Neglected Acromion Fracture Causing Chronic Post-traumatic Shoulder Pain - A Case Report.

INTRODUCTION: Osteomyelitis pubis is a rare disease that presents with fever, pubic or hip pain, pain during hip movement, and claudication to avoid pain. It most frequently affects young athletes involved in sports that require twisting and cutting, but its exact cause is unknown and no definite treatment has been established. We report a case of pubic osteomyelitis in a young football player who was treated using the Masquelet technique and antibiotics with a good clinical course. CASE REPORTS: A 22-year-old male football player in a national-level university club team visited a nearby clin

Fracture of the humeral surgical neck and shoulder dislocation following an epileptic seizure: Case report.

Epilepsy is a pathological condition characterized by seizures of muscle tension and convulsions in which the patient is unable to control himself, resulting in various complications and injuries. In this paper we talk about a rare case that combines a shoulder dislocation with a fracture of the surgical neck of the humerus after an epileptic seizure. The patient came to the hospital with clinical symptoms directed at dislocating the shoulder after an epileptic seizure, but careful examination and radiography revealed the presence of a fracture of the surgical neck of the humerus, so the manag

Open Reduction of a Fracture-Dislocation of the Shoulder in an 11-Month-Old Child: A Case Report.

CASE: An 11-month-old boy was brought to the emergency department because he was refusing to use his right arm. X-rays demonstrated an anteroinferior Salter-Harris I fracture-dislocation of the proximal humeral epiphysis. Closed reduction with sedation was unsuccessful, so open reduction under general anesthesia was pursued. At 2 years, the patient has made a full recovery. CONCLUSION: Pediatric shoulder fracture-dislocations are rare. Clinicians must have a high index of suspicion for nonaccidental trauma when evaluating such injuries. When this injury is encountered in patients younger than

Management of Traumatic Coracoid Fracture and Anterior Shoulder Instability With a Modified Arthroscopic Latarjet Technique.

Coracoid fractures are an uncommon injury and typically occur in the setting of high-energy trauma. Isolated injury to the coracoid is rare; therefore, a high suspicion for concomitant shoulder injuries should exist. These associated injuries have been shown to be acromioclavicular dislocations, clavicular and acromial fractures, scapular spine fractures, rotator cuff tears, and anterior shoulder dislocations. Although most of these shoulder injuries respond to nonsurgical treatment, there are case reports and literature reviews that present more complicated injuries requiring surgical interve

Should closed reduction be attempted in the emergency department for an acute anterior shoulder fracture dislocation?

BACKGROUND: Whether an anterior shoulder fracture dislocation should be reduced under sedation in the emergency department is still a dilemma. This retrospective study aimed to determine when it is safe to perform a closed reduction based on the fracture pattern. METHODS: Surgically treated anterior shoulder fracture dislocations over eight years were classified into three groups. Group 1 involved an isolated greater tuberosity fracture. Group 2 and 3 involved surgical and or anatomical neck fractures. In group 2, the head and the shaft fragments were displaced together anteriorly and inferio

The dual subscapularis procedure: a modified Hawkins' technique for neglected posterior fracture/dislocation of the shoulder.

BACKGROUND: Posterior fracture/dislocation of the shoulder is a rare injury that is frequently missed on initial assessment. It is frequently associated with reverse Hill-Sachs impression fracture. Several orthopaedics procedures have been described on the literature for reconstructable reverse Hill-Sachs lesion. The McLaughlin's procedure and its modifications, anatomic bone grafting procedures, rotational osteotomies, and the remplissage technique were reported by many authors. We advocated a new "dual subscapularis procedure" that consists of the Hawkins lesser tuberosity transfer with addi

[Reverse total shoulder arthroplasty for the treatment of old fracture of proximal humerus in elderly patients].

OBJECTIVE: To investigate clinical results of reverse total shoulder arthroplasty in treating old fracture of proximal humerus in elderly patients. METHODS: From January 2012 to December 2017, 12 elderly patients with old proximal humeral fractures were treated with reverse total shoulder arthroplasty. There were 5 males and 7 females with an average age of 70.2 years old (ranged from 63 to 81 years old) and an average course of 12.3 months (ranged from 9 to 18 months). VAS score was used to evaluate the degree of pain relief of shoulder joint, Constant-Murley score was used to evaluate the i

Is Acromial Fracture after Reverse Total Shoulder Arthroplasty a Negligible Complication?: A Systematic Review.

BACKGROUND: The purpose of this study was to investigate the incidence of acromial fracture after reverse total shoulder arthroplasty (RTSA) and clinical and radiological outcomes of treatment of the fracture. METHODS: A systematic review was performed to identify studies that reported the results of treatment of acromial fractures after RTSA. A literature search was conducted by two investigators using four databases (PubMed, Embase, Cochrane, and Ovid Medline). RESULTS: Fifteen studies (2,857 shoulders) satisfied our inclusion criteria. The incidence of acromial fracture after RTSA was 4.0

When is it safe to reduce fracture dislocation of shoulder under sedation? Proposed treatment algorithm.

INTRODUCTION: Shoulder dislocations are common. It is known that incongruent shoulder should be promptly reduced. However, when associated with fracture of the proximal humerus, there is a clinical dilemma if reduction under sedation is a safe option. We wanted to establish when it is safe to attempt reduction of a shoulder fracture dislocation under sedation in emergency room. METHODS: This is a retrospective cohort study assessing consecutive patients presenting with a dislocation of a gleno-humeral joint with an associated fracture of the humerus between 2007 and 2015. The radiographs and

Reverse Shoulder Arthroplasty for Malunions of the Proximal Part of the Humerus (Type-4 Fracture Sequelae).

BACKGROUND: The treatment of fracture sequelae of the proximal part of the humerus in combination with posttraumatic arthritis is challenging. The reported results of treatment with anatomic shoulder arthroplasty are disappointing. The aim of this multicenter study was to analyze the clinical and radiographic results of reverse shoulder arthroplasty for treatment of posttraumatic sequelae of the proximal part of the humerus with malunion of the tuberosities. METHODS: This was a retrospective, multicenter study of 42 patients (42 shoulders) with the diagnosis of posttraumatic sequelae of the p

Comparison of hemiarthroplasty and reverse shoulder arthroplasty for the treatment of proximal humeral fractures in elderly patients.

BACKGROUND: Treatment of complex three and four-part proximal humeral fractures with hemiarthroplasty in elderly patients has yielded mixed clinical results. Reverse shoulder arthroplasty has emerged as a treatment option for comminuted proximal humeral fractures for these patients. The purpose of the study was to perform a prospective evaluation of patient outcomes comparing hemiarthroplasty and reverse shoulder arthroplasty for the treatment of comminuted proximal humeral fractures in elderly patients. METHODS: Fifty-three consecutive elderly patients (average age, 74.4 years) underwent an

Timing of shoulder arthroplasty in comminuted proximal humerus fracture, how much does it matter?

Shoulder hemiarthroplasty is a salvage treatment for comminuted fracture of the humerus especially in elderly patients. Several factors contribute to the final outcome like bone quality and tuberosity reposition. Timing of the surgery is considered one of these factors. This study was done to assess the effect of delaying the surgery up to 2 weeks on the final outcome. This retrospective study was done on 33 patients with four-part fracture of the humerus, divided into two groups, group 1 (17 patients) who had surgery within the first 3 days after trauma, and group 2 (16 patients) who had surg

Ipsilateral nonunions of the coracoid process and distal clavicle--a rare shoulder girdle fracture pattern.

Coracoid fractures are uncommon injuries, in isolation or in association with other osseoligamentous injuries about the shoulder girdle. We report a case of successful operative management of symptomatic ipsilateral nonunions of a type I coracoid base fracture and a lateral one-third clavicular fracture, which developed following nonoperative treatment of this exceedingly rare injury pattern. Following open distal clavicle excision and reduction of the coracoclavicular interval with screw fixation, radiographic union and excellent clinical outcome were achieved. This rare and potentially troub

A comparison of the Constant and Oxford shoulder scores in patients with conservatively treated proximal humeral fractures.

The Oxford shoulder scores (OSS) and Constant scores were collected prospectively for 103 patients treated conservatively for proximal humeral fractures. The paired scores were analyzed by assessing the degree of correlation, the agreement between the scores, and their sensitivity to change. Linear regression analysis was also performed. The scores correlated well with a coefficient of .84 (P < .001), demonstrating a clear relationship between the scoring systems. There was good agreement between the scores and both were sensitive to change over time (P < .001). We have also produced a regress

Handedness as a predictor of increased risk of knee, elbow, or shoulder injury, fractures and broken bones.

Several recent studies suggest that left-handers have higher susceptibility to accidental injury. Two studies investigated the suggested link between sinistrality and accidental injury by assessing the prevalence of joint problems, bone breaks, and fractures, which may be the sequelae of such mishaps. Study 1 used retrospective responses on medical history checklists completed by 1064 males on active duty with the United States Air Force. It found that left-handers were at significantly higher risk for knee problems (RR = 2.74), elbow and shoulder problems (RR = 3.03), and also swollen and pai

Reverse shoulder arthroplasty with glenoid bone grafting for anterior glenoid rim fracture associated with glenohumeral dislocation and proximal humerus fracture.

BACKGROUND: Large fractures of the anterior glenoid rim can result in persisting instability and osteoarthritis of the glenohumeral joint When this fracture is associated with a glenohumeral dislocation and proximal humerus fracture could be a concern. The goal of this paper was to evaluate the clinical and radiological outcomes and complications of reverse shoulder arthroplasty (RSA) and glenoid bone graft in cases with a significant anterior glenoid fracture associated with a proximal humerus fracture. HYPOTHESIS: RSA and step bone graft harvested from proximal humeral head could be a viabl

[Translated article] Influence of prosthetic lateralization on tuberosity healing in reverse shoulder arthroplasty for proximal humerus fractures.

INTRODUCTION: To evaluate the effect of prosthetic lateralization on tuberosity healing following reverse shoulder arthroplasty (RSA) for proximal humerus fractures (PHFs) in older adults. MATERIAL AND METHODS: This single-center retrospective study included patients aged >65 years with displaced Neer 3- or 4-part PHFs treated with RSA within four weeks of injury. Patients were allocated according to the global lateralization of the prosthesis: very highly lateralized (VHL; 21.4mm, Arrow®, n=53) and lateralized (L; 14.7mm, Unique®, n=20). Tuberosity healing was assessed radiographically and c

Medialized vs. Lateralized Reverse Total Shoulder Arthroplasty for Proximal Humerus Fractures: A Systematic Review and Meta-Analysis.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is increasingly used in the treatment of proximal humerus fractures (PHFs) with reliable clinical improvement. Lateralized RSA implants have conferred superior outcomes compared with the original Grammont design in patients with nontraumatic indications. However, in the setting of a PHF, lateralized components can place increased tension across the tuberosity fracture site and potentially compromise tuberosity healing and outcomes. This systematic review and meta-analysis sought to determine the effect of implant design on clinical outcomes after
