BACKGROUND: In a pilot prospective consecutive series on 50 patients with recurrent breast cancer, results of sentinel lymph node biopsy (SLNB) are reported. The interval between primary operation and recurrence was 8years (range 1-18years). Only three patients had not undergone dissection of the axilla (ALND).
RESULTS: In 51% of patients scintigraphy disclosed sentinel nodes (SN). At operation SN was identified in 45% of patients corresponding to 83% of the SN's visualized by the scintigraphy. SN contained metastases in seven cases (16%), and the treatment plan was changed as a consequence o

Adenocarcinoma of the minor salivary gland (MSG) is a rare neoplasm. It represents about 2% of all MSG malignant tumours. It is more common in the palate and in females. This tumour occurs in a mean age of sixty and its mean size at presentation is 1.5cm. Microscopically, the neoplasms classified as adenocarcinoma not otherwise specified (NOS) are a histologically heterogeneous group: they vary from highly anaplastic adenocarcinoma to moderate and well-differentiated lesions. This report describes a case of adenocarcinoma of MSG occurring in the buccal mucosa of a 24-year-old male. The patient

BACKGROUND: Sentinel lymph node biopsy (SLNB) is rapidly becoming the standard of care in the treatment of women with early stage breast cancer. Male breast cancer although relatively rare, has typically been treated with mastectomy and axillary lymph node dissection (ALND). Men who develop breast carcinoma have the same risk as their female counterparts of developing the morbidities associated with axillary dissection. SLNB has been championed as a procedure aimed at preventing those morbidities. We recently have evaluated the role of SLNB in the treatment of men with early stage breast cance

BACKGROUND: Multifocal or multicentric breast cancer has been suggested as a contraindication for sentinel node biopsy (SNB). However, recent studies have demonstrated that all quadrants of the breast drain through common afferent channels to a common axillary sentinel node. This should mean that the presence of multifocal tumour should not affect the lymphatic drainage. The purpose of this study was to evaluate the feasibility and accuracy of SNB in patients with multifocal breast cancer using a peritumoural injection technique for sentinel lymph node (SN) mapping.

In the period 1997-2002, sentinel lymph node (SLN) surgery was performed on 179 primary skin melanoma patients, one to two months after the removal of the primary. Staining with patent blue was combined with an isotope technique. Histological evaluation of the sentinel lymph nodes was performed in serial sections. Immunohistochemical detection of S100, HMB-45, or Melan-A was used in the case of suspected micrometastases. Demonstration of positive sentinel lymph node was followed, preferably within 2-3 weeks, by regional block dissection. In these cases interferon-a2 in low doses or BCG immune

Lymphatic mapping and sentinel lymph node mapping have changed the standard of surgical care for patients with melanoma and have the potential to do the same for women with breast cancer. The technique will result in more accurate disease staging and in less morbidity. Preoperative lymphoscintigraphy and the intraoperative mapping technique are being standardized across the country. With the lymphatic mapping technique, approximately 10% of the so-called histologic negative population will be more accurately staged. National trials are ongoing to determine the clinical significance of the miss

The purpose of this study was to evaluate enhancement (signal loss) of the axillary lymph nodes on MR lymphography after intramammary injection of superparamagnetic iron oxide (Ferumoxides) for detection of the sentinel lymph node. MR lymphography was performed in a total of 11 patients with breast cancer without palpable axillary lymph node swelling before operation. Coronal and axial images were obtained before and after intramammary injection of 1.5 ml Ferumoxides adjacent to the breast tumor. In all patients, decreased intensity was recognized in the axillary lymph nodes. MR lymphography c

Elective versus therapeutic lymph node dissection has been a controversial field of the surgical treatment of cutaneous malignant melanoma for more than two decades. The identification and biopsy of the sentinel lymph node in different solid malignancies has become feasible by the method described by Morton in 1992. The sentinel lymph node is the first tumor draining lymph node in the regional lymph node basin. If metastasis is not proven in the sentinel node by detailed histological study those are unlikely in other regional lymph nodes and formal lymph node dissection can be omitted. Patient

BACKGROUND AND OBJECTIVES: Sentinel lymph node (SLN) biopsy is not usually performed with respect to the internal mammary lymph node chain. However, the SLN may be located in the internal mammary chain, particularly with medial lesions. We carried out this study to investigate whether lymphatic mapping and SLN biopsy can detect internal mammary involvement in patients with breast cancer.
METHODS: A dye- and gamma probe-guided SLN biopsy was performed in a consecutive series of 41 patients with tumor in situ or clinical stage I or II breast cancer. After the biopsy, these patients underwent ei

A total of 40 patients (mean age 51 yrs; 36-89 yrs) with clinically T1-T2(< 3 cm)N0M0 breast cancer underwent sentinel node (SN) mapping with radioactive tracer (99mTc) injection only in 21 patients, with Patent blue V in 1 patient, or with both techniques in 19 patients. The preoperative injection of 99mTc (20-40 MBq) was followed by lymphoscintigraphy. A handheld gamma probe was used to detect the SN in the operative room. A lumpectomy and an axillary dissection were performed in all the patients. SNs could be identified in 39/40 patients, resulting in a sensitivity of 98%. Successful locali

Recent publication of the results of clinical trials in which lymph node dissection was not associated with any survival benefit in patients with sentinel node metastasis makes it necessary to reconsider the treatment of patients with melanoma. This article provides an update on the available evidence on the diverse factors (routes of metastatic spread, predictors, adjuvant therapy, etc.) that must be considered when treating patients with sentinel node-positive melanoma. The authors propose a decision-making algorithm for use in this clinical setting. The current evidence no longer supports l

BACKGROUND: The objective of this study is to assess the oncologic safety of sentinel lymph node biopsy (SLNB), especially with regard to the axillary recurrence (AR) rate, and to determine the risk factors for AR and disease-free survival (DFS) and overall survival (OS) after negative SLNB.
MATERIALS AND METHODS: Between 2003 and 2006, a total of 1626 patients with invasive breast cancer and clinically axillary node-negative tumors underwent SLNB using a radioisotope at the Asan Medical Center. Of these patients, 1196 were negative on SLNB. Among these, 709 underwent SLNB only, and 487 under

BACKGROUND: A regional nodal recurrence is a major concern after a sentinel lymph node biopsy (SLNB) alone in patients with breast cancer. In this study we investigated patterns and risk factors of regional nodal recurrence after SLNB alone.
PATIENTS AND METHODS: Between January 1999 and March 2005, a series of 1,704 consecutive breast cancer cases in 1,670 patients (34 bilateral breast cancer cases) with clinically negative nodes or suspicious nodes for metastasis who underwent SLNB at a single institute (Saitama Cancer Center) were studied. All 1,704 cases were classified based upon presenc

Inguinal lymph node involvement is the most robust predictor of mortality in vulval cancer and sentinel lymph node (SLN) biopsy is a safe diagnostic modality. We describe a technique based on anatomical landmarks. Our aims are (1) to describe the territorial mapping of SLNs, (2) to evaluate the associated complication rate vis-à-vis those reported in the literature, and (3) to assess its accuracy in detecting SLNs. Data from women who underwent groin SLN biopsies for vulval cancer at a single cancer centre were prospectively collected. All subjects met the GROINSS-V eligibility criteria. The l

INTRODUCTION: Sentinel lymph node (SLN) biopsy is useful for the prognostic stratification of patients with thick melanoma. Identifying which variables are associated with SLN involvement and establishing risk in different subgroups of patients could be useful for guiding the indication of SLN biopsy. The value of complete lymph node dissection (CLND) in patients with a positive SLN biopsy is currently under debate.
MATERIALS AND METHODS: To identify factors associated with SLN involvement in thick melanoma we performed a multicentric retrospective cohort study involving 660 patients with thi

The surgical treatment of vulvar cancer has undergone many changes over the last century. The morbidity of open inguinal incisions prompts the search for a minimally invasive approach to lymph node dissection. This study reports the outcomes of 4 patients with vulvar cancer undergoing robotic sentinel lymph node (SLN) mapping and lymph node (LN) dissection with near-infrared fluorescence. From 2015 to 2017, 3 patients with squamous cell carcinoma of the vulva underwent robot-assisted SLN mapping and inguinal LN dissection. One patient with a vulvar melanoma had robotic bilateral SLN mapping on
