Combined use of cocaine and alcohol: A violent cocktail? A systematic review.

It is generally believed that the use of alcohol and cocaine alone and especially in combination elicits aggression and violent behaviour. Though there is overwhelming proof that heavy alcohol use is associated with violence, this is not the case for cocaine. Still, in the popular press and by spokesmen of the police, cocaine use is seen as a cause of violent incidents. In the current systematic review, available data from human studies on the relation between cocaine and violent behaviour is presented. In particular, we present scientific data on the acute induction of violence by cocaine alo

A preliminary investigation of schedule parameters on cocaine abstinence in contingency management.

Contingency management (CM) interventions are the most effective psychosocial interventions for substance use disorders. However, further investigation is needed to create the most robust intervention possible. This study investigated the effects of 1) reinforcer magnitude; and 2) fixed and escalating and resetting incentives on cocaine abstinence in an outpatient trial. In this analysis, 34 treatment-seeking individuals with Cocaine Use Disorder received either high or low value incentives for providing a benzoylecgonine-negative urine sample or were in a control condition and received incent

Identification of Typologies of Cocaine Use Based on Quantity, Frequency, and Duration of Use: A Latent Profile Analysis.

OBJECTIVES: We identified the patterns of cocaine use during individuals' heaviest use period by considering quantity, frequency, and duration simultaneously and examined the correlates and risk profile for these patterns. METHODS: Latent profile analysis was conducted among the 3117 individuals who reported lifetime use of cocaine in the National Epidemiologic Survey on Alcohol and Related Conditions-III (NESARC-III) with quantity, frequency, and duration used as indicators. Logistic regression analyses examined factors associated with subtype membership based on patterns of cocaine use and

Anxiety disorders and the use and abuse of drugs.

Substance abuse continues to be a growing problem--both in the United States and worldwide. Moderate alcohol consumption reduces anxiety, but alcohol abuse actually increases anxiety and impairs judgment and performance. Research data indicate that phobic anxiety is more common than general anxiety as a prelude to alcohol dependence. Benzodiazepines are widely used throughout the world in the treatment of anxiety disorders. Patients who take benzodiazepines for clinical anxiety rarely exceed the prescribed dosage: benzodiazepine abuse is more likely to occur in those who abuse other drugs. For

Introduction to behavioral addictions.

BACKGROUND: Several behaviors, besides psychoactive substance ingestion, produce short-term reward that may engender persistent behavior, despite knowledge of adverse consequences, i.e., diminished control over the behavior. These disorders have historically been conceptualized in several ways. One view posits these disorders as lying along an impulsive-compulsive spectrum, with some classified as impulse control disorders. An alternate, but not mutually exclusive, conceptualization considers the disorders as non-substance or "behavioral" addictions. OBJECTIVES: Inform the discussion on the r

Three-year recovery outcomes for long-term patients with co-occurring schizophrenic and substance use disorders.

Little is known about the expected treatment outcomes of patients with co-occurring schizophrenic and substance use disorders. This paper reports 3-year outcomes for 152 patients with schizophrenia or schizoaffective disorder and substance use disorders, all of whom received integrated dual disorders treatments in the New Hampshire Dual Diagnosis Study. Outcomes are defined as positive coping behaviors identified by consumers as indicators of recovery. Participants improved steadily in terms of controlling symptoms of schizophrenia, actively attaining remissions from substance abuse, increasin

The utilization and delivery of safer smoking practices and services: a narrative synthesis of the literature.

BACKGROUND: Providing sterile drug smoking materials to people who use drugs can prevent the acquisition of infectious diseases and reduce overdose risk. However, there is a lack of understanding of how these practices are being implemented and received by people who use drugs globally. METHODS: A systematic review of safer smoking practices was conducted by searching PubMed, PsycInfo, Embase for relevant peer-reviewed, English-language publications from inception or the availability of online manuscripts through December 2022. RESULTS: Overall, 32 peer-reviewed papers from six countries wer

Gambling and substance use disorders: current status and future directions.

This paper briefly reviews the clinical presentation of pathological gambling and presents data related to the co-occurrence of pathological gambling and substance use disorders. Both epidemiological and treatment data are reviewed. The putative physiological substrates underlying pathological gambling are described, along with the limited genetics data. Types of treatments applied to gamblers are described, including data on efficacy and overlap of issues that relate to both gambling and substance use disorders. Finally, areas for future research are suggested to advance our understanding of

Eating disorders and substance use at a South African tertiary hospital over a 21-year period.

BACKGROUND: Eating disorders (EDs) and substance-related disorders pose a challenge when they co-occur and have implications for patient management. Clinical information on EDs and substance-related disorders as independent disorders is fairly well established in South Africa, but our understanding of the coexistence of these disorders is limited. AIM: To determine the prevalence, the concurrent nature and the possible trends of substance use among patients diagnosed with EDs at a South African tertiary hospital over a 21-year period. SETTING: The ED unit at Tygerberg Hospital, Cape Town, So…

Prevention and screening, brief intervention, and referral to treatment for substance use in primary care.

Substance use and related disorders are among the leading causes of preventable injury and illness, chronic health conditions, medical complications, disability, increased suffering, and premature death. Primary care clinicians can help patients avoid, reduce, or eliminate high-risk behaviors and negative consequences associated with substance use by integrating prevention and screening, brief intervention, and referral to treatment into their clinical practices. This article provides the necessary information, evidence-based recommendations, and readily available resources to help address sub

Psychiatric Risk in Unstably Housed Sexual Minority Women: Relationship between Sexual and Racial Minority Status and Human Immunodeficiency Virus and Psychiatric Diagnoses.

BACKGROUND: Stress associated with minority statuses has been linked to mental health disorders. However, research conducted exclusively among impoverished women, a population known to be at risk for poor health owing to overlapping risks, is sparse. We sought to determine if homeless and unstably housed sexual minority (i.e., nonheterosexual) women were at greater psychiatric risk than their heterosexual counterparts. We also sought to determine if racial/ethnic minority and human immunodeficiency virus status contributed to psychiatric risks. METHODS: Homeless/unstably housed women living i

Substances of abuse and their effect on SAR-CoV-2 pathogenesis.

Following the emergence of SARS-CoV-2, various reports suggest that there has been a significant increase in substance abuse due to social distancing and related issues. Several reports have suggested the impact of chronic substance use on individuals' physiological and psychological health. Therefore, there is a need to know the impact of SARS-CoV-2 on persons with substance use disorders. Individuals with substance use disorders are the most vulnerable groups and are at a high risk of SARS-CoV-2 infection due to their already existing health issues associated with substance use. This review

Positive screening tests for barbiturates in urine samples in the York area over a 1-year period.

Following the screening of urine samples for a panel of drugs over a period of 1 year, a peak was observed during December 1999 and January 2000 in the numbers of samples testing positive for both morphine and barbiturates. This increase may be due to the adulteration of street heroin with barbiturate during this period.

[Hyperbaric oxygenation in the treatment of patients with drug addiction, narcotic addiction and alcoholism in the post-intoxication and abstinence periods].

Hyperbaric oxygenation was used in the treatment of 340 patients with narcomania, toxicomania, and alcoholism in the post-intoxication and abstinence periods; 223 of these were alcoholics, 68 toxicomaniacs, and 49 opium narcomaniacs. A group of 185 patients administered drug therapy alone were controls. Exposure to hyperbaric oxygenation had a favorable effect on the patient's status during sessions and persisted for some time after them. Patients with different premorbid symptoms and initial status experienced tranquilizing or bioenergizing effects of hyperbaric oxygen. A comparative clinical

Buprenorphine: beyond methadone?

This month's guest authors are affiliated with the substance abuse treatment and treatment research unit of the Connecticut Mental Health Center and the department of psychiatry at Yale University School of Medicine, where Dr. Rosen is instructor and Dr. Kosten is associate professor. They discuss a promising new treatment that may reduce abuse of cocaine by drug addicts who use intravenous heroin and may indirectly reduce risk-taking behavior associated with transmission of the human immunodeficiency virus.
