[Oddi's sphincterotomy in treatment of primary sclerous cholangitis].

In the paper are presented a total of 9 patients with primitive sclerous cholangitis that were hospitalized in the Surgical Department of the "Caritas" Hospital with this rare nosologic entity. The etiology of the disease is neither precise nor incontroversial. Primitive sclerous cholangitis raises particularly difficult problems, because of the high mortality rate (all patients die before the 6-th year after the diagnosis has been established. The ineluctable evolution of the disease toward secondary billiary cirrhosis has generated major difficulties in the choice of the treatment. Corticoid

Endoscopic sphincterotomy in patients with Billroth II gastrectomy.

We attempted endoscopic sphincterotomy in 35 patients previously subjected to Billroth II gastrectomy, and succeeded in 23 of them. It is often difficult to pass the instrument along the afferent loop and the sphincterotomy incision is seldom placed in an optimal position. Although we had complications in only 2 patpients, it is likely that spincterotomy carries a higher risk in Billroth II gastrectomy patients. It should therefore be reserved for patients who are at high risk for surgery.

Anteromedian external urethral sphincterotomy: technique, rationale and complications.

Experiences are presented with bilateral (3 and 9 o'clock incisions) and anteromedian (12 o'clock incision) external urethral sphincterotomy in 84 patients with neuropathic vesicourethral dysfunctions. Hemorrhage and loss of reflexogenic erections have been notably absent in 31 patients who underwent anteromedian sphincterotomy. The rationale is discussed for the preference of anteromedian over bilateral sphincterotomy based on postoperative complications. The postoperative, radiologically aided cystosphincterometric and electromyographic studies during micturition indicated the adequacy of th…

Risk of post-sphincterotomy bleeding in patients with thrombocytopenia.

BACKGROUND: Reports suggest that the rate of adverse events (AEs) post-endoscopic sphincterotomy (ES) to be as high as 10%, with gastrointestinal bleeding being most common after post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis. OBJECTIVE: The aim of this study was to characterize the incidence of bleeding in patients with thrombocytopenia following ES. DESIGN: Retrospective observational cohort study. METHODS: Patients with thrombocytopenia (defined as <150,000 platelets/μL) who underwent ES between May 2017 and December 2020 were identified at a tertiary care medica…

The treatment of ampullary stenosis by endoscopic sphincterotomy (EST).

A total of 247 patients with a diagnosis of ampullary stenosis were treated by endoscopic sphincterotomy (EST). Through the study period from 1983 to 1990, 70% of patients had remission of all symptoms and 82% of patients had most symptoms ameliorated. Four times as many precuts were required for patients with ampullary stenosis compared with a similar number of patients with common bile duct stones. When retained common bile duct stones are ruled out, ampullary stenosis is the most common cause for recurrent episodes of "biliary-like-colic" in postcholecystectomy patients. Ampullary stenosis …

Endoscopic sphincterotomy in the management of posttraumatic biliary fistula.

Complications after nonoperative management of hepatic trauma are rare but include persistent biliary fistula in 4% of cases. Therapy usually involves surgical drainage or hepatic resection to control the fistula. The authors present a case of hepatic biliary fistula treated nonoperatively with percutaneous drainage and endoscopic sphincterotomy. A 16-year-old girl suffered a grade III parenchymal liver fracture to the right lobe in an automobile accident. She was hemodynamically stable with no coexistent injuries and was treated nonoperatively. Over 2 weeks her total bilirubin rose to 3.2 mg/…

Endoscopic sphincterotomy in the management of biliary-cutaneous fistula.

Endoscopic sphincterotomy was performed on two patients who had biliary-cutaneous fistula after cholecystectomy. The outcome was favorable. The author recommends this procedure as a desirable alternative to laparotomy with secondary biliary surgery.
