Delayed primary versus late secondary wound closure in the treatment of postsurgical sternum osteomyelitis.

Sternal osteomyelitis and poststernotomy mediastinitis is a severe and life-threatening complication after the cardiac surgery. The incidenc … bidity. In light of this we propose a randomized study comparing new delayed primary closure of the sternum to the secondary vacuum-assisted closure.

Pectus excavatum surgically repaired using sternum realignment and splint techniques in a young cat.

Successful surgical repair of pectus excavatum deformities has so far only been reported in very young animals. This case report describes an alternative technique for repairing pectus excavatum in a young cat. The cat had shown moderate deformity with slight respiratory impairment when young and had experienced clinical problems with age. A sternum realignment technique involving a pin associated with an external splint was used. This alternative technique was a safe and efficient procedure in the five-month-old cat.

Life-threatening hemorrhage during removal of a Nuss bar embedded in the sternum: a case report.

BACKGROUND: Plate erosion of the sternum leading to hemorrhage during NUSS bar removal procedure for pectus excavatum is a very rare complication, and the few cases reported worldwide are currently only briefly described in the statistical results. CASE PRESENTATION: A 22-year-old female patient underwent a surgical repair for pectus excavatum(Nuss procedure)2.5 years ago. During the removal of the bars in our center, we found that there was great resistance when removing the upper Nuss bar, and a rapid flow of bright red hemorrhage flowed from both incisions immediately after removal. A long…

Pediatric thoracic cage fractures-Mind the sternum: A retrospective analysis of the ACS-TQIP database.

BACKGROUND: The thoracic cage is an anatomical entity formed by the thoracic spine, ribs, and sternum. As part of this osteoligamentous complex, the sternum contributes substantially to the stability of the thoracic spine. This study investigates the influence of a concomitant sternal fracture (SF) on the treatment and hospital course of pediatric patients with a thoracic vertebral fracture (TVF). METHODS: The Trauma Quality Improvement Program data sets from 2016 to 2020 were reviewed. Patients aged 0 year to 19 years with TVF with or without SF following blunt trauma were identified using t…

Sharp cardiac trauma through the sternum caused by an automatic nail gun: A case report.

Automatic nail gun injuries to the hand commonly occur with the use of these machines in construction. However, such injuries to the cardiothoracic area are atypical. Herein, we report a case of emergency surgery to remove a nail, which was accidentally shot through the sternum and reached the heart. A 24-year-old man was working in a narrow space at a construction site, where he tripped over the air hose of an automatic pneumatic nail gun. The trigger was accidentally pulled, while the machine was facing his direction, and a nail entered his sternum. The patient felt chest pain, walked to a n…

Preliminary evaluation of the Sternum Screw: a novel method for improved sternal closure to prevent dehiscence.

Sternal dehiscence is a relatively rare but serious complication of sternal closure with an unacceptable mortality and morbidity rate. The 6 mm cannulated Sternum Screws are a novel approach that aims to prevent dehiscence. The screws are placed on either side of the sternotomy and wire threaded through the cannula and closed customarily. The Sternum Screws make the bone non-penetrable at the same time retaining the familiarity of conventional stainless steel wire. This novel technique was tested in sheep sterna to compare its efficacy to conventional wire closure. Using tensile testing in a t…

[Chronological study for solitary bone metastasis in the sternum from breast cancer with bone scintigraphy].

Since breast cancer is frequently associated with bone metastasis, bone scintigraphies have been performed to determine pre-operative staging and to survey postoperative bone metastasis. The sternum, in particular, is a site at which is difficult to differentiate between benign bone disease and bone metastasis, because of varied uptake and wide individual variations. In this study, chronological bone images were scintigraphied in six cases with solitary sternal metastasis and three cases with benign bone disease including two fracture cases and one arthritis case. On bone scintigrams in which …

Prenatal ultrasonographic findings associated with wide clefting of the upper two thirds of the fetal sternum.

Complete or partial absence or clefting of the fetal sternum is uncommon. A teenage primiparous patient at 34 weeks' gestation with severe preeclampsia was delivered of an infant with wide clefting of the upper two thirds of the sternum. The infant had no respiratory or cardiac symptoms. There were no other congenital abnormalities. Corrective neonatal surgery was performed at 1 month of age. Retrospective analysis of the ultrasonographic examination performed at 34 weeks revealed evidence of this fetal sternal defect.

Primary osteogenic sarcoma of the sternum with intravascular invasion and rapid progression: a rare case report.

INTRODUCTION AND IMPORTANCE: Primary osteosarcoma of the sternum is a rare malignant bone tumor. Only a few cases have been documented, and most have been managed with chemotherapy and surgical intervention. Our case is distinctive due to the vascular invasion of the left brachiocephalic vein and lack of response to chemotherapy. Reporting this case contributes to the limited literature and highlights the diagnostic and therapeutic challenges associated with the management of primary sternal osteosarcoma. CASE PRESENTATION: We describe a 37-year-old woman with a painful, progressively enlargi…

A 12-Year-Old Child With a Sunken Sternum and Progressive Muscle Weakness: A Case Report.

A 12-year-old girl with facioscapulohumeral muscular dystrophy and severe pectus excavatum presented with progressive dyspnea and restrictive ventilatory impairment. She underwent successful Nuss repair with uneventful recovery. At 6-month follow-up, chest contour and pulmonary function improved, suggesting that surgical correction is feasible and beneficial in FSHD when mechanical restriction predominates.

Comparison of the bone marrow composition in samples from rib, ilium, sternum, and humerus of dogs.

Bone marrow (BM) samples in dogs are typically acquired from hematopoietically active trabecular BM at appendicular skeletal sites, such as the proximal humerus and femur, or axial skeletal sites, such as pelvis, sternum, and rib. Suitability for in vivo biopsy varies by site, but equivalency of BM composition at different sites is uncertain. We sought to determine whether the composition of BM is comparable in humerus, ilium, rib, and sternum. Samples from each site were obtained within 24 h after death from 30 dogs with spontaneous disease submitted for postmortem examination. Tissues were f…

Reentry to the Mediastinum When the Ascending Aorta Is Adherent to the Sternum: A Two-Stage Sternotomy Approach.

Reentry to the mediastinum when the ascending aorta aneurysm is adherent to the sternum is characterized by high risk of aneurysm rupture during sternum opening. In such cases, often cardiopulmonary bypass via peripheral vessels is instituted, and reentry done in deep hypothermia and circulatory arrest. To reduce both risks of aneurysm rupture during resternotomy and those related to prolonged cardiopulmonary bypass time, we present a surgical approach consisting of a two-stage sternotomy to avoid the risky zone and extra-anatomic epiaortic vessels anastomoses.

Statistical analysis of biomechanical properties and size of the sternum and its fracture risk in a Japanese sample.

OBJECTIVE: Sternal fractures can have life-threatening complications. To understand chest injury mechanisms, sufficient data regarding the mechanical properties and structure of the sternum are required. The aim of this study was to examine the mechanical properties and size of the sternum in a Japanese forensic sample. MATERIALS AND METHODS: Sterna were obtained from 120 Japanese dead bodies of known age and sex. The sample thickness (ST) and the sample width (SW) were measured using a computed tomographic image. Three-point-bending tests were conducted using a three-point-bending apparatus …

A uniport subxiphoid approach with a modified sternum retractor is safe and feasible for anterior mediastinal tumors.

BACKGROUND: Minimally invasive surgery has been used to treat anterior mediastinal tumors. This study sought to describe a single team's experience of uniport subxiphoid mediastinal surgery using a modified sternum retractor. METHODS: Patients who underwent uniport subxiphoid video-assisted thoracoscopic surgery (USVATS) or unilateral video-assisted thoracoscopic surgery (LVATS) from September 2018 to December 2021 were retrospectively enrolled in this study. A 5-cm vertical incision approximately 1-cm caudal to the xiphoid process was usually made followed by the installment of a modified re…
