In scaphoid fractures, delayed diagnosis and nonunion are fairly common as a result of several factors, including the difficulty of radiographic diagnosis of non-displaced fractures and underestimation of the injury by the patient. Main factors to consider when deciding treatment are the type of fracture and fracture stability. In the stable nonunion (Type D1 according to the Filan and Herbert classification, or linear type of Ikeda's classification), percutaneous screw fixation without bone graft is recommended. The indications of non-vascularized bone grafting are as follows: (1) arthroscopi

Because hydatidosis of the bone (echinococcus infection) is a rare parasitic infection, its diagnosis and treatment poses great difficulties. Radiologic imaging findings are generally helpful to make the diagnosis. But occurrence of disease in atypical places and lack of specific radiological findings may complicate differential diagnosis. Nevertheless, familiarity with imaging findings in patients living at endemic areas provides advantages for diagnosis and treatment. We present a cyst hydatic case in scaphoid bone which has been reported in the literature only once previously.

PURPOSE: The present study is a review of patients with scaphoid non-unions treated with a dorsal vascularized bone graft. The study highlights a subset of patients incorrectly diagnosed as graft failures.
METHODS: A retrospective review of patients who received vascularized grafts for scaphoid nonunions was performed over a four-year period. The vascularized graft of choice for this group was the dorsal radial extensor compartment artery.
RESULTS: Five patients from a scaphoid fracture group who were treated with vascularized grafts were diagnosed as being failures (average of five months).

We report the use of a free vascularised iliac bone graft in the treatment of 21 patients (19 men and 2 women) with an avascular nonunion of the scaphoid in which conventional bone grafting had previously failed. The mean age of the patients was 32 years (23 to 46) and the dominant wrist was affected in 14. The mean interval from fracture to the vascularised bone grafting was 39 months (9 to 62). Pre-operative MRI showed no contrast enhancement in the proximal fragment in any patient. Fracture union was assessed radiologically or with CT scans if the radiological appearances were inconclusive.

During the last few years, several types of vascularized bone grafts have been described for the treatment of scaphoid nonunions reporting rates of union often higher than 90%. The goal of this study was to evaluate the results, complications and failures of Kuhlmann's vascularized bone graft. Thirty patients who had benefited from this graft between 2000 and 2005 were reexamined. The average age was 27 years (15-66 years). Seventeen patients were smokers. There were two cases of proximal pole necrosis. The average follow-up was 28 months (eight to 60 months). Osteosynthesis was carried out in

Background: Percutaneous screw placement can treat nondisplaced or scaphoid waist fractures. Screws positioned in areas of highest trabecular density improve fracture stability and bone healing. This study aims to compare the density of six segments of the intact scaphoid bone to facilitate optimal screw positioning. Methods: This study compared the density of six segments of the scaphoid bone in 214 intact scans using the 3D Quantitative Imaging (3DQI) Platform. The scaphoid was divided into proximal, middle and distal regions, each with radial and ulnar segments. Densities were measured in H…

BACKGROUND/AIM: Management of scaphoid trauma includes imaging with repeated X-rays whose interpretation is difficult and often ambivalent. The aim of the study was to propose a fast magnetic resonance imaging (MRI) protocol permitting exclusion of traumatic lesions of the scaphoid bone in children, which would avoid unnecessary immobilization and irradiation in negative cases.
PATIENTS AND METHODS: Two pediatric radiologists retrospectively reviewed the X-rays and MRIs of 45 children with clinical suspicion of scaphoid trauma. X-Rays and MRI sequences [short tau inversion recovery (STIR), T1…

BACKGROUND: Fractures of the scaphoid are well known to be problematic especially when complicated by avascular necrosis, nonunion and carpal collapse. Fixation techniques have involved nonvascularised bone grafting; however, in the presence of avascular necrosis, generally poor union rates (47%) occur as identified by a meta-analysis performed by Merrell et al. The introduction of pedicled vascularised bone grafts showed further improvement; however, in the presence of carpal collapse, union rates as low as 50% have been reported by Chang et al. amongst others using the 1,2-intercompartmental

Screws with different levels of compression force are available for scaphoid fixation and it is known that the Acutrak screw generates greater compression than the Herbert screw. We retrospectively compared two types of headless compression screw for their effectiveness in the repair of scaphoid nonunion. Twenty-nine cases of proximal scaphoid nonunion were surgically treated with non-vascularised bone graft: the Acutrak screw was used in 17 patients and the cannulated Herbert screw in 12 patients. Wrist range of motion, Mayo wrist score, grip strength and QuickDASH scores were indicators used

PURPOSE: The development of small cannulated screws permitted minimally invasive percutaneous fixation of acute scaphoid fractures. There are known mechanical advantages to increased screw length and central screw placement, as well as documented deleterious effects of screw malposition, including articular protrusion, proximal pole fracture, and nonunion. The purpose of this study was to compare 2 methods of calculating a screw axis accessible via a volar surgical approach.
METHODS: To prevent screw protrusion through the surface of the scaphoid, we required the central screw axis to be cont

Fractures, and especially pseudoarthroses of the scaphoid bone, are a great problem in orthopedic surgery, because the failure percentage is remarkably high (20-30%) even with the top modem treatment methods. Use of grafts, especially well-vascularized ones, along with good fixation, has improved treatment outcomes, but the failure and complication percentages remain high. In the pilot study, we present the first five patients with the pseudoarthrosis of the scaphoid bone, male, of 26.8 years of average age (from 22 to 34-years-old), in whom the method of extrafocal fixation, with distraction …

We examined twenty-one patients in whom an established nonunion of the scaphoid had been treated with Herbert-screw fixation with bone-grafting; our purpose was to determine the long-term morbidity of the procedure (at an average of forty-eight months; range, seventeen to fifty-five months). According to the modified scaphoid outcome scoring system, eighteen patients had a good or excellent result. No patient had failure of the hardware. Radial deviation was significantly decreased (an average of 17 degrees; 55 percent) compared with that of the contralateral wrist (p = 0.02). The other ranges

The authors present their anatomical and clinical findings concerning fractures and nonunions of the carpal scaphoid. They stress the proximal pole type I et II of the Schernberg (type C of Herbert) classification, and they suggest isolation of an anatomical entity, by the study of blood vessels, the ligament connections, and especially the pathomechanics as well as a clinical entity, by the mechanism of fracture and the surgical treatment, emphasizing the proximo-distal screwing, with excellent results. They present a series of 29 patients, with type I or II proximal fractures, either fresh o

Eighty-nine late results (five to twelve years follow-up) of prosthetic replacement of the scaphoid bone with a silastic spacer are presented. In 76% the clinical course was uncomplicated; 24% needed secondary surgical procedures including two arthrodeses of the wrist joint. Flexion and extension of the wrist improved by 10.9%, radial and ulnar deviation by 15.0% on average. The first carpometacarpal joint showed minor restriction in 24.0%. The power grip was diminished by 32.0%, the lateral pinch by 25.0% on average. In 43.0% of the cases development or increase of intraosseous cysts was noti…

This study primarily aimed to report the outcome of the arthroscopic cancellous bone grafting technique for treating scaphoid nonunion. The secondary aim was to compare the bone union rate and time, and the ability to correct the angulation of the scaphoid, between arthroscopically treated patients (n = 27), and patients treated with open cancellous bone grafting (n = 27). Nine surgeons (two in the arthroscopic and seven in the open group) with Level III experience operated on the patients. The pain score, grip strength and Quick Disability of the Arm, Shoulder and Hand score improved signific
