Rare Loxoscelism-Associated IgG Coombs-Positive Hemolytic Anemia Treated Successfully With Systemic Corticosteroids.

Loxoscelism-associated hemolytic anemia is a rare but critical complication of brown recluse spider bites. It may lead to various systemic manifestations, including jaundice, dark urine, and anemia-related symptoms, in addition to general loxoscelism symptoms such as skin lesions, fever, myalgia, nausea, and vomiting. Prompt diagnosis is crucial and requires recognizing typical laboratory findings such as low hemoglobin, elevated lactate dehydrogenase, reduced haptoglobin levels, and possibly a positive direct antiglobulin test. There is no definitive guideline for the treatment of loxoscelism…

[Local dermonecrotic loxoscelism in children bitten by the spider Loxosceles reclusa (the "violin" spider)].

This is an observational retrospective study. Our goal is to describe the local dermonecrotic reaction occurring after a spider bite in eleven pediatric patients. In seven (63.7%), the spider was identified as Loxosceles reclusa, and in four, bites were presumptive. The main symptoms and signs were pain, erythema, swelling, blisters, and vasculitis in five patients. There was a significant relationship between the time of onset before the treatment and the severity of the lesions (63.4 hours in the severe cases vs 14.4 hours in the mild cases, p < 0.05), as well the time spent in the emergency

Necrotic Loxoscelism of the Scalp.

A 15 month old boy was referred to our department after he and his mother moved to our country. He had been bitten by a brown recluse spider (Loxosceles reclusa) at the age of 7 months, in Brazil. Initially, he presented to the emergency department of his local hospital with rash in the scalp, near the bite. After a few days, he developed systemic disease with multiorgan failure and was admitted on intensive care unit, where he has been treated for 2 months. During this hospital stay he developed skin necrosis of the scalp, right preauricular, mandibular, and neck regions. He also developed ri

David and Goliath: potent venom of an ant-eating spider (Araneae) enables capture of a giant prey.

It is rare to find a true predator that repeatedly and routinely kills prey larger than itself. A solitary specialised ant-eating spider of the genus Zodarion can capture a relatively giant prey. We studied the trophic niche of this spider species and investigated its adaptations (behavioural and venomic) that are used to capture ants. We found that the spider captures mainly polymorphic Messor arenarius ants. Adult female spiders captured large morphs while tiny juveniles captured smaller morphs, yet in both cases ants were giant in comparison with spider size. All specimens used an effective

Acute generalized exanthematous pustulosis: Analysis of cases managed in a Tunisian tertiary hospital.

Acute generalized exanthematous pustulosis (AGEP) is a severe and life-threatening cutaneous adverse reaction. Drug-induced AGEP is mainly related to antibiotics. More recently, AGEP following spider bites has been increasingly described. Treatment includes withdrawal of the offending drug and supportive care. In Tunisia, data concerning severe cutaneous adverse reactions (SCARs) in general and especially AGEP is lacking. Herein, we conducted a retrospective study to investigate the epidemiological, clinical characteristics and etiologies of AGEP referred to the Dermatology department. Our stu…

Loxoscelism of the eyelids.

Loxoscelism is a reaction to the bite of spiders of the genus Loxosceles. Several species have been found in the United States; the most commonly encountered is L reclusus, the small brown recluse spider. Two types of reactions occur from the bite. In the localized type, necrotic loxoscelism, a cutaneous lesion with extensive gangrene develops. About 25% of patients have systemic manifestations of viscerocutaneous loxoscelism. There is the same local reaction, but, in addition, fever, chills, vomiting, joint pain, and hematologic abnormalities occur. Hemoglobinemia and hemoglobinuria suggest s

A case of intoxication after a bite by Latrodectus tredecimguttatus.

A case of intoxication in Southern Bulgaria after a bite from the venomous spider Latrodectus tredecimguttatus is reported. The development of both local (acute pain, itching erythema, paraesthesiae in the area of the bite) and general (weakness, headache, dizziness, fever, vomiting, myalgia, muscle cramps) symptoms, which passed relatively easily, is described. The clinical picture and treatment are briefly commented on.

[Poisoning by spiders of Loxosceles genus].

Despite the great number of spiders in the world, only a small group of them is capable of producing death in humans. In Argentina, there are only three of the four genera of spiders considered of high risk to humans: Latrodectus is present in rural areas, Phoneutria is restricted to small regions while Loxosceles is distributed throughout the country. Accidents by Loxosceles represent around 4% of the total number produced by venomous animals in Argentina. The bite is accidental and may produce considerable local necrosis with scar formation and ulcers of slow and difficult healing that may r

[Cutaneous necrotic loxoscelism. A case report].

Brown recluse (Loxosceles spp.) spiders are arachnid species known to cause necrotic arachnidism. The envenomation, described as loxoscelism, is associated with localized pain, erythema, and edema followed by the development of necrosis. However, the specific pathophysiological mechanisms by which Loxosceles venom exerts these noxious symptoms are multifactorial and not fully understood. The causative factor for production of necrotic lesions is generally considered to be the enzyme sphingomyelinase D (SMD), which cleaves sphingomyelin to form choline and ceramide 1-phosphate. Four active form

Loxosceles arizonica bite associated with shock.

Envenomation by the brown recluse spider (Loxosceles reclusa) is associated with shock, significant hemolysis, renal insufficiency, and disseminated intravascular coagulation (DIC). Shock has never been associated with envenomation by L arizonica, a related species indigenous to Arizona, southern California, and northwestern Mexico. We report the case of a 13-year-old girl, bitten by a specimen of L arizonica (the spider was identified by an entomologist), in whom shock and a typical cutaneous lesion developed. She did not experience renal insufficiency or disseminated intravascular coagulatio

[Latrodectism in southern France. A series of cases from the poisoning center of Marseille].

OBJECTIVES: Black widows, which belong to the genus Latrodectus, are particularly dangerous spiders. The clinical syndrome caused by their bite is known as latrodectism. In Provence and in Corsica, there is a locally-found species, Latrodectus mactans tredecimguttatus called "malmignate". A series of 30 cases were collected by the Marseille Poison centre between 1973 and 1993 inclusive. RESULTS: The cases of poisoning involved patients bitten in the summer, mostly in Corsica (only 5 of the cases were from Provence) in rural areas. The symptoms were generally widespread pain and muscle contrac

Loxosceles reclusa envenomation.

Loxosceles species are widely distributed. People are most at risk for bites when they disturb the nocturnal spider's habitat. The bite sequelae range from insignificant, to disseminated intravascular coagulation and renal failure. Patients most frequently seek care for slow-healing necrotic skin lesions. Early surgical excision is not recommended. Conservative local care usually suffices. Dapsone may benefit some patients. Specific antivenom given early may minimize sequelae.

Loxoscelism threatening pregnancy: five cases.

Envenomation by the spider Loxosceles reclusa in five pregnant women proved to have no sustained adverse effects on mother or baby when managed conservatively only with low-dose prednisone. A striking toxic erythema of the skin, common with the bite of the spider, caused the greatest discomfort and concern for the patients but proved to be entirely tractable. No episodes of hemolysis, disorders of coagulation, or renal damage were discovered. There is no clinical evidence of any special risk during pregnancy in the event of loxoscelism.

Black widow spider envenomation.

Black widow spiders are found throughout the continental United States and north into the southern Canadian provinces. Male black widow spiders are of little medical importance. Female black widow spiders can be 20 times larger than males. The female can be identified by the hourglass pattern, red or orange in color, on the ventral aspect of her shiny, globose black abdomen. Black widow spiders control the amount of venom they inject; an estimated 15% of bites to humans are non-envenomating. Cats are very sensitive to the venom and deaths are common. Dogs have severe clinical signs but are con
