Type 4 Acromioclavicular Joint Separation Reconstruction With Suspensory Fixation, Allograft, and Suture Augmentation.

BACKGROUND: This is a case of surgical treatment of a type 4 acromioclavicular (AC) joint separation via an arthroscopically assisted approach and reconstruction with combined suspensory fixation, allograft ligament reconstruction, and suture augmentation. AC joint injuries are relatively common, representing approximately 9% of shoulder injuries and approximately 50% of sport-related shoulder injuries. Most AC joint injuries do well when treated conservatively. However, the literature supports surgical treatment for high-grade injuries, failed conservative treatment, and open injuries or thos

Surgical Outcomes Following Fixation of Peri-Implant Distal Radius Fractures: A Case Series.

PURPOSE: The purpose of this study was to evaluate surgical outcomes following fixation of peri-implant distal radius fractures. METHODS: A retrospective chart review was conducted of peri-implant distal radius fractures treated surgically at a large academic practice over 18 years. Patients were included if they had previously undergone open reduction and internal fixation of a distal radius fracture; subsequently sustained a fracture at, or adjacent to, the existing hardware; and then undergone revision fixation with the removal of hardware. Fractures were categorized into 3 groups: type A

Biomechanical comparison of different internal fixation devices for transversely unstable Mason type II radial head fractures.

Background: The integrity of the radial head is critical to maintaining elbow joint stability. For radial head fractures requiring surgical treatment, headless compression cannulated screw fixation is a less invasive scheme that has fewer complications. The aim of this study was to compare the mechanical stability of different fixation devices, including headless compression cannulated screws and mini-T-plates, for the fixation of transversely unstable radial head fractures. Methods: Forty identical synthetic radius bones were used to construct transverse unstable radial head fracture models.

Biomechanical optimization of different fixation modes for a proximal femoral L-osteotomy.

BACKGROUND: Numerous proposed surgical techniques have had minimal success in managing greater trochanter overgrowth secondary to retarded growth of the femoral capital epiphysis. For reconstruction of residual hip deformities, a novel type of proximal femur L-osteotomy was performed with satisfactory results. Although the clinical outcome was good, the biomechanical characteristics of the femur after such an osteotomy have not been clearly elucidated. Therefore, this study presents a three dimensional finite element analysis designed to understand the mechanical characteristics of the femur a

Initial results with minimally invasive repair of pectus carinatum.

OBJECTIVE: Pectus carinatum is traditionally repaired by using some modification of the open Ravitch procedure, with its possible long-term sequelae, such as poor postoperative compliance of the chest. In this study we assessed our results with a new minimally invasive repair of pectus carinatum that requires neither cartilage incision nor sternotomy. METHODS: From June 2005, we have corrected pectus carinatum using a method analogous to the Nuss procedure for pectus excavatum repair. Thus far, we performed this intervention on 14 patients (mean age, 15 +/- 1.5 years). A steel bar has been in

Cervical corpectomy and strut grafting.

Cervical corpectomy and strut grafting is a deceptively simple procedure that has been performed for many years for a variety of cervical spine disorders (infection, neoplastic disease, and trauma) but most commonly for cervical spondylosis. The procedure requires attention to detail to ensure adequate decompression of the neural structures and avoiding injury to the soft tissues of the neck and the vertebral artery in the transverse foramina. The following description of the technique is one we have successfully used for cervical corpectomy and strut grafting. We also discuss patient selectio

Univariate and multivariate analysis of the styrofoam fixation device on patient setup errors in radiotherapy.

BACKGROUND: Styrofoam is a patient-specific immobilization device in radiotherapy; most previous studies about the impact of styrofoam on setup errors have only analyzed a single tumor type, and have not considered the influence of patient's physical condition on the setup errors of styrofoam. PURPOSE: This study aims to evaluate the impact of styrofoam device on setup errors in radiotherapy and explore which patient population is more suitable for styrofoam immobilization. METHODS: Univariate and multivariate analyses were conducted to compare the setup errors between the experimental group

Successful Surgical Repair of Acute Quadriceps Tendon Rupture Utilizing Cortical Button Fixation: A Case Report.

BACKGROUND Timely diagnosis and surgical treatment are often needed to restore function of the extensor mechanism after rupture of the quadriceps tendon. Several techniques for quadriceps tendon repair have been reported, including suture anchors and bone tunnels. Cortical button fixation, or the use of an adjustable cortical fixation device, is a local and biomechanically strong internal brace technique used to treat ligament and tendon injuries. This report is of a 69-year-old man who experienced a quadriceps tendon rupture while golfing and underwent a successful surgical repair using corti

Injury to Anatomic Structures Due to Fixation Device in Management of Mandibular Fracture.

INTRODUCTION: ORIF is the gold standard for the management of mandibular fractures. The chances of postoperative complications are due to infection, damage of the inferior alveolar nerve canal, or tooth root during the fixation of the fracture fragments. Fixation hardware placed in the vicinity of the vital structures may encroach on the anatomic structures, leading to postoperative complications. AIM: The aim of this study is to evaluate the injury to the anatomic structures in the proximity of the fracture line due to the fixation device. MATERIAL AND METHODS: This retrospective study was

Comparative Assessment of Functional Outcomes of Surgical Treatment in Patients with Distal Radial Fractures.

BACKGROUND: Unstable and comminuted distal radial fractures require surgical treatment by percutaneous insertion of Kir-schner wires, open reduction and fixation with a non-locking or locking plate or with an external device. The aim of this paper was to try to answer the following question: are there differences in functional treatment outcomes in patients after surgery with the use of Kirschner wires vs LCP plate fixation? MATERIAL AND METHODS: The study group included 100 patients after surgical treatment by closed reduction and simple fixation with Kirschner wires (50 patients) and by ope

A reduction-fixation system for unstable thoracolumbar burst fractures.

Thirty-three patients with unstable burst fractures of the lower thoracic and lumbar spine were treated with a reduction-fixation system. The new system is used both as a reduction and a fixation device for disorders of the lower thoracic and lumbar spine. In treatment of spinal fractures, it provides symmetric lordotic distraction to obtain the best possible reduction of intracanal fragments, and rigidly stabilizes the fractured vertebra while involving the minimum number of segments. All patients had a minimum follow-up of 24 months. Most patients in this series had a near-anatomic reduction

Urological surgery in epidermolysis bullosa: tactical planning for surgery and anesthesia.

Epidermolysis bullosa (EB) is characterized by extreme fragility of the skin and mucosae. Anesthetic and surgical techniques have to be adapted to those children and routine practice may not be adequate. Urological problems are relatively common, but surgical techniques adapted to those children have not been well debated and only low evidence is available to this moment. Herein we discuss the specifics of anesthetic and surgical techniques chosen to treat a six year old EB male presenting with symptomatic phimosis.

Cervical spinal cord neurapraxia in the setting of Klippel-Feil anomaly: a diagnostic and therapeutic challenge.

STUDY DESIGN: Case report. SETTING: Temple University Children's Medical Center in USA. OBJECTIVES: To report a patient whose recurrent and transient episodes of quadriplegia mimicked cervical cord neurapraxia. Cervical spine neuroimaging revealed congenital intersegmental fusion of C5 through C7 (consistent with Klippel-Feil anomaly), corresponding abnormal spinal cord signals on T2-weighted images and enhancing focal lesion opposite the C4 vertebral body. A posterior cervical decompression at C4-C5 and lateral mass fixation was performed, and the episodic quadriplegia has not recurred. CO

Hip Arthroplasty After Surgical Treatment of Intertrochanteric Hip Fractures.

BACKGROUND: Although use of intramedullary hip screws (IMHS) for intertrochanteric (IT) hip fractures has become more common, limited data have suggested difficulties in conversion to hip arthroplasty. The present study investigates whether conversion of failed IT fracture fixation with an intramedullary vs extramedullary device leads to different rates or types of complications or decreased arthroplasty survivorship. METHODS: One hundred eleven patients were converted to hip arthroplasty after previous surgical treatment of an IT fracture from 2000 to 2010. Seventy hips had been treated with

Outcome of intramedullary nailing treatment for intertrochanteric femoral fractures.

INTRODUCTION: The OTA/AO type 31 A3 intertrochanteric fracture has a transverse or reverse oblique fracture at the lesser trochanteric level, which accentuates the varus compressive stress in the region of the fracture and the implant. Intramedullary fixation using different types of nails is commonly preferred. The purpose of this study is to evaluate intertrochanteric femoral fractures with intramedullary nail treatment in regard to surgical procedure, complications, and clinical outcomes. METHODS: From one level 1 trauma center, 216 consecutive adult intertrochanteric femoral fractures (OT

Design and Validation of a Novel Orthopedic Device for Enhanced Fracture Stabilization and Reduction Efficiency: A Randomized Controlled Trial With Implications for Surgical Outcomes.

OBJECTIVE: Traumatic long bone fractures require precise reduction and stable fixation to achieve optimal outcomes during open reduction and internal fixation (ORIF). Conventional bone-holding instruments are often associated with prolonged operative time, increased blood loss, reliance on surgical assistance, and higher complication rates. This study aimed to evaluate the clinical effectiveness of a newly designed orthopedic repositioning device in reducing operative time, intraoperative blood loss, postoperative pain, and complications compared with conventional ORIF techniques. METHODS: Th

Minimally invasive laser vibrometry (MIVIB) with a floating mass transducer - A new method for objective evaluation of the middle ear demonstrated on stapes fixation.

Ossicular fixation through otosclerosis, chronic otitis media and other pathologies, especially tympanosclerosis, are treated by surgery if hearing aids fail as an alternative. However, the best hearing outcome is often based on knowledge of the degree and location of the fixation. Objective methods to quantify the degree and position of the fixation are largely lacking. Laser vibrometry is a known method to detect ossicular fixation but clinical applicability remains limited. A new method, minimally invasive laser vibrometry (MIVIB), is presented to quantify ossicle mobility using laser vibro
