Dens invaginatus is a critical condition for endodontic treatment. It frequently presents a complex internal anatomy and might be associated with incomplete root and apical development. This article presents one of the few reported cases of endodontic retreatment of Type III dens invaginatus. First, the internal anatomy was modified using burs under an operating microscope. Next, conventional chemical and mechanical preparation with hand files and 2.5% sodium hypochlorite was performed. Finally, an intracanal dressing with calcium hydroxide was used for nine months, at which point the anatomic

Endodontic therapy was performed in a mandibular first molar with three canals in the mesial root. The mesiobuccal and the mesiolingual canals were found in their normal locations. The third was located in the middle of the distance between the other two. Radiographically, it ended in its own distinct foramen. Many reports deal with three orifices in the mesial root, but very few describe three independent canals, indicating a rare anatomical configuration. To locate the third possible intermediate canal it was suggested to reduce the mesial dentinal wall of the pulp chamber after instrumentin

This case report demonstrates the simultaneous development of furcation bone loss in three-rooted tooth #30 with vital pulp tissue in identical twins. In the first report, resolution of furcal bone loss was seen after nonsurgical root canal treatment. In the second report, furcal bone loss resolved after non-surgical root canal treatment, and periodontal therapy reduced probing depths to maintainable levels.

Labiomandibular paresthesia after root canal treatment is an accident that is still too frequent despite the development of new endodontic techniques. The aim of this anatomical and clinical study is to advance the understanding of how accidents occur so as to avoid them. This anatomical study made it possible to determine the variability of proximity of the apex of the tooth root to the mandibular bundle, as well as the relationship between the nerve and its satellite artery, and to understand how endodontic filling material spreads into the cancellous bone. The clinical study, which included

The aim of this study was to synthesize evidence on the influence of Enterococcus faecalis on endodontic treatment failure and strategies for its elimination. This study followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and was registered in PROSPERO (CRD420251022741). PubMed and SciELO were searched using PICO terms, and 76 studies (1997 and 2025) met the inclusion criteria. Reported E. faecalis prevalence ranged from approximately 4%-78%, depending on detection methods and sampling strategies. Canal isolates frequently expressed virulence fac

Surgical endodontic treatment encompasses a broad spectrum of procedures, amongst which root-end cavity preparation and filling, retrograde root canal treatment and through-and-through endodontic surgery, may be classified under the umbrella term 'Root-end surgery'. This narrative review considers the available data on periapical healing, soft tissue healing, tooth survival and oral health-related quality of life (OHRQoL), following root-end surgery and the factors that affect its outcomes. The pooled periapical healed rate for the studies published up to 2021 was 69% (95% CI: 65%, 73%) but in

PURPOSE: To compare the curative effect of microscopic revascularization and apexification in the treatment of pulp necrosis of permanent teeth.
METHODS: Seventy-five cases of pulp necrosis in young permanent teeth were divided into two groups according to different treatment methods. Group A (n=30) underwent revascularization under microscope, while group B (n=45) underwent apexification. The treatment effect and pain improvement of the two groups were compared. The changes of the wall thickness and root canal length of the affected teeth before and after treatment were observed, and the bon

AIM: This 60-month prospective study aimed to evaluate tooth survival and healing rates after root canal treatment in patients taking bisphosphonates (BPs). Secondary outcomes were complications and clinical variables observed during and after treatment.
METHODS: Root canal treatment was performed using manual K-file canal instrumentation and a carrier-based filling technique with an epoxy resin-based sealer. Teeth without adequate root/crown integrity were restored by trained operators at the tissue level (TL group) to prevent occlusal/mechanical stress and to enable periapical lesion healin

The purpose of this in vitro study was to compare the antimicrobial efficacy of five different irrigating solutions against complex polymicrobial bacterial biofilms harvested from root canals to model actual endodontic irrigation as closely as possible, and to test the efficacy of these irrigants in these conditions. Two multi-species in vitro biofilms were generated from bacterial samples taken from patients presenting with acute pulpitis. The microbial composition of these samples was characteristic of the disease. The biofilms were incubated with 1000 p.p.m. Solumium Dental (ClO(2)), 0.2% c

Electronic apex locators (EAL) have been used to establish the working length (WL) in root canal treatment. In teeth diagnosed with apical periodontitis, resorption of tooth apical structures can lead to difficulties to obtain an appropriate WL. The aim was to compare the capacity of three EAL's (Root ZX II, Raypex 6 and Endo-Eze Quill) to locate the tip of the K-file between 0 to -0.5 mm from the apical foramen (AF) on teeth diagnosed with asymptomatic apical periodontitis (AAP). Electronic working length was performed on 60 roots with AAP. A K-file #15 was inserted in the root canal until th

It is difficult to summarize an article of this type, in which the author is certain that his case has been presented fairly, but is aware that other views that are contrary could be stated and endorsed by pertinent clinical cases. Bearing this fact in mind, I wish to reiterate the following: Lateral canals are demonstrated in endodontic cases with much less frequency than they exist. This variance rarely, if ever, causes an endodontic failure. Some demonstrated lateral canals may really be other conditions. Preoperative evaluation should include examination of radiographs for lateral, as well

Endodontic treatment is an important step of tooth replantation protocols, but the ideal moment for definitive obturation of replanted teeth has not yet been established. In this study, a histomorphometric analysis was undertaken to evaluate the repair process on immediate replantation of monkey's teeth after calcium hydroxide (CH) therapy for 1 and 6 months followed by root canal filling with a CH-based sealer (Sealapex(®) ). The maxillary and mandibular lateral incisors of five female Cebus apella monkeys were extracted, kept in sterile saline for 15 min, replanted and splinted with stainles

This study is aimed at evaluating the effects of FiberSite and glass fiber postsystems on the fracture resistance of endodontically restored teeth, when combined with various root canal sealers. Forty human mandibular premolar teeth, each having a single root with anatomically comparable dimension and length, were selected. The teeth in each experimental group (n = 10) were instrumented, obturated, and restored with the following procedures: in group 1, AH Plus root canal sealer and a glass fiber post with a composite core; in group 2, AH Plus root canal sealer and a FiberSite postsystem; in g

A previous attempt to locate the root canal system in the maxillary left central incisor with a bur created a perforating root defect at the level of the alveolar crest. The patient received root canal therapy. Next, vertical root extrusion was chosen to expose sound tooth structure apical to the defect so that a crown could be constructed. With an anterior open bite, the natural crown was left intact and it was possible to position the anchor bar in an incisal and lingual position, maintaining esthetics for the patient during both the extrusive and stabilization phases of treatment.

Apical periodontitis is caused primarily by microorganisms residing in the root canals of affected teeth. Nevertheless, there is convincing evidence implicating other independent factors that adversely affect the outcome of conventional root canal therapy. In this paper, morphological evidence is presented in support of the potential role of two endogenous factors that may interfere with post-endodontic healing of the periapex. The specimens consisted of a surgical biopsy of an asymptomatic periapical lesion which persisted for a follow-up period of 44 months. The biopsy was processed for corr…

Periapical surgery is required when periradicular pathosis associated with endodontically treated teeth cannot be resolved by nonsurgical root canal therapy (retreatment), or when retreatment was unsuccessful, not feasible or contraindicated. Endodontic failures can occur when irritants remain within the confines of the root canal, or when an extraradicular infection cannot be eradicated by orthograde root canal treatment. Foreign-body reponses towards filling materials, towards cholesterol crystals or radicular cysts might prevent complete periapical healing. Following enhanced microsurgical …
