Ethics and policy perspectives on personalized medicine in the post-genomic era.

The completion of the human genome project and the accompanying biotechnological revolution hold great promise for the creation of pharmaceutical agents to combat not only previously incurable diseases, but also those for which therapeutics exist, yet they either cause severe adverse effects or exhibit no benefit in subsets of the population. In many instances variation in therapy response can be attributed to genetic differences, more particularly single nucleotide polymorphism (SNPs). Detection of the genetic differences which affect drug response, commonly referred to as pharmacogenomics, m

Whole genome sequencing for the diagnosis of neurological repeat expansion disorders in the UK: a retrospective diagnostic accuracy and prospective clinical validation study.

BACKGROUND: Repeat expansion disorders affect about 1 in 3000 individuals and are clinically heterogeneous diseases caused by expansions of short tandem DNA repeats. Genetic testing is often locus-specific, resulting in underdiagnosis of people who have atypical clinical presentations, especially in paediatric patients without a previous positive family history. Whole genome sequencing is increasingly used as a first-line test for other rare genetic disorders, and we aimed to assess its performance in the diagnosis of patients with neurological repeat expansion disorders. METHODS: We retrospe

The complete genome sequences of Erythroxylum coca and Erythroxylum novogranatense.

The flowering plant genus Erythroxylum contains approximately 300 species, including the economically and socially consequential crops called coca. We present the genome sequences of Erythroxylum coca and E. novogranatense, two cultigens produced for medicinal and quotidian use in the Andes and Amazon regions of South America, as well as the international cocaine industry. Sequencing was performed on an Illumina X-Ten platform, and reads were assembled by a de novo method followed by finishing via comparison with several species from the same genus. The BioProject, raw and assembled data can b

Practical and ethical issues with genetic screening.

Clinical hematologists are faced with a growing list of new genetic-based tools for identifying a patient's risk of disease. While many of the disease-specific tests are readily available, validation studies are required. Furthermore, genetic-based tests are being pushed to their technical limits, such as testing a single cell prior to embryo selection and transfer for couples at risk of genetic disease. As a result, misdiagnosis or misinterpretation of the data may result. As new genetic testing opportunities proliferate, the hematologist needs to be aware of the medical and legal issues surr

Association between a high-risk autism locus on 5p14 and social communication spectrum phenotypes in the general population.

OBJECTIVE: Recent genome-wide analysis identified a genetic variant on 5p14.1 (rs4307059), which is associated with risk for autism spectrum disorder. This study investigated whether rs4307059 also operates as a quantitative trait locus underlying a broader autism phenotype in the general population, focusing specifically on the social communication aspect of the spectrum. METHOD: Study participants were 7,313 children from the Avon Longitudinal Study of Parents and Children. Single-trait and joint-trait genotype associations were investigated for 29 measures related to language and communica

Complex maternal genetic landscape in the Koṅkaṇī population of India.

The Koṅkaṇ region on the west coast of India is a hotspot of culture, folklore and ethnolinguistic diversity, yet its maternal genetic landscape remains largely unresolved. The present study captures the genetic affinities of the Koṅkaṇī population, residing along the Koṅkaṇ Malabar coast by analysing complete mitochondrial genomes (n = 85) and control region hypervariable sequences (n = 210). Our results reveal divergent maternal histories between the Sārasvata and the non-Sārasvata groups. Comparative analysis against over 5000 mitochondrial genomes from Eurasia demonstrates that the Koṅkaṇī

Complete Mitogenomes of the Critically Endangered Hainan Gibbon (Nomascus hainanus) Reveal Very Low Mitochondrial Diversity and Two Major Maternal Clades.

The Hainan gibbon (Nomascus hainanus) is one of the most threatened primates, yet its mitochondrial haplotype diversity and genealogy remain poorly understood. This study investigated mitochondrial haplotype structure, genealogical history, and selective constraints on mitochondrial protein-coding genes using complete mitochondrial genomes. We generated mitogenomic data from 13 wild individuals representing five of seven extant social groups, including the recently formed F and G groups, which were established through natural dispersal and had not previously been genetically characterized. Behavioral Testing Design for Evaluation of Cognitive Disabilities.

Intellectual disabilities (ID) constitute a class of human neurodevelopmental diseases and are a major medical and socioeconomic problem owing to their high incidence and enormous burden to the families of those affected. In the past three decades, mutant mouse technologies have provided powerful tools for elucidating the pathophysiological mechanisms underlying behavioral and developmental alterations related to IDs and for addressing new therapeutic strategies, and major progress has been made revealing previously unidentified genes involved in ID. However, the pathological hallmarks of IDs

Chronic psychosocial stress: A primary driver of sleep apnea onset in rats and humans.

Poor sleep quality and chronic psychosocial stress are interrelated risk factors for hypertension, metabolic disease, and neuropsychiatric disorders. Sleep apnea (SA) is a respiratory disorder that disrupts sleep, affecting ~1 billion adults worldwide. However, the origins of SA remain incompletely understood. Clinically, activation of the stress pathways is often considered a consequence of SA rather than a causal factor. Here, we combined preclinical experiments with Mendelian randomization (MR) analysis to show that chronic psychosocial stress is a primary driver of SA onset. In adult rats,

Assessment of causal association between the socio-economic status and osteoporosis and fractures: a bidirectional Mendelian randomization study in European population.

The correlation between socio-economic status (SES) and bone-related diseases garners increasing attention, prompting a bidirectional Mendelian randomization (MR) analysis in this study. Genetic data on SES indicators (average total household income before tax, years of schooling completed, and Townsend Deprivation Index at recruitment), femoral neck bone mineral density (FN-BMD), heel bone mineral density (eBMD), osteoporosis, and five different sites of fractures (spine, femur, lower leg-ankle, foot, and wrist-hand fractures) were derived from genome-wide association summary statistics of Eu

Autism in three patients with cystic or hyperechogenic kidneys and chromosome 17q12 deletion.

BACKGROUND: We report autism in 3 out of 53 children with cystic or hyperechogenic kidneys and heterozygous 17q12 region deletion encompassing hepatocyte nuclear factor-1beta (HNF1B). RESULTS: They presented mental retardation, social interaction impairments, verbal and non-verbal communication deficits and stereotyped behaviours. Deletion size and location of breakpoints were similar to those reported in patients with renal disease/diabetes only. CONCLUSION: Reciprocal genomic rearrangements of the 17q12 region, reported in patients with mental retardation and epilepsy, could also be involv

[A pseudo-outbreak of pharyngeal gonorrhoea related to a false-positive PCR-result].

Nucleic acid amplification tests, including the polymerase chain reaction (PCR), are sensitive and specific tests that are often used for diagnosing sexually transmitted diseases (STDs). A pseudo-outbreak of pharyngeal gonorrhoea in a group of prostitutes turned out to have been caused by false-positive test results due to commensal oropharyngeal Neisseria species. Specific molecular tests may yield erroneous results. When the results of an STD study have major consequences at a legal or social level, it is advisable, in consultation with a medical microbiologist, to take a sample for culture

State of Cardiovascular Disease and Stroke in Hispanic/Latino Adults in the United States: A Scientific Statement From the American Heart Association.

Cardiovascular disease became the leading cause of death among Hispanic individuals in the United States in 2022. Hispanic adults experience a disproportionate burden of cardiometabolic risk factors, including obesity, diabetes, and dyslipidemia. Hispanic populations are highly heterogeneous, with substantial variations in genetic ancestry and sociocultural influences that shape cardiovascular disease risk and outcomes. The "Hispanic paradox," describing lower cardiovascular disease mortality despite higher risk factor burden, is increasingly recognized as an oversimplification that does not a

The Sickle Africa Data Coordinating Centre (SADaCC): a data science hub for interdisciplinary sickle cell disease research and training.

Sickle cell disease (SCD) is one of the most prevalent monogenic disorders worldwide, with the highest burden in Africa, where ~75% of the 7.74 million global cases occur. Scientific progress in understanding its epidemiology, clinical heterogeneity, and treatment outcomes has been constrained by heterogeneous, non-standardized, and non-interoperable datasets that limit data integration and cross-country analyses. To address this, the Sickle Africa Data Coordinating Centre (SADaCC) was established as the data science hub of the SickleInAfrica consortium to support the development and expansion

How can polygenic inheritance be used in population screening for common diseases?

Advances in genomics have near-term impact on diagnosis and management of monogenic disorders. For common complex diseases, the use of genomic information from multiple loci (polygenic model) is generally not useful for diagnosis and individual prediction. In principle, the polygenic model could be used along with other risk factors in stratified population screening to target interventions. For example, compared to age-based criterion for breast, colorectal, and prostate cancer screening, adding polygenic risk and family history holds promise for more efficient screening with earlier start an

Influenza A deletion-containing viral genome production is altered by metabolites, metabolic signaling molecules, and cyanobacterial extracts.

RNA virus infections generate abundant non-standard viral genomes that have been associated with milder clinical outcomes, leading to research into harnessing them as novel antivirals. We recently showed that altering host cell metabolism away from pro-growth pathways using alpelisib increased the production of influenza A nonstandard viral genomes (specifically deletion-containing viral genomes, DelVGs). To uncover more drugs that could induce increased DelVGs, we subjected influenza infections (A/H1N1 and A/H3N2) to a screen of metabolites, metabolic signaling molecules, and cyanobacterium-d
