The HCL-32: towards a self-assessment tool for hypomanic symptoms in outpatients.

BACKGROUND: Bipolar disorders (BP) are frequently diagnosed and treated as pure depression initially; accurate diagnosis often being delayed by 8 to 10 years. In prospective studies, the presence of hypomanic symptoms in adolescence is strongly predictive of later bipolar disorders. As such, an instrument for self-assessment of hypomanic symptoms might increase the detection of suspected and of manifest, but under-treated, cases of bipolar disorders. METHODS: The multi-lingual hypomania checklist (HCL-32) has been developed and is being tested internationally. This preliminary paper reports t

Further assessments of the relationship between jet lag and some of its symptoms.

The disruption of circadian rhythms following time-zone transitions gives rise to the syndrome of jet lag. The power of some of the symptoms of jet lag to predict the amount of jet lag measured at the same and at different times of the day has been investigated. Eleven healthy subjects were studied in an Isolation Unit for two days after a simulated flight from the UK to Beijing (8 time zones to the east). At six time-points (08:30, 11:00, 14:00, 17:00, 20:00, and 23:00 h), the subjects recorded their jet lag, and the differences from "normal" (that is, from days in which there is no jet lag)

Assessing the community prevalence of bipolar spectrum symptoms by the mood disorder questionnaire.

BACKGROUND: The goal of this study was to assess the frequency and spectrum of symptoms related to bipolar disorders in a community sample by means of a self-rating questionnaire. METHOD: The Mood Disorder Questionnaire, a self-rating scale which evaluates DSM-IV manic/hypomanic symptoms, was administered to a community sample of 1,034 individuals recruited through advertisements. Symptom interrelationships were studied by factor analysis. RESULTS: 275 individuals (26.6%) reported moderate or severe impairment due to the symptoms. Manic/hypomanic symptoms were present in a high proportion of

Assessment of symptoms and exercise capacity in cyanotic patients with congenital heart disease.

OBJECTIVES: Patients with cyanotic congenital heart disease are generally thought to be limited by hypoxemia. To correlate exercise tolerance to the severity of the cardiac abnormality and to further characterize dyspnea in affected patients, we examined 25 adults with uncorrected cyanotic congenital heart disease. DESIGN AND SETTING: Cohort study at a university hospital. METHODS: Symptom-limited cardiopulmonary exercise testing (CPX) was performed on a treadmill. Expiratory gas was analyzed breath by breath for evaluation of maximal exercise performance, ventilation, and ventilatory effici

Multiple assessments of depressive symptoms as an index of depression in population-based samples.

The incremental validity of repeated measurements of a short depression questionnaire was examined regarding the clinical diagnosis of depression. Participants were 951 randomly selected women of around menopausal age. They completed the Edinburgh Depression Scale (EDS) at two time points, with approximately 18 months in between. At the second time point, they participated in a structured clinical interview for depression diagnosis based on the Research Diagnostic Criteria (RDC). With repeated assessments, specificity and negative predictive value (NPV) did not change much relative to a single

Skeletal scintigraphy in the multimodality assessment of young children with acute skeletal symptoms.

The authors review the scintigraphic manifestations of acute osteomyelitis, septic arthritis, transient synovitis, Legg-Calvé-Perthes disease, fractures of toddlers, and some systemic conditions that cause skeletal symptoms during childhood. They offer suggestions regarding incorporation of skeletal scintigraphy into a multimodality approach for assessing children with skeletal symptoms.

Questionnaire for the Assessment of Symptoms and Psychological Effects in Cardiovascular Therapy (the ASPECT Scale).

This brief review summarizes the background and principles for the development of a new psychometric instrument (Assessment of Symptoms and Psychological Effects in Cardiovascular Therapy--ASPECT Scale). The test consists of two parts, one for assessment of three aspects of global well-being (hedonic tone, relaxation, and activity) and one for evaluation of 34 commonly experienced symptomatic side-effects of cardiovascular drugs. All items are assessed by visual-analogue scales, and six clusters of symptoms have been identified: cardiac symptoms, central nervous system or cognitive symptoms, f

Assessment of long COVID symptom burden in patients testing positive for SARS-CoV-2 at a nationwide retail pharmacy.

BACKGROUND: Numerous grouping and scoring methodologies have been proposed to assess long COVID symptomatology. One approach is to use symptom count as a simple, quantifiable measure of long COVID symptom burden. METHODS: This was a secondary analysis of a nationwide patient-reported outcomes (PRO) study that recruited symptomatic adults testing positive for SARS-CoV-2 at a Retail Pharmacy in the spring of 2023. EQ-5D-5LTM, work productivity and impairment (WPAI), the Patient-Reported Outcomes Measurement Information System (PROMIS®) fatigue, and a long COVID symptom questionnaire were admini

[Concepts and assessment methods in lower urinary tract symptoms: From symptom score to Patient-Reported Outcomes and Outcomes (PRO)].

INTRODUCTION: The evaluation of pelvic perineal treatments has changed significantly in recent years. Initially focused on the assessment of symptoms, quality of life or patient satisfaction, it has gradually turned to new concepts, such as Patient Reported Outcome (PRO) and Goal Attainment (GA). OBJECTIVE: To describe the different concepts and methods of assessment available, in recent years, in the context of urinary functional pathologies such as bladder overactivity or urinary incontinence. METHODS: We conducted a non-systematic literature review to identify the main questionnaires and

Efficacy of neuroradiological imaging, neurological examination, and symptom status in follow-up assessment of patients with high-grade gliomas.

OBJECT: It is standard practice for the oncological follow-up of patients with brain tumors (especially in the setting of clinical trials) to include neurological examination and neuroradiological studies such as computerized tomography (CT) or magnetic resonance (MR) imaging in addition to evaluation of the patients' symptomatology and performance score. The validity of this practice and its impact on the welfare of patients with high-grade gliomas has not been adequately assessed. The purpose of this study is to provide such an assessment. METHODS: The authors studied 231 similarly treated
