Hypothesis about an existent biomechanical cause-effect relationship between Schëuermann's kyphosis and scoliosis.

Schëuermanńs kyphosis is usually observed with a mild idiopathic scoliosis, and there is parity between these two diseases. The aim of this work is to establish a hypothesis about the existence of a biomechanical causal relationship between Schëuermann's kyphosis and scoliosis. To achieve this, a literature review was conducted. A simple mechanical model of the passive thoracolumbar subsystem was created to support part of the discussion. This mechanical model describes the passive thoracolumbar subsystem under ideal conditions of equilibrium. After giving consideration to the system under the

Kyphosis of the thoracic and thoracolumbar spine in the pediatric patient: normal sagittal parameters and scope of the problem.

As measured by the Cobb angle, normal sagittal kyphosis is 20 degrees to 40 degrees, which encompasses most of the angulated consecutive vertebrae in the thoracic region of the spine. With pathologic kyphosis, however, the segmental analysis of different regions of the thoracic spine plays an important role. Methods of determining sagittal measurements as well as the causes of kyphosis of the thoracic and thoracolumbar spine, including Scheuermann's disease, spinal cord injury, laminectomy, neurofibromatosis, genetic origins, Marfan syndrome, and tuberculosis, are also important in determining

Discitis--a rare early symptom of ankylosing spondylitis.

This case report describes a patient who suffered from backache at the age of 12 years. The initial diagnosis was Scheuermann's disease but repeated examinations showed a progressive discitis, without signs of arthritis of the sacro-iliac joints. There was also a recurring effusion of the left knee. Three years later the clinical and radiological evaluation showed irreversible immobilisation of the dorsolumbar spine, whereas the sacro-iliac joints were mildly affected. Discitis seems to be a rare early symptom of ankylosing spondylitis.

Scheuermann's kyphosis: surgical management.

Indications for surgery in Scheuermann disease are not well codified and remain rare, as the natural history of the disease is in most cases benign. In the immature adolescent, conservative treatment, such as bracing or casting, can be tried for moderate curves. For larger curves, or in the adult, conservative treatment is usually not effective, and surgery can be considered. Such indications are mostly cosmetic for large curves above 75 degrees. Pain over the deformity or in the low back may represent another surgical indication, especially in the adult group. The question of anterior release…

Childhood-onset osteoarthritis, tall stature, and sensorineural hearing loss associated with Arg75-Cys mutation in procollagen type II gene (COL2A1).

OBJECTIVE: To define the clinical, radiologic, and molecular genetic characteristics of a family with early progressive osteoarthritis mimicking childhood rheumatoid arthritis, Scheuermann-like changes of the spine, tall stature, short 3 and 4 metatarsals, and moderate sensorineural hearing loss. METHODS: We describe a 22-year-old woman and her 54-year-old mother with early progressive osteoarthritis mimicking childhood rheumatoid arthritis. The index case, her mother, and 3 other family members underwent a physical examination, anthropometric measurements, and radiologic studies. Their DNA w

Rapidly progressive paraplegia due to an extradural lumbar meningocele mimicking a cyst. Case report.

Unlike arachnoid meningoceles, arachnoid cysts frequently cause local pressure effects probably because there is no free communication betwe … gia in an otherwise healthy 31-year-old man in whom magnetic resonance imaging revealed stigmata of Scheuermann disease. Intraoperatively, the lesion was classified as a transitional-type lesion, in accordance with Type IA of the Nabors classification, because a communication with the subarachnoid space was observed. After complete removal of the meningocele, the pat …

[Vertebral compression revealing acute lymphoblastic leukemia].

BACKGROUND: Vertebral compression is rarely the first manifestation of acute leukemia (AL) in children. CASE REPORT: Three children, 13, 4 and 13 years of age respectively, suffered from recent and persistent back pain. The first patient was considered as having psychosomatic problems despite thrombocytopenia at the first examination. The second patient was also pale with hematoma on his legs so that AL could be rapidly recognised. An initial diagnosis of Scheuermann disease was proposed for the third patient in whom hemogram was performed due to worsening of his general condition. The three …

Cricket injuries.

English and Australian cricket teams on tour used to travel by ship, and many enjoying shipboard life, reached their destination considerably overweight and thus unfit and vulnerable to injury in their sport. Now they travel by air and most national teams employ a fitness coach to supervise sessions of rigorous exercise before and during the season. However the international cricket season is longer than it was and the incidence of overuse injuries is increasing.

Bone scintigraphy in discitis and related disorders in children.

Thirty-nine children with suspected pathology involving the vertebral column were investigated haematologically, radiographically, and by bone scintigraphy using technetium 99M pyrophosphate. Fifteen children were shown to have inflammatory disease of the vertebrae. A further six suffered from Scheuermann's disease, two from benign tumours, and the remainder from miscellaneous diseases not specifically involving bony pathology. The nuclide scan was abnormal in all cases of discitis and osteomyelitis, and in the two tumours. All of the other conditions were associated with a normal bone scan. T

Low back pain in the athlete. From spondylogenic injury during recreation or competition.

Low back pain is a common complaint of athletes, and accurate diagnosis is essential, since many causes of back problems can lead to disability. If the obvious mechanical causes, such as muscle strain, "kissing spines," and leg-length inequality, have been ruled out, the more serious sources of low back pain, such as disk herniation, Scheuermann's disease (swimmer's back), compression fractures of the vertebral body, and stress fractures of the posterior elements, should be considered. Treatment goals for all these disorders include pain relief and prevention of further injury.

Juvenile kyphosis.

Scheuermann's disease is a common cause of kyphosis developing in the juvenile period. Untreated, the deformity may progress, producing significant pain and, rarely, neurologic signs and symptoms. Successful treatment is possible with the Milwaukee brace, provided that the child has not reached skeletal maturity. Bracing will produce a superior result in terms of correcting the deformity and reversing vertebral wedging. Surgery may occasionally be necessary for severe kyphosis, especially in the adult who presents with pain and/or neurologic problems. Combined anterior and posterior approach i
