Tendoscopy of Achilles, peroneal and tibialis posterior tendons: An evidence-based update.

BACKGROUND: Tendoscopy has been proposed in treating several conditions affecting tendons around the ankle. We reviewed literature to investigate the efficacy of Achilles, peroneal and tibialis posterior tendoscopy. METHODS: Following the PRISMA checklist, the Medline, Scopus and EMBASE databases were searched, including studies reporting patients affected by disorders of Achilles, peroneal and tibialis posterior tendons and treated by tendoscopy (or tendoscopic-assisted procedures). The tendoscopic technique, rehabilitation protocol, clinical scores, patient satisfaction, success and failure…

Static tensioning promotes hamstring tendons force relaxation more reliably than cycling tensioning.

BACKGROUND: Graft elongation might be a major reason for increased anterior laxity after anterior cruciate ligament (ACL) reconstruction. This study analyzed the force relaxation values and their stabilization when single strands of the gracilis and semitendinosus tendons underwent cyclic and static tensioning at 2.5% strain level, and compared the efficiency of static and cyclic tensioning in promoting force relaxation. METHODS: Eighteen gracilis tendons and 18 semitendinosus tendons from nine male cadavers (mean age: 22.44years) were subjected to 10 in vitro cyclic loads at 2.5% strain leve…

Danger zones for flexor tendons in volar plating of distal radius fractures.

PURPOSE: To define a danger zone for volar plates using magnetic resonance imaging by analyzing the position of the flexor tendons at risk around the watershed line. METHODS: We analyzed 40 wrist magnetic resonance images. The location of the flexor pollicus longus (FPL) and index flexor digitorum profundus (FDPi) tendons was recorded at 3 and 6 mm proximal to the watershed line of the distal radius. We measured the distance between the volar margin of the distal radius and the FPL and FDPi tendons, and the coronal position of the tendons. RESULTS: At a point 3 mm proximal to the watershed l…

Damage-induced hydrolyses modelling of biodegradable polymers for tendons and ligaments repair.

The use of biodegradable synthetic grafts to repair injured ligaments may overcome the disadvantages of other solutions. Apart from biological compatibility, these devices shall also be functionally compatible and temporarily displayed, during the healing process, adequate mechanical support. Laxity of these devices is an important concern. This can cause failure since it may result in joint instability. Laxity results from a progressive accumulation of plastic strain during the cyclic loading. The functional compatibility of a biodegradable synthetic graft and, therefore, the global mechanica

The effects of aging on quantitative sonographic features of rotator cuff tendons.

PURPOSE: Grayscale analysis is a practical, objective, and easy way to quantify echogenicity during ultrasonography. The purpose of the current study was to measure the changes in thickness and echogenicity that result from aging of the rotator cuff and long head of the biceps tendons. METHODS: The study comprised 45 volunteers, aged between 20 and 84 years and without history of shoulder pain. Participants were divided into three groups: young, middle-aged, and old. All subjects underwent standard ultrasonography of both shoulders. Tendon thickness and tear were recorded, and images in both …

Ultrasonographic evaluation of Achilles tendons in clubfeet before and after percutaneous tenotomy.

PURPOSE: To measure the size of the Achilles tendons in congenital clubfeet using ultrasonography before and after percutaneous tenotomy. METHODS: 15 boys and 12 girls (39 feet) aged one to 11 (mean, 5) months with idiopathic clubfeet underwent percutaneous tenotomy (Ponseti method). The size of each Achilles tendon was measured ultrasonographically by a single radiologist before and after surgery. In patients with unilateral clubfoot, the normal foot was used as a referent. Longitudinal scans assessed the echotexture, echogenicity, and continuity of the tendon. Transverse scans assessed the …

Medial patellofemoral ligament reconstruction using semitendinosus tendons: polyester suture augmentation versus nonaugmentation.

BACKGROUND: The comparative clinical outcome of medial patellofemoral ligament reconstruction (MPFLR) using semitendinosus tendons with and without polyester suture augmentation for recurrent patellar instability is unknown. HYPOTHESIS: Medial patellofemoral ligament reconstruction with polyester suture augmentation will yield better results than MPFLR without augmentation for recurrent patellar instability in adults. STUDY DESIGN: Randomized controlled trial; Level of evidence, 2. METHODS: One hundred patients with recurrent patellar instability receiving MPFLR using semitendinosus tendons…

Extensor hood syndrome--osteophytic irritation of digital extensor tendons in rock climbers.

OBJECTIVE: Injuries to the flexor tendons and flexor tendon pulleys are frequently reported in rock climbers. Osteoarthritic changes with bone spurs are also well known. We report on the less commonly described extensor tendon irritation caused by such osteophytes. METHODS: Thirteen high-level rock climbers (12 men, 1 woman; average age 33.8 years [range 17-55]; average years of climbing experience 19 [range 5-30]; average climbing level 10.2) with extensor hood irritation caused by dorsally located osteophytes of proximal interphalangeal (n = 10) or distal interphalangeal joints (n = 3) were…

[Flexor tendons involvement in gout. A case report and review of literature].

We present a case of synovial and flexor tendons gouty involvement associated to the flexion contracture of a single digit. The case was treated surgically by tenosynovectomy and resection of the flexor superficialis tendon followed by arthrolysis of the proximal interphalangeal joint. This rare etiology of flexion contracture must be taken into account in every patient with a history of gout and tendon pathology.

["Splint dishabituation" in treatment of distal injuries of the digital extensor tendons].

Results of treatment of 61 patients with "mallet finger" injuries have been analyzed. After discontinuation of permanent immobilization the patients of the main group (n = 20) underwent "splint dishabituation" consequentially and bit by bit during 8 weeks, according to an original protocol developed with special reference to the stages of biological process of union of the damaged tendon. Following permanent immobilization patients of the group of comparison (n = 35) were recommended to use a removable splint at night during two weeks. The proposed protocol of "splint dishabituation" improved

The management of concomitant tears of the peroneus longus and brevis tendons.

BACKGROUND: Concomitant chronic tears of both peroneal tendons rarely are reported. We present our experience in treating these injuries and suggest an algorithm for surgical treatment, determined by the presence of a functioning tendon or tendons, mobility of the remaining peroneal musculature, ankle stability, and position of the heel. METHODS: Twenty-eight consecutive patients (29 feet) were followed postoperatively for a mean of 4.6 (range 1.5 to 8) years. The mean age was 36 (range 19 to 54) years, and all patients had chronic ankle pain with or without instability and with combinations …

Thenar insertion of abductor pollicis longus accessory tendons and thumb carpometacarpal osteoarthritis.

Although the etiology of osteoarthritis of the thumb carpometacarpal (CMC) joint remains unclear, some theories have focused on variations in the local anatomy of the abductor pollicis longus tendon insertion. This cadaver study of 68 specimens analyzed the relationship between a thenar insertion of an accessory abductor pollicis longus tendon and the presence and severity of thumb CMC osteoarthritis. The joint cartilage surfaces were visually graded for degenerative changes. Thirty-five of 68 specimens (51%) had a thenar insertion, most frequently inserting on either the abductor pollicis bre

Biomechanical evaluation of wrist motor tendons after fractures of the distal radius.

We conducted a biomechanical study of changes in parameters of wrist motor tendons in fractures of the distal radius in 7 cadaveric extremities. Extra-articular distal radius fractures were simulated by distal radius osteotomy and fracture angulation was maintained by external fixators. Eight positions of the distal radius fractures were studied: dorsal angulation of 10 degrees, 20 degrees, 30 degrees, and 40 degrees and radial angulation of 5 degrees, 10 degrees, 15 degrees, and 20 degrees. Dorsal and radial angulation of the fractures were measured with respect to the shaft of the radius. Ex…

The Singapore operation for recurrent dislocation of peroneal tendons: long-term results.

Recurrent dislocation of peroneal tendons is uncommon and there are few reports of the long-term results after repair. The Singapore operation, first described in 1985, is an anatomical repair based on the Bankart-like lesion seen in the superior peroneal retinaculum. We reviewed 21 patients after a mean follow-up of 9.3 years, and found no recurrence. Eighteen had good functional results and had returned to their previous levels of vocational and sports activities. The three fair results were due to painful scars or neuromas.

Fibrocartilages in the extensor tendons of the interphalangeal joints of human toes.

The extensor tendons of the fingers and toes form part of the capsule of the interphalangeal joint and press against the proximal phalanx during flexion. Previous work on the fingers has shown that there is a "sesamoid" fibrocartilage on the deep surface of each tendon that labels immunohistochemically for a variety of glycosaminoglycans and collagens. However, we know little about the molecular composition of the tendon in the toes. This question is of special interest, because the mechanics of the interphalangeal joints differ in the upper and lower limbs-the toes balance the forefoot, distr…

[Treatment of complications of plastic surgery on the tendons of finger flexors].

From experience in the treatment of more than 800 patients by various methods of autotendoplasty which had been developed in the clinic earlier, the authors analysed the most characteristic complications of these operations. Separation of the wound edges and partial marginal necroses of the skin occurred in 7-8% of patients, mainly in bilateral damage to the neurovascular bundles of the fingers, ligature fistulas in 6-7%, ruptures in 5%, flexion contractures of the fingers formed in 2% of patients who had been operated on, suppuration of the wounds was practically not encountered. The peculiar

The reversed Esmarch tourniquet technique for the retrieval of cut flexor tendons.

The problem of retrieving a retracted proximal end in a zone II flexor tendon injury in the hand is a common one. We describe our experience of applying an Esmarch tourniquet from the elbow distally to retrieve the proximal end of the cut tendon. Our technique succeeded in presenting the proximal end through the wound in eight out of thirteen patients. This technique does not compromise the tendon, flexor sheath, the vinculae or the blood supply. It obviated the need for further incisions while keeping open other options for tendon retrieval.
