To consider the effects of several years of competitive sports training on children and adolescents with spondylolisthesis, we carried out a retrospective radiologic and clinical study of 86 young athletes with spondylolysis or spondylolisthesis (24 girls and 62 boys between the ages of 6 and 20 years). The mean degree of displacement was 10.1% at the beginning of the observation. The radiologic tests showed an increase in displacement over time in 33 athletes. The average progression of spondylolisthesis in this group was 10.5%. For 36 athletes, spondylolisthesis did not progress during the p…

Sporting activity in wrestlers operated on for spondylolisthesis is not well documented in the literature. We present the case of a wrestler with a grade I L5-S1 spondylolisthesis who was L4-S1 arthrodesed due to pain not relieved by conservative methods. One year after surgery he had returned to full wrestling activity, including competitions. Two years later, he continues to wrestle at the same level of competition.

Classification of hangman's fracture according to Levine has been presented. The classification enables individualization of the management depending on the mechanism of the injury. A series of 37 patients with traumatic spondylolisthesis of the axis has been analyzed. Skull traction in flexion injury cases led to the increased dislocation.

STUDY DESIGN: The amount of intervertebral motion seen during dynamic radiography when imaged with the patient in the standing position was compared with that obtained with the patient in the lateral decubitus position.
SUMMARY OF BACKGROUND DATA: Regarding analysis of spondylolisthesis, whether flexion/extension radiographs should be obtained with the patient in the lateral decubitus or standing position has been anecdotal.
METHODS: Fifty consecutive adult patients with spondylolisthesis underwent flexion/extension lumbar spine radiographs in both positions. Abnormal motion was considered a…

The incidence of spondylolysis is approximately 5-6% in the white population. Males and females are equally affected. In about one third of the cases spondylolysis is associated with spondylolisthesis. For the etiology mechanical and genetic factors are important. At the L4 level mechanical factors play the dominant role, while at the L5 level genetic factors are equally important (size and orientation of the facet joints, dysplasia of the vertebral arch). Increased risk for low back pain is found in the presence of the following factors: spondylolisthesis exceeding 10 mm, a low lumbar index, …

Plain x-rays of the lumbar spine are not helpful in diagnosing most known causes of low back pain but can reveal structural changes such as disc space narrowing, scoliosis, lumbarization or sacralization, spondylolysis, and spondylolisthesis. Spondylolisthesis is defined as the forward displacement of one vertebrae upon another, usually at the L5-S1 level, but also occurring at L4-5. For spondylolisthesis to occur there must be an anatomical change in the structures which normally resist the anterior displacement of one vertebrae upon the other. Forward slippage is resisted by the bony block o…

In this series spondylolisthesis was treated operatively in children and adolescents who had a displacement greater than 50% even if they were asymptomatic. In youngsters with less than 50% displacement and in adults, operative stabilization was performed if the displacement was progressive or if there were persistent symptoms unresponsive to conservative treatment that interfered with the patient's normal activities. The operation consisted of a posterolateral fusion in situ. Usually a laminectomy was not performed unless there were signs of severe neural compression. No reduction of the spon…

The authors present their analysis of stability of the "scoliotic arch--Tarrington's distractor" system in various versions of surgical stabilization. A modification of Tarrington's technique is suggested which improves the initial fixation strength and reduces the risk of complications.

Fifty-two patients with traumatic spondylolisthesis of the axis were admitted to the University of Maryland Spinal Injury Center between 1977 and 1982. There were fifteen Type-I fractures, twenty-nine Type-II fractures, three Type-IIa fractures, and five Type-III fractures. Associated neurological deficits were found in only four patients, although unassociated neurological deficits such as closed head injury were seen in eleven patients. Thirteen patients had other fractures of the cervical spine. Type-I fractures were stable injuries and were treated with collar protection. Most Type-II inju…

Two groups of surgically treated patients with degenerative spondylolisthesis were compared. Those who had decompression accompanied by fusion had more favorable outcomes than those treated with decompression alone.

We performed a prospective roentgenographic study to determine the incidence of spondylolysis, spondylolisthesis, or both, in 500 unselected first-grade children from 1955 through 1957. The families of the children with spondylolysis were followed in a similar manner. The incidence of spondylolysis at the age of six years was 4.4 per cent and increased to 6 per cent in adulthood. The degree of spondylolisthesis was as much as 28 per cent, and progression of the olisthesis was unusual. The data support the hypothesis that the spondylolytic defect is the result of a defect in the cartilaginous a…
