Efficacy of 10-day melarsoprol schedule 2 years after treatment for late-stage gambiense sleeping sickness.

In 2000, we reported that a new short treatment schedule of melarsoprol was not worse than the longer and demanding standard treatment for late-stage human African trypanosomiasis. This alternative schedule was assessed in an open, randomised clinical equivalence trial of 500 patients in Angola. 24 h after treatment, all patients were parasite free. Of 442 patients, 12 (3%) had relapsed after 1 year, of whom seven (3%) had had standard treatment and five (2%) the alternative treatment. After 2 years, 23 (5%) relapsing patients were reported, 11 (5%) in the standard treatment group and 12 (6%)

Neuro-inflammatory risk factors for treatment failure in "early second stage" sleeping sickness patients treated with pentamidine.

In a clinical trial on efficacy of Pentamidine in second stage Trypanosoma brucei gambiense patients with </=20 cells/microl in cerebrospinal fluid (CSF), 43% of treatment failures were observed. We hypothesised that unsuccessful treatment was caused by uncured brain infection. The relationship between treatment outcome and CSF cell count, protein concentration, presence of trypanosomes, the intrathecal immune response, and CSF total IgM and trypanosome specific antibodies detected by LATEX/IgM and LATEX/T.b. gambiense card agglutination tests was examined. Cell counts of 11-20 cells/microl, i

Development and Expert Consensus of a Stage-Based, Needs-Oriented Non-Pharmacological Sleep Management Program for Liver Transplant Recipients.

PURPOSE: Sleep disturbance is common among liver transplant recipients and is associated with impaired daytime functioning, psychological distress, impaired post-transplant recovery, and reduced quality of life. However, sleep-related problems and care needs may change across the transplant trajectory, whereas existing non-pharmacological interventions have not been systematically structured to address these stage-specific needs. This study aimed to develop a stage-based, needs-oriented, non-pharmacological sleep management program for liver transplant recipients. METHODS: This methodological

Outcomes in single-stage multilevel surgery for obstructive sleep apnea: Transoral robotic surgery, expansion sphincter pharyngoplasty and septoplasty.

BACKGROUND: Many patients with obstructive sleep apnea syndrome (OSAS) have multiple obstructive sites simultaneously such as the oropharynx, hypopharynx, and larynx. Multilevel surgery is starting to be widely performed by ENT surgeons and accepted by patients. METHODS: Twenty consecutive patients with moderate or severe OSAS were treated with single-stage multilevel surgery. They underwent transoral robotic surgery for tongue base reduction or epiglottoplasty, expansion sphincter pharyngoplasty, and septoplasty. RESULTS: The average length of hospitalization was 5.2 ± 0.9 days. No serious

Biological data and clinical symptoms as predictors of astrogliosis and neurodegeneration in patients with second-stage Trypanosoma brucei gambiense sleeping sickness.

Concentrations of glial fibrillary acidic protein (GFAp) and light subunit neurofilament protein (NFL) in cerebrospinal fluid (CSF) were measured in patients with second-stage Trypanosoma brucei gambiense sleeping sickness. Correlations between GFAp and NFL in CSF as markers for astrogliosis and neurodegeneration, and clinical and biological data were investigated. Abnormal levels of GFAp and NFL were significantly associated with increasing CSF cell number and protein concentration, and with the absence of lymph nodes or the absence of trypanosomes in lymph node aspirate. Late-stage interventional trials reporting sleep-related outcomes in older adults with Parkinson's disease: a ClinicalTrials.gov registry review.

BACKGROUND: Sleep disturbances are among the most disabling non-motor symptoms in Parkinson’s disease (PD), particularly affecting older adults, in whom age-related vulnerability and multimorbidity may exacerbate sleep dysfunction. Despite their high prevalence, interventional evidence reporting sleep-related outcomes in PD remains incompletely characterized, and methodological heterogeneity limits the comparability of findings across studies. This registry-based descriptive review aimed to characterize the landscape of late-stage interventional trials reporting sleep-related outcomes in PD, w

Cortical and subcortical gray matter alterations underlying sleep disturbances in patients with end-stage renal disease undergoing hemodialysis.

Although several studies have indicated a connection between cortical and subcortical structural abnormalities and sleep disturbances (SD) in patients with end-stage renal disease undergoing hemodialysis (HD), detailed characterization of these abnormalities remains unclear. In this study, twenty HD patients with SD (HD-SD), 20 HD patients without SD (HD-NSD) and 22 healthy controls were recruited and underwent clinical assessments, neuropsychological tests, and 3.0T MRI scans. Vertex-based analyses of cortical thickness, surface area and volume were conducted to examine cortical abnormalities

Disease-stage-specific immunometabolic remodeling in pediatric obstructive sleep apnea: a single-cell transcriptomic atlas of adenoid tissue.

Hypertrophied adenoids in children can impair breathing and lead to obstructive sleep apnea (OSA), often accompanied by abnormal growth and weakened stamina and immunity. However, the cause of the pathological transformation in these originally immune-enhancing lymphoid tissues remains unclear. Our study provides the first single cell transcriptomic and immune repertoire atlas of adenoids from normal snoring to mild, moderate, and severe OSA, and identified markedly asynchronous functional modules, transcriptional regulatory networks and intercellular communications during the progression of O

Stage-specific EEG spectral signatures of primary insomnia: regional and sex-related differences across NREM and REM sleep.

Sleep architecture is characterized by dynamic transitions between non-rapid eye movement (NREM) and rapid eye movement (REM) stages, each defined by distinct oscillatory signatures. Although quantitative EEG has been widely used to study primary insomnia (PI), reported findings remain inconsistent, partly due to limited consideration of cortical regional differences, sleep-stage specificity, and sex-related variability. This study integrates spectral power analysis with explainable machine learning to provide a comprehensive characterization of neural dynamics in PI. EEG recordings from 100 a

Validation of a deep learning-based system for sleep staging, arousal detection, and respiratory event scoring in atrial fibrillation patients.

PURPOSE: Obstructive sleep apnea is a common yet underdiagnosed sleep disorder in patients with atrial fibrillation (AF). Although polysomnography (PSG) is the gold standard, its costs and complexity limit routine use. Home sleep apnea testing (HSAT) is a more accessible alternative, but it lacks electroencephalography (EEG), limiting direct assessment of sleep stages and arousals. This study evaluates the performance of DeepRESP, an automated scoring solution that infers sleep stages and arousals using respiratory inductance plethysmography (RIP) signals and scoring respiratory events, in a c…

Validation of non-REM sleep stage decoding from resting state fMRI using linear support vector machines.

A growing body of literature suggests that changes in consciousness are reflected in specific connectivity patterns of the brain as obtained from resting state fMRI (rs-fMRI). As simultaneous electroencephalography (EEG) is often unavailable, decoding of potentially confounding sleep patterns from rs-fMRI itself might be useful and improve data interpretation. Linear support vector machine classifiers were trained on combined rs-fMRI/EEG recordings from 25 subjects to separate wakefulness (S0) from non-rapid eye movement (NREM) sleep stages 1 (S1), 2 (S2), slow wave sleep (SW) and all three sl…
