A 14-year-old male child presented with redness and decreased vision in the right eye for 7 days. He was being treated for viral conjunctivitis for right eye at a local hospital. His visual acuity was 6/24 OD and 6/9 OS. Slit-lamp examination revealed diffuse conjunctival congestion in the right eye. Dilated fundus examination revealed mild disc hyperaemia and retinal striae in both the eyes. A peripapillary serous detachment of macula in the right eye was seen on optical coherence tomography. B scan ultrasonography revealed increased scleral thickening and characteristic 'T' sign in both the

AIMS: To describe and compare clinical features, complications and outcomes in patients with granulomatosis with polyangiitis (GPA)-associated scleritis with those seen in idiopathic and other autoimmune-associated scleritis, and to further describe the features that may serve as an indicator of life-threatening systemic disease.
METHODS: We retrospectively reviewed electronic health records of all patients with scleritis seen at two tertiary care centres. Of 500 patients, 14 had GPA-associated scleritis and were included in this analysis. Measures included were age, gender, laterality, visua

Scleritis is a heterogeneous group of diseases characterized by inflammation of the sclera, which may be due to local or systemic infections or immune mediated diseases. Numerous studies over the last decade have lead to significant progress in understanding the pathogenesis and treatment of this severe and potentially blinding disease. Immunological investigations of non-infectious scleritis and associated diseases have indicated that scleritis is an autoimmune disease and studies on the nature of the local inflammatory response have revealed the prominent role of T and B cells and cytokines,

UNLABELLED: Scleritis is an ocular inflammatory disorder commonly associated with systemic autoimmune diseases. We report a case of nodular scleritis with an etiological diagnosis of tuberculosis wherein diagnosis was possible only after histopathological examination of the enucleated eye.
METHOD OF STUDY: A 52 year female patient was referred as a case of nodular scleritis not responding to topical and oral anti-inflammatory agents. She was being treated with immunosuppressives for rheumatoid arthritis by her rheumatologist. Scleritis improved initially but worsened in few months with develo

Scleritis is a very uncommon manifestation in patients with IgA nephropathy. Here, we report the case of a patient presenting with diffuse anterior scleritis in which the laboratory disclosed microscopic haematuria and nephrotic range proteinuria. Renal function was normal. Serology for lupus, vasculitis and cryoglobulinaemia was negative. Rheumatoid factor was negative, and serum C3 and serum C4 were on the normal range. Serology for human immunodeficiency virus types 1 and 2, hepatitis B, hepatitis C, syphilis, and Lyme disease was also negative. A renal biopsy was performed and revealed IgA

The purpose of the present investigation was to develop an optimal approach to rehabilitating patients with episcleritis and scleritis of various etiology. The paper summarizes the results of examination and treatment in 128 patients (142 eyes) with the acute chronic course of the diseases. To specify the etiopathogenesis of a disease, the patients were examined by a rheumatologist, an immunologist, a virologist, a phthisiatrician, an ENT specialist, and a roentgenologist. Rhinofrequency ultrasound biomicroscopy was made in the diagnosis of scleral inflammatory diseases. Ultrasound studies (ul

Posterior scleritis is a serious ocular inflammatory and potentially blinding disorder. It is uncommon and often under-recognized due to its varied presentations, and general ophthalmologists are not familiar with it. Posterior scleritis may be idiopathic or associated with systemic diseases. Visual disturbances such as blurring or distortion are the commonest symptoms. There may be pain, tenderness, and deep-seated discomfort in and around the orbit. One study reported a 49% recurrence rate of posterior scleritis. Posterior scleritis responds well to systemic nonsteroidal anti-inflammatory ag

CASE REPORT: A 55 year-old Spanish woman with a personal history of lepromatous leprosy treated for 10 years with anti-lepromatous triple therapy was referred to us presenting nodular scleritis in her left eye with no other clinical manifestations. Ophthalmological evaluation disclosed several inflammatory features in both eyes. Complementary tests performed were negative and the clinical picture was diagnosed as an immuno-mediated manifestation of leprosy. A favorable outcome was achieved with steroidal treatment.
DISCUSSION: Scleritis and some immunomediated conditions may appear during the

OBJECTIVE: To describe a pseudoepidemic of infectious scleritis following eye surgery.
METHODS: Retrospective cohort study with selected procedural and laboratory investigations.
RESULTS: Twenty-one patients with postoperative scleritis were identified during a 2-month outbreak. Neither an infectious etiology nor a causative pre-, intra-, or postoperative exposure was found. The clinical findings, when carefully reviewed, were consistent with poor surgical-wound closure.
CONCLUSIONS: The art of clinical diagnosis involves the subjective interpretation of clinical history, physical findings,

A 50-year-old diabetic man developed necrotizing scleritis with adjacent keratitis 4 weeks after uncomplicated cataract extraction and intraocular lens implantation through a scleral tunnel incision. Cultures of the necrotic sclera grew Rizopus species. Severe destruction of the globe ensued despite topical, subconjunctival, and intravenous amphotericin B, in combination with hyperbaric oxygen therapy. Histopathological examination of the enucleated globe was consistent with Rhizopus infection. One year later, the patient was well without signs of recurrence.

A case of a 66-year-old woman suffering from polymyalgia rheumatica with necrotizing scleritis followed central retinal artery occlusion in the left eye was studied clinico-pathologically. The left eye was enucleated because of severe pain in spite of the steroid therapy, and was examined histopathologically. The necrotic granulomatous changes of the deep sclera in the ciliary body and peripheral fundus were observed. Lymphocytes, plasma cells and epitheloid cells infiltrated in the central retinal artery at the papilla. It seemed that subconjunctival injection of steroid had induced scleral n

The clinical course of 47 patients with posterior scleritis is reviewed. Though clinical presentation varied widely, 73% of the patients presented with a visual acuity of 6/18 or less. Because the posterior scleritis was not always associated with pain or with anterior scleritis, the diagnosis was often not considered when the patient was first seen. The most common findings in the fundus were disc swelling, retinal detachment, and macular oedema and the most useful investigation was B scan ultrasound. No common aetiology was found, although 60% had a systemic disorder which was accompanied by

A report is presented on an 83-year-old female patient with seropositive rheumatoid arthritis who developed a necrotizing scleritis of the right eye. The clinical course could not be influenced by indomethacin, cysteine, and the cytostatic chlorambucil. Local cyclosporin treatment, however, which was initiated after a 4-week therapy-free period, led to considerable improvement within a few days. The disease disappeared after 6 weeks of treatment. There is no recurrence as yet, the follow-up period being 13 months.
