BACKGROUND: The Bridge to Better Health (BRIDGE) project is a program that focuses on providing rural high school youth with motivation, knowledge, and skills essential to cancer prevention. In this pilot intervention, we used instruction in personal health genealogy as a means of increasing awareness and knowledge of health risk and motivation to change several screening and cancer-related behaviors.
METHODS: We administered a Bridge to Better Health survey to 173 ninth- and 10th-grade students from a rural Southeastern Virginia high school before and after delivery of the BRIDGE pilot inter

This study was conducted in the Guatemalan highland department of Sacatepequez, in a sample of 146 rural women insured by the Social Security system. It examined their health care behaviour when their children presented common childhood symptoms such as diarrhoea, fever, cough and worms. The mothers generally sought help and treatment advice from an older woman in the family, and did so more often for diarrhoea (82%) and fever (64%) than for cough (43%) or worms (28%). Obtaining advice in a pharmacy or from a drug seller ranked second (range: 8%-38%, depending on the symptom), before the procu

INTRODUCTION: Catastrophic health expenditure (CHE) represents a major cause of poverty among rural households in China. Although health insurance coverage in rural China exceeds 95%, CHE incidence remains high, and existing research has predominantly focused on external protection mechanisms such as health insurance, with insufficient attention to household financial management capacity.
METHODS: Drawing on data from 17,156 rural households in the 2019 China Household Finance Survey, this study constructed a financial management behavior index encompassing four dimensions: saving behavior, a

BACKGROUND: Leishmaniasis is a neglected tropical disease endemic in Bolivia that disproportionately affects people with little social and political capital. Although the treatment is provided free of charge by the Bolivian government, there is an under-utilization of treatments in relation to the estimated affected population. This study explores the experiences of patients with leishmaniasis and the challenges faced when searching for diagnosis and treatment in Bolivia using a human rights approach.
METHODS: We conducted open-ended interviews with 14 participants diagnosed with leishmaniasi

In 2008, the Rural Health Project (Health XI) was initiated in 40 Chinese counties to pilot interventions aimed at improving local health systems. Performance targets were pre-specified (results-based), and project counties were allowed to tailor their interventions (bottom-up) in recognition of the substantial regional variations. Using household data from the China National Health Services Survey in a difference-in-differences strategy combined with matching, we find that project counties have improved outcomes (both incentivized and not-directly-incentivized) in all three domains examined-m…

BACKGROUND: In 2005, India launched the National Rural Health Mission (NRHM) to strengthen the primary healthcare system. NRHM also aims to encourage pregnant women, particularly of low socioeconomic backgrounds, to use institutional maternal healthcare. We evaluated the impacts of NRHM on socioeconomic inequities in the uptake of institutional delivery and antenatal care (ANC) across high-focus (deprived) Indian states.
METHODS: Data from District Level Household and Facility Surveys (DLHS) Rounds 1 (1995-99) and 2 (2000-04) from the pre-NRHM period, and Round 3 (2007-08), Round 4 and Annual…

PURPOSE: Rural communities face persistent shortages of behavioral health clinicians, requiring primary care physicians to assume expanded mental health care roles. Our study assessed physicians' access to behavioral health resources, such as referral networks, licensed and non-licensed mental health providers, and telehealth supports, and examined whether perceived sufficiency of access upon referral differs between rural and urban physicians.
METHODS: We analyzed data from the 2022 National Sample Survey of Physicians, a nationally representative survey of active US physicians. Practice ZIP

The U.S. carceral health care system presents a unique opportunity to expand reproductive health services. Evidence suggests that women incarcerated in jails want to initiate contraception, but access varies across facilities and is limited by concerns about cost, reproductive coercion, and a lack of consistent policies. To better understand the reproductive health needs of rural incarcerated women, a cross-sectional study of women incarcerated in four rural Maine jails was conducted using a 59-question survey on contraceptive history, needs, and preferences. Seventy-three women with complex h

BACKGROUND: Community health volunteers (CHVs) are an important resource for supporting health service delivery, surveillance, and social programmes. However, retention and attrition of CHVs remain a big challenge. This study explored factors affecting the retention and attrition of CHVs working as village reporters (VRs) responsible for community-based death notification in the Malaria Vaccine Implementation Program (MVIP) in Malawi.
METHODS: This mixed-methods exploratory study, which intersected with the case studies, was conducted from November 2022 to March 2023 in nine rural districts i

Background: Adapting health systems for climate change can lessen the negative impact of climate change on human health. Even when not targeting climate-health links explicitly, broad health system strengthening interventions (HSSis) can ensure that the health workforce, infrastructure, and networks are robust enough to respond to and recover from climate-driven shocks. Objective: We explored the ability of an HSSi in a rural health district of southeastern Madagascar to serve as a climate change adaptation in response to Cyclone Batsirai in 2022. Method: We conducted interrupted time series a…

BACKGROUND: Virtual care (VC) services (e.g., telemedicine) could help address transportation barriers to healthcare among rural Veterans. The Veterans Health Administration (VA) Virtual Health Resource Centers (VHRCs) provide in-person and telephone technology support for Veterans to use VC, but few are in rural locations. Our objective was to understand Veteran perceptions of a novel implementation of a rural, community-based VHRC. METHODS: We conducted our study using an established learning health system model and a rapid qualitative analysis approach to collect and analyze data. Data we

BACKGROUND/OBJECTIVES: Social support is essential to development, mental and emotional health, and reducing risk for psychological distress. Youth are exposed to various opportunities for socialization with peers, including through social media. Strong online social networks may be most critical for youth from isolated communities, including those from rural areas or LGBT+ (lesbian, gay, bisexual, transgender, etc.) youth. With this study, we investigated associations between social media use and online social support among adolescents. Additionally, we compared the online experiences of rura

Happiness is one of the important manifestations of the psychological health of rural residents. Based on the data of the 2020 China Rural Revitalization Comprehensive Survey (CRRS), this paper constructs a theoretical analysis framework of "material compensation-emotional immersion" and adopts OLS model and instrumental variables method to study the influence of digital skills on the happiness of rural residents and the underlying mechanism. The regression results of the OLS model show that there is an "inverted U" curve relationship between digital skills and rural residents' happiness, whic

INTRODUCTION: Rural-living veterans with chronic kidney disease and refractory hypertension have a higher mortality rate and are hospitalized more frequently than veterans living in urban or suburban areas. They also face particularly unique challenges in accessing nephrology specialty care. Previous studies suggest virtual nephrology care can be used to increase access to care for veterans. The purpose of this study was to examine veteran's perceptions and experiences with a veteran administration (VA) virtual nephrology program.
METHODS: We conducted semi-structured interviews with forty-fo
