Assessment of the intrasinusidal volume before and after maxillary sinus augmentation using mri - a pilot study of eight patients.

PURPOSE: The purpose of this study was to evaluate the suitability, accuracy, and reliability of a non-invasive 3-Tesla magnetic resonance imaging technique (3 T-MRI) for the visualization of maxillary sinus grafts in comparison to conventional, X-ray-based, established standard imaging techniques. METHODS: A total of eight patients with alveolar bone atrophy who required surgical sinus floor augmentation in the course of dental implantation were included in this pilot study. Alongside pre-operative cone-beam computed tomography (CBCT), 3 T-MRI was performed before and 6 months after sinus fl…

The Palatal Approach for Direct Maxillary Sinus Floor Elevation: A Case Report.

INTRODUCTION: The traditional techniques of maxillary sinus floor elevation via a direct or indirect approach are suitable for the majority of cases. However, in cases of unfavorable anatomy and/or a thick lateral bony wall, we propose here a new approach for sinus floor elevation. CASE PRESENTATION: Forty-two-year-old female presents for sinus floor elevation after a failed attempt due to anatomical limitations and intraoperative complications. During the second surgical procedure, the access to the sinus membrane was performed from the palatal side due to thickness of the buccal wall rangin

The Maxillary Sinus Floor Elevation Using a Poly-L-Lactic Acid Device to Create Space Without Bone Graft: Case Series Study of Five Patients.

Maxillary sinus floor elevation using autologous or alloplastic bone grafting is often performed for implant treatment of maxillary molars; however, issues related to the donor site and complications such as infection have been reported. We performed maxillary sinus floor elevation using poly-L-lactic acid (PLLA) as a space-making material in patients with an insufficient bone mass (<3 mm) for simultaneous implantation between the alveolar crest and floor of the maxillary sinus and evaluated the newly formed bone. Conventional antrostomy of the maxillary sinus from the lateral wall was perform

The patient's perception of recovery after maxillary sinus augmentation: a prospective study.

BACKGROUND: Little information is available that documents a patient's perception of recovery after sinus-floor augmentation. The aim of the present prospective study was to evaluate the patient's perception of immediate postoperative recovery after sinus-floor augmentation. METHODS: Seventy-six patients (41 males and 35 females) who had been scheduled for sinus-floor augmentation were asked to enroll in a prospective clinical study. A health-related quality-of-life questionnaire was given to the patient, which was designed to assess patient perception of recovery in four main areas: pain, or

A Modified Transalveolar Sinus Floor Elevation Approach With a Bilaterally Enlarged Osteotomy.

Sinus floor elevation is the most commonly used method for vertical bone augmentation in the maxillary posterior area. This clinical report describes a modified transalveolar approach to elevate the Schneiderian membrane when placing implants on a severely resorbed maxillary posterior ridge with a buccal-palatal width of more than 8.0 mm. In this approach, the osteotomy prepared on the crestal is bilaterally enlarged to 8.0-10.0 mm. The enlarged osteotomies provide better access, allowing instruments to be in direct contact with the bone of the sinus floor and thus elevate the Schneiderian mem

Sinus Floor Elevation via an Osteotome Technique without Biomaterials.

According to classic Hirschfeld studies, the first teeth to be lost are the first and second maxillary molars. After the teeth are extracted and the alveolar process is developed, the maxillary sinus is reabsorbed and pneumatized with a decrease in bone availability in the posterior sector of the maxilla. This process often creates the need to perform regeneration techniques for the placement of implants in this area due to the low availability of bone. The most frequently used and documented technique for the elevation of the sinus maxillary floor is elevation by the side window, as proposed

Microscope and micro-camera assessment of Schneiderian membrane perforation via transcrestal sinus floor elevation: A randomized ex vivo study.

OBJECTIVE: We sought to assess the effectiveness of using a microscope and non-invasive camera for assessing sinus membrane perforations during transcrestal sinus floor elevation (TSFE). MATERIALS AND METHODS: Five fresh human cadaver heads corresponding to eight maxillary sinuses (six bilateral and two unilateral) underwent 4 TSFEs per sinus (a total of 32 single site elevations). Each elevation was randomly assigned to receive a three or six mm membrane elevation height (MEH). A microscope and micro-camera were used to assess the sinus membrane perforation. Afterwards, radiological and clin

Risk factors contributing to membrane perforation in lateral window maxillary sinus elevation: systematic review and meta-analysis.

OBJECTIVE: To scrutinize the risk factors related to membrane perforation during lateral window preparation for maxillary sinus elevation. STUDY DESIGN: The investigation was conducted across different electronic databases, followed by a critical evaluation of studies that matched the inclusion criteria. RESULTS: The studies identified several risk factors contributing to sinus membrane perforation. Thin membranes (<1.5 mm) are at significant risk of perforation compared to thicker ones. Smoking significantly increased the likelihood of perforation by 1.6 times, with one perforation occurrin

Histologic, Histomorphometric, and Clinical Analysis of the Effects of Growth Factors in a Fibrin Network Used in Maxillary Sinus Augmentation.

This randomized controlled clinical trial evaluated the effect of mineralized plasmatic matrix (MPM), comprised of synthetic graft and platelet concentrates, on new bone formation and volume stability over time in maxillary sinus lifting (MSL). Unilateral MSL was performed in 20 patients with either beta-tricalcium phosphate (β-TCP) or MPM grafts (10 sinuses each). Six months postsurgery, specimens were obtained with a trephine bur prior to implant placement in 39 cases. Volumetric changes in sinus augmentation were analyzed between 1 week (T-I) and 6 months (T-II) postsurgery. Histomorphometr

The Sinus Membrane-Maxillary Lateral Wall Complex: Histologic Description and Clinical Implications for Maxillary Sinus Floor Elevation.

Maxillary sinus floor elevation has been documented as a safe and predictable procedure for gaining vertical bone height in the atrophic posterior maxillae. Conversely, there is a lack of basic research on the characteristics of the union between the sinus membrane (SM) and the bone. Clinical implications of an impaired union in healthy or pathologic membranes remain unknown. The objective of this study was to present a comprehensive histologic and morphologic description of the sinus membrane-lateral bone wall complex. In 14 fresh cadaver heads, 28 lateral wall sinus augmentation procedures w

Computerized tomography-assisted calculation of sinus augmentation volume.

PURPOSE: This study was intended to calculate the augmentation volume for a sinus lift procedure based on cross-sectional computerized tomography (CT) scans for 2 different augmentation heights. MATERIALS AND METHODS: Based on area calculations of cross-sectional CT scans, the volume of additional bone needed was calculated for 44 sinus lift procedures. The amount of bone volume needed to raise the sinus floor to heights of both 12 and 17 mm was calculated. RESULTS: To achieve a sinus floor height of 12 mm, it was necessary to increase the height by a mean of 7.2+/-2.1 mm (range, 3.0 to 10.5

Three-dimensional analysis of bone formation after maxillary sinus augmentation by means of microcomputed tomography: a pilot study.

PURPOSE: Although many studies have analyzed the suitability of different grafting materials for maxillary sinus augmentation by means of histomorphometry in conventional histologic strains, the three-dimensional (3D) structure and remodeling of these grafts after healing beneath the sinus membrane remain unclear. The aim of the present study was to determine whether microcomputed tomography is a suitable method to evaluate the 3D structure and remodeling of grafts after sinus floor augmentation. MATERIALS AND METHODS: Sinus floor augmentation was performed in five patients using autogenous b…

[Use of bone blocks by sinus floor lifting].

The paper presents clinical cases illustrating two sinus floor lifting technique performed for dental implant placement in areas with maxillary sinus floor atrophy: bone block fixation for vertical augmentation (used for immediate implant placement by sinus floor atrophy to 1 mm thickness) and perforation closure. Sinus floor perforations wider than 5 mm were repaired by mandible bone graft providing the possibility of immediate implantation. Bone block was fixed between cortical layers of the maxilla with no internal fixation. Free bone blocks provide additional primary stability of dental im

Application of 3D-Printed Implant-Osseous-Membrane Guide for One-Stage Sinus Floor Elevation: A Clinical Report.

Sinus floor elevation with the lateral window approach has proven to be an effective treatment modality for vertical bone augmentation in the posterior region of the maxilla. The simultaneous implant placement during the procedure can be achieved if enough remaining bone height is available to obtain implant primary stability. However, the proper identification of the maxillary sinus boundaries for the window demarcation along with membrane protection for simultaneous implant placement can be challenging. This clinical report demonstrates a novel technique for sinus floor augmentation using a …
