Using a multi-state Learning Community as an implementation strategy for immediate postpartum long-acting reversible contraception.

BACKGROUND: Implementation strategies are imperative for the successful adoption and sustainability of complex evidence-based public health practices. Creating a learning collaborative is one strategy that was part of a recently published compilation of implementation strategy terms and definitions. In partnership with the Centers for Disease Control and Prevention and other partner agencies, the Association of State and Territorial Health Officials recently convened a multi-state Learning Community to support cross-state collaboration and provide technical assistance for improving state capac

Access to Fertility Preservation: A Policy Review on State-Level Mandates for Iatrogenic Infertility.

BACKGROUND: Fertility preservation (FP) services are a critical piece of family planning for patients with iatrogenic infertility, which is defined as infertility resulting from undergoing a gonadotoxic medical treatment or procedure. There is currently no national mandate for health care plans to cover FP services, and state mandates for coverage are quite heterogenous. METHODS: Between 2016 and 2024, 18 states have passed FP mandates that vary greatly in which diagnoses are covered, for how long the coverage lasts, and if there are additional measures impacting the coverage. Particularly as

Ragsdale v. Turnock, 10 March 1988, as amended on 13 April 1988.

The plaintiffs, physicians and women seeking abortions, challenged licensing regulations imposed by the state of Illinois on abortion clinics. The Court found the following requirements to place unconstitutional burdens on a woman's right to obtain an abortion: 1) that separate licenses be obtained for facilities devoted primarily to performing first trimester abortions; 2) that a physician who was to perform an abortion also perform a pregnancy test, even if such a test had already been performed by another physician; 3) that persons undergoing abortions be given counseling which includes a d

A mathematical model to estimate the state-specific impact of the Health Resources and Services Administration's Ryan White HIV/AIDS Program.

BACKGROUND: Access to and engagement in high-quality HIV medical care and treatment is essential for ending the HIV epidemic. The Health Resources and Services Administration's (HRSA) Ryan White HIV/AIDS Program (RWHAP) plays a critical role in ensuring that people living with diagnosed HIV (PLWH) are linked to and consistently engaged in high quality care and receive HIV medication in a timely manner. State variation in HIV prevalence, the proportion of PLWH served by the RWHAP, and local health care environments could influence the state-specific impact of the RWHAP. This analysis sought to

Preparedness for Zika Virus Disease - New York City, 2016.

The rapid spread of Zika virus across the World Health Organization's Region of the Americas has had a direct effect on the U.S. health care delivery system. Hospitals in New York City (NYC) have been implementing prevention and response efforts consistent with CDC guidance. As of September 21, 2016, a total of 715 cases of laboratory-confirmed Zika virus disease had been diagnosed in New York state among travelers who returned from affected areas, their sexual contacts, or infants infected in utero. This represents the highest number of reported cases in any state to date, and underscores the

Assessment of State, Local, and Territorial Zika Planning and Preparedness Activities - United States, June 2016-July 2017.

The emergency response to Zika virus disease required coordinated efforts and heightened collaboration among federal, state, local, and territorial public health jurisdictions. CDC activated its Emergency Operations Center on January 21, 2016, with seven task forces to support the national response. The State Coordination Task Force, which functions as a liaison between jurisdictions and federal operations during a response, coordinated the development of CDC Guidelines for Development of State and Local Risk-based Zika Action Plans, which included a Zika Preparedness Checklist (1). The checkl

Impact of a Medicaid primary care provider and preventive care on pediatric hospitalization.

OBJECTIVE: This study evaluates the impact that a Medicaid managed care program had on avoidable hospitalization, a form of health care misuse that we hypothesize can be reduced by improved access to and quality of primary care in the context of a managed care program. Ambulatory care sensitive (ACS) hospitalizations, a previously defined categorization of hospitalization, as well as all pediatric hospitalizations were also studied. INTERVENTION: The Maryland Access to Care (MAC) was a fee-for-service, gatekeeper, Medicaid managed care program with assigned primary medical providers and requi

Estimating undersupply of nursing home beds in states.

This examination of nursing home bed supply estimates undersupply in each of the states for the purpose of identifying the states with the greatest undersupply of beds. New data on state nursing home bed supply for the period 1979-1982 are used. The study employs selected independent variables in two different types of analyses to estimate bed supply for each state. Where a state is found to have a bed shortage, state public policymakers may wish to employ policies that differ from those suitable for states with an adequate supply of beds. Because of limitations in the data, issues of oversupp

The nationwide evaluation of fetal and infant mortality review (FIMR) programs: development and implementation of recommendations and conduct of essential maternal and child health services by FIMR programs.

OBJECTIVE: An evaluation of fetal and infant mortality review (FIMR) programs nationwide was conducted to characterize their unique role in improving the system of perinatal health care. The aim of this paper is to examine intermediate outcomes of the FIMR, in particular the development and implementation of recommendations produced by the FIMRs and the conduct of essential MCH services by the FIMRs. METHODS: We report on 74 FIMRs whose communities were selected for the nationwide evaluation and for whom we had data from the FIMR director or comparable respondent. We focus on the recommendati

Policy issues in mental health among the elderly.

Americans are living longer than ever before in history. With age comes an increased risk for chronic mental health disorders. About 1 in 8 baby boomers is expected to be diagnosed with Alzheimer disease, which will amount to some ten million members of this age cohort. The prevalence of mental health disorders among the elderly is often unrecognized. One in four older adults lives with depression, anxiety disorders, or other significant psychiatric disorders. Mental health disorders are frequently comorbid in older adults, occurring with a number of common chronic illnesses such as in diabete

In sickness and in health insurance: Gender in household benefit plan choice.

I document that households in the United States are more likely to choose health plans offered by the husband's employer than the wife's. This gap may reflect gender disparities in employment status, plan quality, or an inherent gender effect. Using a novel identification strategy that holds employment status and plan quality fixed, I find that the gender effect increases the probability of selecting the husband's plan by 6.3 percentage points.

Access to Contraception in the Context of Zika: Health System Challenges and Responses.

Women in areas of the Americas with endemic Aedes mosquito populations are at risk for exposure to Zika virus, which can cause fetal brain abnormalities and associated congenital microcephaly. Individual health care providers may encounter health system barriers to providing evidence-based care. We focus on Mexico and the state of Texas to highlight the role of health system factors in contraceptive access in the context of Zika and highlight efforts in Puerto Rico as an example of initiatives to improve access to contraception. In Mexico, states with the highest unmet need for contraception a

Efforts to Improve Perinatal Outcomes for Women Enrolled in Medicaid.

Improving women's health and perinatal health outcomes is a high priority for Medicaid, the jointly financed federal-state health coverage program. The authorities provided by the Affordable Care Act give Medicaid new resources and opportunities to improve coverage and perinatal care. Given that the Medicaid program currently covers almost half of all births in the United States, the Centers for Medicare and Medicaid Services, working in partnership with states and other stakeholders, is using new and existing authorities to improve birth outcomes. Quality measurement, quality-improvement proj

School-based clinics: their role in helping students meet the 1990 objectives.

Service statistics and observations from site visits across the country indicate that school-based clinics (SBCs) may be having an impact on several of the problems targeted in the 1990 health objectives, including unplanned pregnancy and substance abuse. At least 120 junior and senior high schools in 61 communities are currently operating or developing clinics. Growth is attributed to increasing concern about high-risk youth, especially among educators in their roles of "surrogate parents"; to disillusion with categorical interventions and a movement toward more comprehensive services; and to

Community Health Centers' Practices, Barriers, and Facilitators for Providing a 1-Year Supply of Oral Contraception on Site.

INTRODUCTION: High-quality family planning services help women to achieve their preferred family size and birth spacing, which in turn leads to improved health outcomes and better quality of life. This study investigates whether women have access to a 1-year supply of oral contraceptives (OCs) on site when they receive care at community health centers and whether states require coverage for a 1-year supply. METHODS: This study used a concurrent, mixed-methods approach, with a single phase of quantitative research (survey of health centers) and two phases of qualitative research (50-state poli
