Adults with metabolic dysfunction-associated steatotic liver disease (MASLD) and steatohepatitis (MASH) are at risk for cardiometabolic complications and major adverse liver-related outcomes (MALO), including cirrhosis, cancer, and liver transplantation. Non-invasive tests (NIT) for hepatic fibrosis, including vibration-controlled transient elastography (VCTE), magnetic resonance elastography (MRE), or enhanced liver fibrosis (ELF) score, can replace liver biopsy to identify high risk for MALO (e.g. VCTE 10-20 kPa, MRE 3.1-5.0 kPa, or ELF 9.8-11.3), in which pharmacotherapy may be considered.

Chronic kidney disease is a major health burden. Given the therapeutic potential of GLP-1 receptor agonists (semaglutide, tirzepatide, and retatrutide) in diabetic nephropathy, we evaluated their anti-fibrotic and anti-inflammatory effects in non-diabetic renal fibrosis using HK-2 cells and murine UUO and aging models. Integrating in vivo and in vitro data showed model- and drug-specific patterns of action among the three agents. In the UUO model, semaglutide effects were associated with PI3K-AKT inhibition, tirzepatide effects with PI3K-AKT inhibition and PPAR pathway activation, and retatrut

Glucagon-like peptide-1 (GLP-1) receptor agonists are widely used for the management of obesity; however, respiratory complications are rarely documented. A 40-year-old person without diabetes with obesity (body mass index [BMI] 36 kg/m2) developed excessive daytime sleepiness and morning headaches four weeks after semaglutide dose escalation to 1 mg per week. Arterial blood gas analysis revealed respiratory acidosis (pH 7.33 [reference range (RR): 7.35-7.45], partial pressure of arterial carbon dioxide [PaCO2] 56 mmHg [RR: 35-45 mmHg]) with a normal alveolar-arterial (A-a) gradient. Metabolic dysfunction-associated steatotic liver disease (MASLD) is a leading cause of chronic liver disease, and its progressive inflammatory phenotype, metabolic dysfunction-associated steatohepatitis (MASH), is characterized by hepatocellular injury, inflammation, and fibrosis. These processes are closely linked to altered metabolite networks, including lipotoxic lipids, oxidized lipid mediators, bile acids, amino acids, acylcarnitines, redox-related metabolites, gut-derived metabolites, and extracellular matrix remodeling products. The recent introduction of resmetirom and semaglutide for

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) and its progressive form, metabolic dysfunction-associated steatohepatitis (MASH), are increasingly prevalent conditions closely linked to obesity, insulin resistance, and cardiometabolic diseases. Although lifestyle modification remains the cornerstone of management, it frequently fails to achieve sustained histologic improvement, suggesting that patients require effective pharmacologic therapies.
SUMMARY: Incretin-based treatments, including glucagon-like peptide-1 (GLP-1) receptor agonists and newer dual agonists

Semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist increasingly prescribed for weight loss, is generally well tolerated but may have underrecognized gastrointestinal complications. We present the case of a 59-year-old woman with an extensive abdominal surgical history who developed small bowel obstruction (SBO) shortly after semaglutide administration. Given semaglutide's effects on gastric motility and its rising use, this case highlights the importance of caution when prescribing GLP-1 agonists to patients with a history of intra-abdominal surgeries and intra-abdominal adhesions

OBJECTIVE: This proof-of-concept study evaluated the impact of adding semaglutide as an adjuvant therapy to sleeve gastrectomy (SG) on weight loss and metabolic outcomes in obese mice.
METHODS: C57Bl6/J male received for 12 weeks high-fat diet and 20% fructose-enriched water, then they were randomized to undergo SG or a Sham surgery and were subsequently treated with either semaglutide or a vehicle for 8 weeks. Mice were weighed weekly, and food intake, plasma glucose levels, and adipose tissue weights were measured.
RESULTS: Both SG and semaglutide alone significantly reduced body weight co

Semaglutide has recently received conditional accelerated approval in the US for treatment of metabolic dysfunction-associated steatohepatitis (MASH) with significant or advanced liver fibrosis (stage F2/F3). Phase 2 and 3 clinical trials show that subcutaneous semaglutide 2.4 mg/week leads to significant improvements in hepatic steatosis, disease activity, resolution of MASH and reduction in liver fibrosis. These benefits parallel weight loss and are accompanied by improved metabolic outcomes, including better glucose control and lipid profiles, as well as consistent benefits for cardiovascul

The transforming growth factor-β (TGF-β)/Suppressor of Mothers against Decapentaplegic (Smad) signaling pathway plays an important role in the pathogenesis and progression of liver fibrosis. This current study was conducted to investigate the effect of semaglutide (SEMA), a glucagon-like peptide-1 (GLP-1) receptor agonist, in a mouse model of liver fibrosis. The mice received thioacetamide (TAA) (150 mg/kg, biweekly) via intraperitoneal (i.p.) injection for nine consecutive weeks to induce liver fibrosis. SEMA was administered orally once daily at a dose of 0.12 mg/kg. Administration of SEMA i

BACKGROUND: Evidence suggests that glucagon-like peptide 1 receptor agonists (GLP-1RAs) might have pro-cognitive effects. No prior study has evaluated the efficacy and safety of a GLP-1RA for the treatment of cognitive dysfunction in adults with major depressive disorder (MDD) in a randomized clinical trial.
METHODS: This was a 16-week, randomized, double-blind, placebo-controlled, parallel-group trial (NCT04466345). Eligible adults met DSM-5-defined criteria for MDD, exhibited pre-treatment evidence of cognitive impairment, and were overweight/obese. Patients were randomized (1:1) to receive

Semaglutide, a glucagon-like peptide-1 receptor agonist, has emerged as a promising pharmacological intervention in obesity management. This systematic review aimed to evaluate the efficacy and safety of semaglutide in promoting weight loss in adults with obesity. A comprehensive literature search was conducted in the PubMed, Scopus, and Web of Science databases, and nine studies met the inclusion criteria. The studies included randomized controlled trials and case-control studies involving participants aged 18-65 years with BMI > 30 kg/m². Semaglutide administered once a week, particularly at

BACKGROUND Although hypomagnesemia is common in type 2 diabetes, clinical presentations with severe hypomagnesemia are rare. A number of oral blood glucose-lowering medications can reduce serum magnesium concentrations, and several severe cases have been reported in the presence of marked glucagon-like peptide-1 receptor agonist (GLP-1RA)-associated gastrointestinal adverse effects. In the present case, an acute presentation with severe hypomagnesemia was likely due to polypharmacy including semaglutide, albeit with a delayed relationship to discontinuation of this GLP-1RA, due to nausea and v

BACKGROUND: Peripheral artery disease (PAD) is prevalent in women and men with type 2 diabetes (T2D). Sex-based differences exist in its epidemiology, clinical presentation, functional impact, outcomes, and potentially in responses to treatments. Recently, semaglutide 1.0 mg has been shown to improve functional outcomes and health-related quality of life in individuals with early symptomatic PAD and T2D in the STRIDE trial.
OBJECTIVES: The objective of this study was to describe baseline characteristics, functional status, and therapeutic efficacy of once-weekly semaglutide 1.0 mg between fem

Gastroparesis is a chronic disorder characterized by delayed gastric emptying without mechanical obstruction, causing symptoms such as nausea, vomiting, and abdominal discomfort. Semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist used for type 2 diabetes, can slow gastric motility and induce gastroparesis. We present a case of a 48-year-old woman with type 2 diabetes, who developed persistent nausea and vomiting after resuming semaglutide at 2 mg subcutaneously once weekly without following the recommended stepwise titration schedule (typically initiated at 0.25 mg weekly with gra

IMPORTANCE: Semaglutide and tirzepatide, the new generation of glucagon-like peptide-1 receptor agonist used to treat type 2 diabetes and obesity, were recently reported to be associated with increased risk of nonarteritic anterior ischemic optic neuropathy (NAION). However, reported associations were inconsistent, and it remains unknown if semaglutide or tirzepatide is associated with other disorders of the optic nerve and visual pathways.
OBJECTIVE: To examine associations of treatment with either semaglutide or tirzepatide with first-time diagnoses of optic nerve and visual pathway disorde

Semaglutide, a glucagon-like peptide 1 receptor agonist, is an antidiabetic that has recently shown promising immunomodulatory and antitumor effects. Breast cancer is the most common type of cancer affecting women worldwide. The aim of this study was to analyze the effects of semaglutide on the antitumor immunity in a 4T1 mouse breast cancer model. After induction of breast cancer, BALB/C mice were treated intraperitoneally with semaglutide. Semaglutide administration decelerated tumor appearance, growth and progression. The antidiabetic drug showed neither a direct cytotoxic effect in vitro,

BACKGROUND: Obesity is associated with adverse cardiac remodeling and is a key driver for the development and progression of heart failure (HF). Once-weekly semaglutide (2.4 mg) has been shown to improve HF-related symptoms and physical limitations, body weight, and exercise function in patients with obesity-related heart failure with preserved ejection fraction (HFpEF), but the effects of semaglutide on cardiac structure and function in this population remain unknown.
OBJECTIVES: In this echocardiography substudy of the STEP-HFpEF Program, we evaluated treatment effects of once-weekly semagl
