective dorsal rhizotomy: a retrospectively registered study.

UNLABELLED: Cerebral palsy is the most common pediatric disability, characterized by a spectrum of permanent disorders that hinder movement, posture, and overall activity, causing long-term functional limitations. For those unresponsive or unsuitable to conventional treatments, neurosurgical interventions such as selective dorsal rhizotomy or intrathecal baclofen may be considered. Selective dorsal rhizotomy (SDR) aims to reduce lower limb spasticity while preserving sensory and sphincteric functions. Despite its established effectiveness in the control of lower limb increased muscle tone, its…

Intrathecal baclofen pump versus combined dorsal/ventral rhizotomy for spastic quadriplegia: healthcare cost and complication analysis.

OBJECTIVE: Combined dorsal/ventral rhizotomy (CDVR) has emerged as a tone management option for pediatric patients with cerebral palsy and medically refractory spasticity. However, its costs to the patient and the healthcare system compared with those of an intrathecal baclofen (ITB) pump are understudied. The authors aimed to evaluate ITB and CDVR with respect to healthcare cost, resource utilization, and clinical safety. METHODS: The records for all pediatric patients who underwent ITB pump placement or CDVR at a single institution between 2003 and 2024 were retrospectively reviewed. Hospit…

Retrogasserian Glycerol Rhizotomy (RGGR) for Trigeminal Neuralgia Induced by Postvaccination Herpetic Reactivation: A Rare Case Report.

Reports on surgical treatment for postherpetic trigeminal neuralgia after COVID vaccination have not been found in the literature. Here, we described a case of postherpetic trigeminal neuralgia after COVID vaccination that was treated with percutaneous retrogasserian glycerol rhizotomy (PRGR) resulting in complete pain relief. We reported a case involving a 70-year-old female who had herpes zoster infection in the ophthalmic branch (V1) dermatome following COVID vaccination and presented with paroxysmal electric shock-like sensation without a trigger. Her pain was refractory to medical managem…

Navigated minimally invasive puncture of the trigeminal cistern in horses-a cadaveric study in preparation for a controlled rhizotomy.

INTRODUCTION: Trigeminal-mediated headshaking is a neuropathic disorder in horses, characterized by signs of regional pain similar to trigeminal neuralgia in humans. The injection of glycerol into the trigeminal cistern to ablate pain-conducting nerve fibers within the trigeminal ganglion -known as glycerol rhizotomy- is a well-established treatment in human medicine. This study compares two approaches to the equine trigeminal cistern using a navigation system for guiding needle placement, a previously described ventral and a newly developed transmandibular lateral approach. The surgical accur…

Percutaneous Rhizotomy of the Gasserian Ganglion in Patients With Mass Lesion-Associated Trigeminal Neuralgia: A Case Series.

BACKGROUND: Patients with trigeminal neuralgia (TN) secondary to mass lesions are typically treated by directly addressing the underlying pathology. In cases of TN not alleviated by treatment of the pathology, percutaneous balloon compression (PBC) and glycerol rhizotomy (Gly) are simple and effective ways to alleviate pain. However, there is limited literature on the use of these techniques for patients with TN caused by mass lesions. OBJECTIVE: To describe the use of PBC/Gly to treat mass lesion-related TN. METHODS: We report a retrospective, single-institution, descriptive case series of …

Long term outcome of Selective Dorsal Rhizotomy for the management of childhood spasticity-functional improvement and complications.

BACKGROUND: Selective dorsal rhizotomy (SDR) for the management of lower extremity spasticity is a surgical technique that has existed since the 1900s. While much evidence supports its efficaciousness in reducing tone in the short term, limited information exists detailing the long-term outcome and evolution over time of patients undergoing SDR. METHODS: All publications with 10 years or more of outcome data on patients undergoing SDR were identified from Medline and Embase databases using the search term "Rhizotmy." Only publications that were in English included patients with cerebral palsy…
