Alterations in brain cholecystokinin receptors after fasting.

Cholecystokinin (CCK) is the parent molecule of a family of polypeptide hormones, some of which are secreted from the small intestine after food ingestion and stimulate both exocrine secretion and gall bladder contraction. CCK-like molecules have also been identified in the central and peripheral nervous systems. The significance of CCK in the brain is unknown, but it could mediate satiety. The satiety produced by introduction of food into the intestine can be mimicked by systemic injections of CCK and its analogues-these hormones are also effective when injected into the hypothalamus and the

Gastroduodenal Disorders.

Symptoms that can be attributed to the gastroduodenal area are classified into 5 categories: (1) functional dyspepsia, with 2 subcategories that can overlap: postprandial distress syndrome, with meal-induced symptoms of postprandial fullness or early satiation and epigastric pain syndrome, with epigastric pain or burning that does not occur exclusively postprandially; (2) nausea and vomiting disorders, which include 3 subcategories: chronic nausea vomiting syndrome; cyclic vomiting syndrome; and cannabinoid hyperemesis syndrome; (3) excessive belching disorders, defined as audible escapes of a

Functional Dyspepsia in Review: Pathophysiology and Challenges in the Diagnosis and Management due to Coexisting Gastroesophageal Reflux Disease and Irritable Bowel Syndrome.

Functional dyspepsia is a common disorder which imposes significant diagnostic and treatment challenges for patients and physicians. The most recent update of the diagnostic criteria subdivides functional dyspepsia into two subcategories based on the main symptom of epigastric pain or postmeal fullness. As we discuss in this review, several studies have shown significant overlap in symptoms and pathophysiology between functional dyspepsia, irritable bowel syndrome, and the spectrum of reflux disorders. This overlap in symptoms can be informative in helping us to understand the underlying patho

Effects of mental imagery on gambling behavior.

The influence of a brief mental imagery task was examined to determine if imagining engaging in gambling behaviors would satiate the participant on gambling-related reinforcers and subsequently reduce gambling behavior. Thirty-one participants underwent a mental imagery task consisting of either imagining gambling on a slot machine 30 times and placing quarters into a laundry machine 3 times, or imagining gambling on a slot machine 3 times and placing quarters in a laundry machine 30 times. Following the imagery task, participants were allowed to play a real slot machine and stop whenever they

Opioids in the nucleus of the solitary tract are involved in feeding in the rat.

We evaluated the effect of selective opioid peptides and naltrexone on feeding when injected into the nucleus of the solitary tract (NTS). Doses of 0, 1, 2, 4, and 8 nmol of [D-Ala2,N-Me-Phe4,Gly5-ol]enkephalin (DAMGO, mu-agonist), dynorphin A-(1-17) (DynA-(1-17), kappa-agonist), and [D-Ser2]leucine enkephalin-thr, (delta-agonist) were injected into the NTS in satiated male rats, and food intake was measured at 1, 2, and 4 h. Only DAMGO significantly increased feeding above control levels at doses of 2, 4, and 8 nmol. Doses of 10 and 50 microg naltrexone in the NTS significantly decreased 18-h

Appetite, food intake, and gut hormone responses to glycomacropeptide protein ingestion in older adults: A feasibility, acceptability, and pilot study.

Glycomacropeptide (GMP) has a unique amino acid profile which may make less satiating than other dietary proteins. This study assessed the feasibility and likely acceptability of a leucine-enriched GMP drink and determined appetite response in older adults (OA). Thirteen OA (11f; 70 ± 4 years) were recruited for sensory assessments of a leucine-enriched GMP drink when mixed with water and with fruit smoothie, compared with whey protein isolate (WHEY). Participants also partook in a single focus group exploring acceptability to protein and supplementation. Separately, a counterbalanced, double-

Levodropropizine: Comprehensive Review of the Peripheral Antitussive.

In Indian primary care settings, cough is the second most common presenting symptom, only after fever. Despite cough being just a symptom of an underlying condition, it can be distressing and impact quality of life (QoL). Hence, symptomatic treatment of cough may be prudent along with definitive treatment directed at the underlying etiology. In patients with nonproductive cough, antitussives satiate this role for symptomatic management. On this accord, levodropropizine, a nonopioid, peripheral antitussive, is a valuable agent compared to traditional central antitussives. It demonstrates compar

The neuropsychology of starvation: set-shifting and central coherence in a fasted nonclinical sample.

OBJECTIVES: Recent research suggests certain neuropsychological deficits occur in anorexia nervosa (AN). The role of starvation in these deficits remains unclear. Studies of individuals without AN can elucidate our understanding of the effect of short-term starvation on neuropsychological performance. METHODS: Using a within-subjects repeated measures design, 60 healthy female participants were tested once after fasting for 18 hours, and once when satiated. Measures included two tasks to measure central coherence and a set-shifting task. RESULTS: Fasting exacerbated set-shifting difficulties

Functional dyspepsia: past, present, and future.

Functional dyspepsia (FD) is a highly prevalent gastrointestinal disorder characterized by symptoms originating from the gastroduodenal region in the absence of underlying organic disease that readily explains the symptoms. The Rome II consensus, which defined FD as the presence of unexplained pain or discomfort in the epigastrium, had a number of drawbacks, including an unjustified focus on pain, inclusion of a large number of nonspecific symptoms, and an unclear position on overlap with gastroesophageal reflux disease (GERD) and irritable bowel syndrome (IBS). The Rome III consensus redefine

[Frequent Gastro-Intestinal Disorders: Management of Functional Dyspepsia and Irritable Bowel Syndrome in Clinical Practice].

Frequent Gastro-Intestinal Disorders: Management of Functional Dyspepsia and Irritable Bowel Syndrome in Clinical Practice Abstract: Functional dyspepsia (FD) and irritable bowel syndrome (IBS), two common gastrointestinal entities with overlapping symptoms, should be diagnosed according to Rome IV criteria. This includes one or more of the following symptoms: in FD, postprandial fullness, early satiation, epigastric pain or burning; in IBS, recurrent abdominal pain associated with defecation, change in frequency of stool or form of stool. To exclude structural diseases, attention should be pa

Natural and Drug Rewards Engage Distinct Pathways that Converge on Coordinated Hypothalamic and Reward Circuits.

Motivated behavior is influenced by neural networks that integrate physiological needs. Here, we describe coordinated regulation of hypothalamic feeding and midbrain reward circuits in awake behaving mice. We find that alcohol and other non-nutritive drugs inhibit activity in hypothalamic feeding neurons. Interestingly, nutrients and drugs utilize different pathways for the inhibition of hypothalamic neuron activity, as alcohol signals hypothalamic neurons in a vagal-independent manner, while fat and satiation signals require the vagus nerve. Concomitantly, nutrients, alcohol, and drugs also i

Leptin Sensitizes NTS Neurons to Vagal Input by Increasing Postsynaptic NMDA Receptor Currents.

Leptin signaling within the nucleus of the solitary tract (NTS) contributes to the control of food intake, and injections of leptin into the NTS reduce meal size and increase the efficacy of vagus-mediated satiation signals. Leptin receptors (LepRs) are expressed by vagal afferents as well as by a population of NTS neurons. However, the electrophysiological properties of LepR-expressing NTS neurons have not been well characterized, and it is unclear how leptin might act on these neurons to reduce food intake. To address this question, we recorded from LepR-expressing neurons in horizontal brai

Rome Foundation and international neurogastroenterology and motility societies' consensus on idiopathic gastroparesis.

To establish a consensus on the definition and management of idiopathic gastroparesis, international experts (selected by neurogastroenterology and motility societies and initiated by the Rome Foundation) devised 144 statements using the Delphi method, with at least 80% agreement required. This consensus defined idiopathic gastroparesis as the presence of symptoms associated with delayed gastric emptying in the absence of mechanical obstruction. Nausea and vomiting were identified as cardinal symptoms. Frequently co-existing symptoms are early satiation and postprandial fullness. Diagnosis req

The Role of GIP in the Regulation of GLP-1 Satiety and Nausea.

Gastric inhibitory peptide (GIP) is best known for its role as an incretin hormone in control of blood glucose concentrations. As a classic satiation signal, however, the literature illustrates a mixed picture of GIP involvement with an at best weak anorectic response profile being reported for GIP receptor (GIPR) signaling. Not surprisingly, the pursuit of exploiting the GIP system as a therapeutic target for diabetes and obesity has fallen behind that of the other gastrointestinal-derived incretin, glucagon-like peptide 1 (GLP-1). However, recent discoveries highlighted here support potentia

A Vagal-NTS Neural Pathway that Stimulates Feeding.

A fundamental question of physiology is how gut-brain signaling stimulates appetite. While many studies have emphasized the importance of vagal afferents to the brain in inducing satiation, little is known about whether and how the vagal-mediated gut-brain pathway senses orexigenic signals and stimulates feeding. Here, we identified a previously uncharacterized population of fasting-activated catecholaminergic neurons in the nucleus of the solitary tract (NTS). After characterizing the anatomical complexity among NTS catecholaminergic neurons, we surprisingly found that activation of NTS epine

Current and emerging therapeutic options for the management of functional dyspepsia.

Introduction: Functional Dyspepsia (FD), defined as chronic symptoms originating from the gastroduodenal region in absence of readily identifiable organic disease, is one of the most common gastrointestinal disorders. FD is divided into two subgroups: Post-Prandial Distress Syndrome (PDS) or meal-related FD, characterized by postprandial fullness and early satiation, and Epigastric Pain Syndrome (EPS) or meal-unrelated FD, characterized by epigastric pain and burning.Areas covered: This review summarizes the existing and off-label therapeutic options for FD.Expert opinion: The identification o

Stress-induced modulation of vagal afferents.

Vagally dependent gastric functions, including motility, tone, compliance, and emptying rate, play an important role in the regulation of food intake and satiation. Vagal afferent fibers relay sensory information from the stomach, including meal-related information, centrally and initiate co-ordinated autonomic efferent responses that regulate upper gastrointestinal responses. The purpose of this mini-review is to highlight several recent studies which have uncovered the remarkable degree of neuroplasticity within gastric mechanosensitive vagal afferents and the recent study by Li et al, in th

Transient severe hypercholesterolemia following bariatric surgery treated successfully with increased food intake.

We describe a case of transient severe hypercholesterolemia after bariatric surgery treated successfully with increased food intake. A 25-y-old policeman who had sleeve gastrectomy for morbid obesity 10 mo previously presented with generalized weakness, constipation, and significant weight loss after severe dietary restriction. All his preoperative and prior investigations were normal. Further investigation revealed severe total and low-density lipoprotein hypercholesterolemia. After all other causes of secondary hypercholesterolemia were excluded, a diagnosis of starvation-induced hypercholes

Therapeutic options for functional dyspepsia.

Functional dyspepsia (FD) is defined by the presence of chronic gastroduodenal symptoms in the absence of organic or systemic disease that explains them, and a negative upper endoscopy. According to the Rome III consensus, FD can be subdivided into PDS (postprandial distress syndrome) and EPS (epigastric pain syndrome). In patients with mild symptoms, reassurance and lifestyle adjustments are often sufficient. Pharmacotherapy, for those with more severe or persisting symptoms, includes the use of proton pump inhibitors (PPIs), prokinetics and psychotropic agents. In those diagnosed with Helico
